Gymnastic wrist injuries.

During gymnastic activities, the wrist is exposed to many different types of stresses, including repetitive motion, high impact loading, axi … id stress reactions/fractures, capitate avascular necrosis, ganglia, carpal instability, triangular fibrocartilage complex tears, ulnar impaction syndrome, and lunotriquetral impingement. It is important to diagnose quickly and accurately the specific injury to initiate expediently the proper treatment and limit the extent of injury. In addition, a gymnast's tra …

Arthroscopic Evaluation of Associated Soft Tissue Injuries in Distal Radius Fractures.

The best outcome in distal radius fractures is achieved if anatomy is restored, in particular the intra-articular congruity. This is achieved partly with improved fixation, such as using volar locking plates, and partly by using an arthroscopy-assisted reduction and fixation technique. In addition to improving the intra-articular congruity, associated ligament and chondral injuries can be detected and treated. This article outlines various associated injuries with suggested management in a stepwise fashion. It is hoped that overall outcomes will be improved once patient-related and treatment-r

Navigating the Alphabet Soup of Labroligamentous Pathology of the Shoulder.

Because of the widespread use of eponyms and acronyms to describe labroligamentous findings in the shoulder, interpreting shoulder magnetic resonance imaging reports can be challenging. A summary of the appearance of these lesions on shoulder magnetic resonance images can help the orthopedic surgeon to understand these entities as imaging findings and to determine the appropriate treatment for patients with shoulder injuries.

Anatomy of the capsulolabral complex and rotator interval related to glenohumeral instability.

The glenohumeral joint with instability is a common diagnosis that often requires surgery. The aim of this review was to present an overview of the anatomy of the glenohumeral joint with emphasis on instability based on the current literature and to describe the detailed anatomy and anatomical variants of the glenohumeral joint associated with anterior and posterior shoulder instability. A review was performed using PubMed/MEDLINE using key words: Search terms were "glenohumeral", "shoulder instability", "cadaver", "rotator interval", "anatomy", and "anatomical study". During the last decade,

Glenoid labrum pathology.

The glenoid labrum is the fibrocartilage of the shoulder joint, anchoring the joint capsule and shoulder ligaments. Morphology varies regionally, especially in the superior and anterior region; these variants can sometimes be confused with pathological aspects. The labrum is often involved in shoulder pathology, by single trauma or, more often, repeated microtrauma. It seems logical to classify and to describe tears according to two criteria: the sector involved, and associated pain or instability. In the superior labrum, SLAP lesions are the most frequent. These combine labral lesion and lesi…

Labral and cartilage abnormalities in young patients with hip pain: accuracy of 3-Tesla indirect MR arthrography.

OBJECTIVE: Assess the diagnostic accuracy of 3-T indirect magnetic resonance arthrography (iMRA) for hip cartilage and labral pathology detection using arthroscopy as the reference standard and compare it to the published performance of direct magnetic resonance arthrography (dMRA). MATERIALS AND METHODS: Between 2009 and 2011, 290 patients suspected of having femoroacetabular impingement underwent iMRA. Our study group consisted of 41 of these patients (17 males, mean age 35 years; 24 females, mean age 33 years) who did not have a prior history of hip surgery and who subsequently underwent a

Distal radioulnar joint kinematics in simulated dorsally angulated distal radius fractures.

PURPOSE: To examine the effects of dorsal angulation deformities of the distal radius with and without triangular fibrocartilage complex (TFCC) rupture on the 3-dimensional kinematics of the distal radioulnar joint (DRUJ) during simulated active motion. METHODS: Nine fresh-frozen cadaveric specimens were tested in a forearm simulator that produced active forearm rotation. Dorsal angulation deformities of the distal radius with 10°, 20°, and 30° angulation were created. Changes in the position of the ulna relative to the radius at the DRUJ as a consequence of each dorsal angulation deformity w…

Flouroscopic arthrography versus MR arthrography of the lesser metatarsophalangeal joints for the detection of tears of the plantar plate and joint capsule: a prospective comparative study.

BACKGROUND: Derangements of the plantar plate and joint capsule are an underrecognized cause of lesser metatarsalgia. Fluoroscopic arthrography and magnetic resonance (MR) arthrography are both used for diagnosis. Currently there are no studies comparing the effectiveness of these two modalities. METHODS: Patients suspected of having plantar plate or capsular tears underwent both fluoroscopic arthrography and MR arthrography; the imaging findings were then compared and correlated with intraoperative findings, when available, to evaluate the effectiveness of the different imaging modalities. Intrafocal pin plate fixation of distal ulna fractures associated with distal radius fractures.

Subcapital ulnar fractures in association with distal radius fractures in elderly patients increase instability and pose a treatment challenge. Fixation of the ulnar fracture with traditional implants is difficult due to the subcutaneous location, comminution, and osteoporosis. We describe an intrafocal pin plate that provides fixation by a locking plate on the distal ulna and intramedullary fixation within the shaft. The low profile and percutaneous technique make this device a useful alternative for treatment of subcapital ulna fractures in the elderly.

An unusual case of enthesophyte formation following an adductor longus rupture in a high-level athlete.

We present a case of a high-level rugby player with severe groin pain following a partial rupture of his left adductor longus enthesis during a game. Conservative treatment proved unsuccessful and the athlete had persistent symptoms, affecting his quality of life and ability to play sports. Further assessments revealed a large bony spur/enthesophyte at adductor longus origin. The patient underwent a successful surgical resection of the active bone formation.

Volar Distal Radioulnar Joint Dislocation With Ulnar Styloid Fracture.

Volar dislocation of the distal radioulnar joint (DRUJ) is an uncommon traumatic injury that may be overlooked due to subtle clinical findings and difficulty obtaining proper radiographs in the acute setting. The mechanism typically involves a strong pronation force applied to a fixed wrist, leading to disruption of the stabilizing soft tissue structures, particularly the triangular fibrocartilage complex (TFCC). Although standard wrist radiographs are recommended initially, computed tomography (CT) is often required when positioning is inadequate or the diagnosis remains uncertain. We report …

Double lesion of TFCC injury from penetrating wrist trauma: A case report.

Bony avulsion injuries of the triangular fibrocartilage complex (TFCC) caused by penetrating wrist trauma are rare, and there is limited literature available regarding their treatment. This case report describes a 54-year-old male who sustained a penetrating injury to his right wrist, resulting in "double lesion" of TFCC injury, which are ulnar avulsion (Palmer 1B) and radial avulsion (Palmer 1D) lesion with a sigmoid notch fracture. The patient underwent an arthroscopic examination and a novel arthroscopic technique for fixation and repair of the TFCC injury. The procedure involved debridemen…

Ulnocarpal Abutment Syndrome With Triquetrum Degeneration.

We present the case of a 47-year-old patient with a congenital positive ulnar variance and elucidate its effects on nearby structures in relation to ulnocarpal abutment syndrome (UAS). While magnetic resonance imaging (MRI) helped to identify soft tissue changes in the wrist, the use of an arthrogram, in this case, allowed for a more comprehensive and detailed analysis of the ligaments and soft tissues. Image findings included a complex degenerative tear of the disc of the triangular fibrocartilage (TFCC), a degenerated triquetrum, and partial tears of the scapholunate and lunotriquetral ligam…

Subungual Osteochondroma of the Great Toe: A Case Report.

Bony outgrowths of the distal phalanx of the great toe have been described in the literature but rarely. These subungual bony outgrowths can be caused by subungual exostosis or subungual osteochondromas. Both of these abnormalities are bony outgrowths with differences in the cartilage cap wherein the exostoses have fibrocartilage, and osteochondromas have hyaline cartilage. The subungual exostosis and osteochondroma that are protruding present symptoms of pain, redness, and deformed nail bed, whereas the nonprotruding osteochondromas have only a lump as the presenting symptom. In both conditio…

Mechanical stress can regulate temporomandibular joint cavitation via signalling pathways.

The temporomandibular joint (TMJ), composed of temporal fossa, mandibular condyle and a fibrocartilage disc with upper and lower cavities, is the biggest synovial joint and biomechanical hinge of the craniomaxillofacial musculoskeletal system. The initial events that give rise to TMJ cavities across diverse species are not fully understood. Most studies focus on the pivotal role of molecules such as Indian hedgehog (Ihh) and hyaluronic acid (HA) in TMJ cavitation. Although biologists have observed that mechanical stress plays an irreplaceable role in the development of biological tissues and o…

An Updated View on Temporomandibular Joint Degeneration: Insights from the Cell Subsets of Mandibular Condylar Cartilage.

The high prevalence of temporomandibular joint osteoarthritis (TMJOA), which causes joint dysfunction, indicates the need for more effective methods for treatment and repair. Mandibular condylar cartilage (MCC), a typical fibrocartilage that experiences degenerative changes during the development of TMJOA, has become a research focus and therapeutic target in recent years. MCC is composed of four zones of cells at various stages of differentiation. The cell subsets in MCC exhibit different physiological and pathological characteristics during development and in TMJOA. Most studies of TMJOA are…
