Scaling up antenatal multiple micronutrient supplementation in Africa: A unified call for action.

Maternal undernutrition in Africa remains a public health challenge, contributing to negative pregnancy outcomes, neonatal mortality, and perpetuating intergenerational cycles of poor health. Antenatal multiple micronutrient supplementation (MMS), a cost-effective intervention recognized for its potential to improve maternal and neonatal health, reduces risks of low birth weight, preterm birth, small for gestational age, and stillbirth while offering a $37 return for every $1 invested. Despite its benefits, MMS adoption across African countries remains suboptimal. This position paper synthesiz

Health System Factors Influencing Availability and Access to Comprehensive Care for Cleft Lip, Palate, and Craniofacial Anomalies in Dar es Salaam, Tanzania.

BACKGROUND: Cleft lip and palate are common craniofacial anomalies. In low- and middle-income countries, patients with these conditions face barriers to comprehensive care. In Tanzania, despite some hospitals performing surgical procedures, the health system factors affecting access to full cleft care remain unclear. This study aimed to assess the health system factors influencing the availability and access to comprehensive care for cleft lip, palate, and associated craniofacial anomalies in Tanzania. METHODS: A multicenter cross-sectional study was conducted across 37 health facilities sele

Building functional and sustainable pharmacovigilance systems: an analysis of pharmacovigilance development across high-, middle- and low-income countries.

BACKGROUND: There has been notable progress in pharmacovigilance (PV) in low- and middle-income countries (LMIC) in the last decade. However, only a few of these PV systems are fully functional, unlike in high-income countries where stringent legislation, regulations and operational guidelines have enabled the establishment of effective PV systems. The key challenges faced in LMIC include organisational inefficiencies; weak infrastructure; inconsistent and poorly enforced regulations; and inadequate financing and shortage of trained personnel. Furthermore, low adverse event volume and poor dat

The fall and rise of cost sharing in Kenya: the impact of phased implementation.

The combined effects of increasing demand for health services and declining real public resources have recently led many governments in the developing world to explore various health financing alternatives. Faced with a significant decline during the 1980s in its real per capita expenditures, the Kenya Ministry of Health (MOH) introduced a new cost sharing programme in December 1989. The programme was part of a comprehensive health financing strategy which also included social insurance, efficiency measures, and private sector development. Early implementation problems led to the suspension in

Malaria Vaccine Rollout in Nigeria: Current Status and Challenges.

Malaria remains a leading cause of morbidity and mortality in Nigeria, particularly among children under 5 years of age, despite ongoing control efforts. The introduction of RTS,S/AS01, and R21/Matrix-M vaccines provides a promising complementary prevention strategy. This systematic review assessed malaria vaccine rollout in Nigeria, focusing on implementation, awareness, acceptance, willingness to pay, and health system readiness. A comprehensive search of PubMed, ScienceDirect, Google Scholar, African Index Medicus, and gray literature (2015-2026) identified 12 eligible studies. Caregiver ac

Medicaid and the Financing of Pediatric Healthcare: Strengths and Opportunities.

Medicaid provides health insurance coverage for nearly half of all US children, offering broad service coverage for low-income individuals with minimal waiting periods and copays. The Children's Health Insurance Program (CHIP) insures children from households with incomes above Medicaid thresholds, but with less comprehensive benefits. Both Medicaid and CHIP typically reimburse at about 70% of Medicare payment rates. Unlike Medicare, which has full federal funding, Medicaid is a federal-state partnership. States receive 50-77% of their total Medicaid budgets from federal sources but retain adm

Examining Integration of Comprehensive Substance Use Services in Certified Community Behavioral Health Clinics: A Cross-sectional Study.

Despite policy efforts to expand integrated behavioral health care in the United States, substantial gaps remain in service delivery for people with co-occurring mental health (MH) and substance use disorders (SUD). Certified Community Behavioral Health Clinics (CCBHCs) are intended to close these gaps through enhanced financing and certification standards that mandate comprehensive, integrated MH-SUD services. Using the 2023 National Survey of Substance Use and Mental Health Services (N-SUMHSS) data for 1,480 facilities (198 CCBHCs, 448 community mental health centers [CMHCs], and 834 other o

Water, sanitation, and hygiene (WASH) practices in Africa: exploring the effects on public health and sustainable development plans.

BACKGROUND: Suboptimal water, sanitation, and hygiene (WASH) practices constitute a serious public health risk, affecting one-third of the world's population. Remarkable progress has been made to improve WASH; however, challenges remain, with rapid population growth adding pressure on WASH systems. This study explores the current state of WASH practices and diseases in Africa, identifies challenges, and proposes public health recommendations for sustainable implementation. MAIN BODY: The staggering burden of WASH-related diseases in low- and middle-income countries (LMICs), particularly in Af

Impact of state vaccine financing policy on uptake of heptavalent pneumococcal conjugate vaccine.

OBJECTIVE: We examined heptavalent pneumococcal conjugate vaccine (PCV7) uptake among children aged 19 to 35 months in the United States and determined how uptake rates differed by state vaccine financing policy. METHODS: We analyzed data from the 2001-2003 National Immunization Survey. States that changed their vaccine financing policy between 2001 and 2003 (n=17) were excluded from analysis. Logistic regression was performed to identify the association between state vaccine financing policy and receipt of 3 or more doses of PCV7 after control for demographic characteristics. RESULTS: The p

Applying the World Report on Disability to children's communication.

PURPOSE: The World Report on Disability is an important milestone in the recognition of people with disabilities; however, the Report acknowledges that people with communication difficulties may be underrepresented in estimates of disability. Consequently, this article applies the nine recommendations from the World Report on Disability to supporting children's communication skills. METHOD: Australia is similar to most Minority World countries since it places high regard on articulate and literate communication. Recent large-scale Australian studies of children with speech, language and commu

Impact of the COVID-19 Pandemic on Tobacco Sales and National Smoking Cessation Services in Korea.

This study aimed to describe the effect of the COVID-19 pandemic, combined with the Korean government's response to the pandemic on tobacco consumption and national smoking cessation services among the Korean population. We obtained tobacco sale data from the Ministry of Finance and analysed the data on smokers' visits to national smoking cessation clinics during the pandemic from a member of the National Assembly. We also conducted an online search to understand smokers' thoughts about their tobacco use during the pandemic. We found that after the emergence of COVID-19 in 2020, the sale of co

Factors associated with the decline of malaria in Myanmar's Ayeyarwady Region between 2013 and 2017.

The burden of malaria in Myanmar has declined rapidly in recent years; cases decreased from 333,871 in 2013 to 85,019 in 2017 (75% decrease). Decline of malaria in the Ayeyarwady Region of Myanmar reflects this trend with an 86% decrease in cases over this period. In this exploratory analysis, quantitative and qualitative information were assessed to explore potential factors responsible for the decline of malaria in Ayeyarwady. Data on malaria incidence, programmatic financing, surveillance, case management, vector control interventions, climate and ecological factors, and policies and guidel

Can mothers afford maternal health care costs? User costs of maternity services in rural Tanzania.

Following the difficult economic situation various countries introduced health sector reforms, including user charges to finance the system. The assessment of user costs for maternity services in Tanzania was part of a larger study, which covered inputs, outputs and efficiency of services. The study was carried out from October 1997 to January 1998 in Mtwara urban and rural district in South Tanzania. One hundred and seven women attending a quarter of government health facilities were randomly selected and interviewed. Twenty one key informants were also interviewed and service procedures obse

Is the money following the clients with learning disabilities?

For the past decade patients with learning disabilities living in long stay mental handicap hospitals have been resettled in the community. Local authorities have also taken on the care of new patients who would once have been long stay residents. The imperfect data that are available suggest that in England about half the residents in mental handicap hospitals in 1981 are now the responsibility of local authorities; the figures for Wales and Northern Ireland are 38% and 33%. Data on revenue suggest that the savings to the health service are much less--perhaps 9% in Northern Ireland and 3.6% i

Clinical characteristics and risk factors for severe disease among hospitalized patients with COVID-19 in Japan after cessation of public funding: A single-center retrospective study in 2024-2025.

Although the mortality rate of COVID-19 is declining, disease threats remain, particularly in older individuals and those with underlying conditions. Furthermore, there are concerns about the decrease in antiviral prescriptions following public funding cessation for COVID-19 treatment in Japan. In this study, we aimed to investigate patient characteristics, pre-admission COVID-19 diagnosis, treatment status, severity, and risk factors for severe disease and mortality among hospitalized patients with COVID-19 after ending public funding. This single-center retrospective study, conducted at the

Financial burden of survivors of medically-managed myocardial infarction and its association with selected social determinants and quality of life in a lower middle income country.

BACKGROUND: Burden from ischemic heart disease is rising in Sri Lanka due to the demographic and epidemiological transitions. Documented literature is scarce on quality of life, financial burden and its determinants in relation to myocardial infarction (MI). This study was done to describe the financial burden among the survivors of MI managed only with drugs (i.e. those who did not undergo Percutaneous Coronary Intervention or Coronary Artery Bypass Graft) and its association with selected social determinants (SDHs) and quality of life (QOL).
