Paradoxical extension was first described, and termed "lumbrical-plus" finger, by Parkes in 1970. It is characterized by extension of the interphalangeal joints when flexing the fingers. The It is most common in the middle finger. Treatment involves lumbrical muscle release or a flexor digitorum profundus graft of appropriate length. A search of the literature found no reports of results of conservative management. The present case report describes a novel conservative technique for lumbrical-plus deformity.

Mallet finger injuries are a common presentation in the emergency department. These injuries result from a forced flexion at the distal interphalangeal joint (DIPJ) that causes extensor tendon disruption, and possibly bone avulsion, at the base of the distal phalanx. This article describes the anatomy, mechanisms, classification and assessment of mallet finger injury. It reviews different types of splints used in the conservative management of closed tendinous mallet finger injury and discusses the latest evidence regarding immobilisation methods and treatment duration. Maintaining the DIPJ in

In brief: Mallet finger is common among athletes who play contact sports. This deformity results from the loss of extensor function of the distal interphalangeal joint. Most sports injuries of this type can be treated conservatively by splinting. Patients usually wear the splint for eight weeks or more; they must remove it to clean the finger every few days, making sure that the finger does not bend while the splint is off. Surgery should be reserved for deformities resulting from a fracture with a subluxated palmar fragment, and then only when conservative treatment has I failed.

INTRODUCTION: Distally based dorsal metacarpal artery (DMCA) flaps are an established technique for the reconstruction of extensive finger defects. In many cases, such defects also include an injury of the extension tendon over the proximal and distal finger joint, which can lead to a reduced range of motion or finger deformation such as boutonniére deformity. To prevent this, operative techniques are necessary that allow complete defect coverage while simultaneously stabilizing the extension apparatus.
METHODS: In two cases, DMCA flaps were combined with vascularized extensor indices tendons…

BACKGROUND: Traditional transarticular distal interphalangeal joint (DIPJ) pinning for bony mallet fingers risks stiffness and iatrogenic cartilage injury. This study compared a modified non-transarticular K-wire external fixator with traditional transarticular pinning for surgically treated bony mallet finger.
METHODS: This single-center prospective nonrandomized comparative cohort study included 120 adults with acute bony mallet finger. Sixty patients underwent traditional transarticular DIPJ pinning, and 60 underwent modified non-transarticular K-wire external fixation. The primary outcome

BACKGROUND: Compliant pneumatic actuators possess many characteristics that are desirable for wearable robotic systems. These actuators can be lightweight, integrated with clothing, and accommodate uncontrolled degrees of freedom. These attributes are especially desirable for hand exoskeletons, where the soft actuator can conform to the highly variable digit shape. In particular, locating the pneumatic actuator on the palmar side of the digit may have benefits for assisting finger extension and resisting unwanted finger flexion, but this configuration requires suppleness to allow digit flexion

BACKGROUND: Current approaches to characterizing deficits in upper limb movements after stroke typically focus either on changes in a functional measure, for example, how well a patient can complete a task, or changes in impairment, for example, isolated measurements of joint range of motion. However, there can be notable dissociations between static measures of impairment versus those of function.
OBJECTIVE: We develop a method to measure upper limb joint angles during performance of a functional task and use measurements to characterize joint impairment in the context of a functional task.

Ulnar deviation of fingers in rheumatoid arthritis is a common deformity that can markedly compromise hand function. These deformities also result in an aesthetic impairment that is often troubling to patients. A variety of surgical procedures have been recommended to correct the ulnar deviation, either at the wrist level or at the metacarpophalangeal (MCP) joints. We present a surgical technique that uses the extensor indicis proprius as a tenodesis across the dorsum of the MCP joints of the fingers. The technique can also be used in conjunction with other surgical procedures, including prost

QUESTIONS: In case of Heberden's nodes the osteoarthritis starts with a so called tidemark flaking as a reaction to a subchondral ossification. The question was, if a differentiation in relation to a control group by an objective score is possible.
METHODS: 218 finger joints from 56 cadavers were investigated morphologically (score).
RESULTS: The osteoarthritis starts with a subchondral ossification. At this time the surface of the cartilage is not destroyed yet. Reactive tidemark flaking is the beginning of the general degradation. In relation to a control group a significant differentiatio…

Accidental self-injection injury is a common occurrence among veterinary and farm workers handling automatic syringe injectors. Most of the time, these injuries are asymptomatic or cause self-resolving mild symptoms, but these injuries may lead to significant morbidity. The aim of the study was to evaluate hand function after inadvertent injection of a poultry influenza or cholera vaccine in patients admitted to our department with infection. We retrospectively gathered data from admission to last follow-up. Functional assessment and physical exam of the hand were done at each stage by either

Contractures of the intrinsic muscles of the fingers disrupt the delicate and complex balance of intrinsic and extrinsic muscles, which allows the hand to be so versatile and functional. The loss of muscle function primarily affects the interphalangeal joints but also may affect etacarpophalangeal joints. The resulting clinical picture is often termed, intrinsic contracture or intrinsic-plus hand. Disruption of the balance between intrinsic and extrinsic muscles has many causes and may be secondary to changes within the intrinsic musculature or the tendon unit. This article reviews diagnosis,

PURPOSE: Complete release of the A4 pulley is recommended for flexor tendon repair to allow smooth tendon excursion, provided the proximal sheath is mostly intact. However, it is unclear whether the active range of motion of finger joints differs when the flexor digitorum profundus tendon is repaired at different levels with A4 pulley release. We evaluated differences in the outcomes of flexor tendon repair of zone 1 and zone 2A injuries using a six-strand suture (the Yoshizu #1 technique) and complete release of the A4 pulley, followed by an early active mobilization protocol.
METHODS: This …

OBJECTIVE: High-resolution images of finger joints with chemical-shift elimination can be obtained using an interleaved water-fat (IWF) sequence. This study assessed IWF imaging of finger joints in the delineation of bone structures by comparing images of cadaver fingers with those of microcomputed tomography (CT) that served as a standard reference.
MATERIALS AND METHODS: IWF images with spatial resolution of 176 µ × 176 µ × 300 µ were obtained from the distal and proximal interphalangeal joints of two cadaver finger specimens using a custom-built radiofrequency receive coil at 1.5T. Regular…

Arthritis is an inflammatory condition associated with joint damage, the incidence of which is increasing worldwide. In severe cases, arthritis can result in the restriction of joint movement, thereby affecting daily activities; as such, early and accurate diagnosis crucial to ensure effective treatment and management. Advances in imaging technologies used for arthritis diagnosis, particularly Single Photon Emission Computed Tomography/Computed Tomography (SPECT/CT), have enabled the quantitative measurement of joint inflammation using SUVmax. To the best of our knowledge, this is the first st
