We have operated on 42 patients with bone metastasis from various primary tumors over the past fourteen years. The number of operated cases of metastasis secondary to thyroid and breast carcinoma was especially high, since patients with such types of cancer have prolonged life expectancies in general. The sites of these operation were as follows: 27 in the spine, 12 in the femur, 2 in the tibia, and 1 in the humerus.

BACKGROUND: The rising incidence of cancer has highlighted the need for effective treatment strategies, particularly for bone tumors like distal femoral tumors. These tumors are among the most prevalent types of bone malignancies, often necessitating extensive surgical intervention to achieve optimal outcomes. Limb-salvage surgery, which involves resecting the tumor and reconstructing the limb with prosthetic devices, has become the standard approach for managing both malignant and aggressive benign tumors of the distal femur. This technique has largely replaced amputation, allowing patients t

Managing chronic periprosthetic infections in patients who have undergone limb-salvage surgery following a malignant bone tumor with megaprosthesis often involves a two-stage revision surgery with the use of a cement-spacer. This paper show details the preparation of a self-made intramedullary metal-stabilized mega-cement spacer for patients needing a two-stage revision surgery due to infection after oncologic bone tumor resection and limb-salvage surgery with megaprosthesis and present 2clinical cases treated with this technique. The report provides a practical surgical technique to create a

CASE: A 64-year-old man suffered a pathologic left femoral neck fracture. Biopsy demonstrated metastatic urothelial cancer with a nonmuscle invasive bladder cancer primary confirmed by cystoscopy. The patient underwent hemiarthroplasty, chemotherapy, radiation, and eventually, a conversion to total hip arthroplasty. Today, over a decade from the initial surgery, the patient remains alive and highly functional. To our knowledge, this is the only report of bone metastatic bladder cancer with over 10-year survival.
CONCLUSION: Combined chemotherapy, radiation, and surgical resection of metastasi

BACKGROUND: Endoprosthetic replacement has become the mainstream method of reconstruction after tumor resection around the knee for decades, but there is a lack of comprehensive review evaluating the implant outcomes. We performed a systematic review to analyze the implant survival and complication profiles of distal femoral replacement (DFR) and proximal tibial replacement (PTR) in adults, and to evaluate the effects of different fixation methods and hinge mechanisms.
METHODS: A systematic review of 40 studies with 4748 DFR cases and 1713 PTR cases was performed after searching the PubMed an

PURPOSE: To report the use of a composite ceramic bone graft substitute containing calcium sulphate and hydroxyapatite (HA) in the treatment of large expansive osteolytic benign bone tumours.
METHODS: 4 women and 9 men aged 8 to 49 (mean, 22) years with aneurysmal bone cysts (n=6) or giant cell tumours (n=7) in the epi- or meta-physeal areas of the lower limbs underwent curettage, phenolisation, and filling with bone graft substitute containing calcium sulphate and HA. The mean tumour size was 38.5 (range, 18-65) ml. The patients were followed up for a mean of 41 (range, 33-52) months. Range

Colonic carcinoma metastasis in order of frequency to the liver, lung, bone and brain. Metastases elsewhere are unusual. We report a very rare case of metastatic spread to skeletal muscle and skin. There follows a review of the theories of anomalous metastatic localization. Inflammatory oncotaxis is offered as the most likely explanation of this phenomenon. The importance of good history taking in colorectal cancer follow up clinics is emphasised.

Three different types of operation were performed in 31 osteosarcoma patients, and some cases received chemotherapy. It revealed, wide resection of tumor supplemented with chemotherapy improved the survival and limb salvage rate enormously. Chemotherapy and wide resection were all absolutely essential. The life of prosthesis was long and most patients were satisfied with it. Chemotherapy and limb salvage can enhance the survival and life quality in patients with osteosarcoma.

OBJECTIVE: Granulocyte colony-stimulating factor (GCSF) is used to stimulate myeloid cell production and function in children undergoing chemotherapy for osteosarcoma. We hypothesize that GCSF can cause reconversion of marrow from fatty to hematopoietic and that this change can be detected by MR imaging at sites away from the primary tumor. This benign effect of treatment should not be confused with tumor spread.
MATERIALS AND METHODS: MR images of marrow of the affected and contralateral limbs were retrospectively reviewed for 16 patients with osteosarcoma of the femur or tibia; nine of thes

Dedifferentiated chondrosarcoma is an uncommon and aggressive variant of chondrosarcoma. The authors report the flow cytometric characteristics and cytogenetic findings in culture of two cases of dedifferentiated chondrosarcoma. The first case was DNA diploid by flow cytometry but had cytogenetic abnormalities consisting of breaks in the short arms of both chromosomes 1, resulting in deletion in one homolog and recombination in the other. In addition, cells from this tumor showed a balanced translocation between chromosomes 4 and 5, deletion of chromosome 9, and monosomy for chromosome 10. The

Orally administered phosphate supplements are the mainstay of therapy for hypophosphatemic osteomalacia of diverse causes and are generally believed to be free from harmful side effects. Two cases are reported, however, in which long-term therapy (14 and 10 years, respectively) resulted in hypercalcemic hyperparathyroidism associated with surgically proved adenomatous hyperplasia. This complication occurred despite concomitant treatment with pharmacologic doses of vitamin D. Thus, long-term oral phosphate therapy can produce tertiary hyperparathyroidism in susceptible patients.

The Anaesthesia and Recovery Service at the Galeazzi Orthopaedic Centre in Milan has an HOT unit where various cases of necrosis following radiotherapy for neoplasias have been and are still being treated. Some cases of radiodermitis and osteoradionecrosis only cured after HOT treatment are presented. The recovery of the scarring and granulation processes in these cases is attributable to the increased perfusion and above all diffusion of oxygen which activates the heating processes in the hypoxic tissues. An additional advantage of HOT is that patients can be treated in outpatients department

Osteosarcoma of bone is a recognizable entity if the histopathologist designates tumors as such when their malignatn cells produce osteoid substance even if only in small foci. Such definition distinguishes this lesion from other sarcomas that arise in bone, especially chondrosarcoma and fibrosarcoma. There is a general tendency to consider that osteosarcomas represent a stereotyped form of disease for which new modalities of treatment can be applied and assessed. The question of whether a given osseous lesion is actually malignant and not a benign neoplasm or even a reactive non-neoplastic co

BACKGROUND AND PURPOSE: Most lung cancer patients with skeletal metastases have a short survival and it is difficult to identify those patients who will benefit from palliative surgery. We report complication and survival rates in a consecutive series of lung cancer patients who were operated for symptomatic skeletal metastases.
METHODS: This study was based on data recorded in the Karolinska Skeletal Metastasis Register. The study period was 1987-2006. We identified 98 lung cancer patients (52 females). The median age at surgery was 62 (34-88) years. 78 lesions were located in the femur or s

Skeletal metastases disrupt bone remodeling, compromising mechanical integrity and increasing fracture risk. Cancer therapies can further influence bone quality. This study aimed to evaluate the impact of systemic cancer therapies (docetaxel (DTX) and zoledronic acid (ZA)) on bone quality in a preclinical model of mixed femoral metastases. Mixed metastases were induced in 5-6-week-old athymic male rats via intracardiac injection of luciferase-transfected ACE-1 canine prostate cancer cells (day 0). Healthy and inoculated rats were randomly assigned to receive no treatment, DTX (5 mg/kg), or ZA

This retrospective study aimed to evaluate patients with similar clinical and radiological features of diaphyseal intramedullary osteoid osteoma, an enigmatic location subtype. Sixteen patients (11 males and 5 females) with an average age of 12.3 years at the time of presentation were reviewed. The lesion was located in the tibia unilaterally in 10 patients, bilaterally in three, and in the femur unilaterally in three. Diagnosis was established based on the typical clinical presentation of OO and the identification of nidus on computed tomography (CT) scan and magnetic resonance imaging (MRI),
