Positron Emission Tomography/Computed Tomography in Bladder Cancer: The Role of [18F]FDG and Non-FDG Radiotracers.

Background and Objectives: Bladder cancer is one of the most common malignancies of the urinary tract and poses a significant clinical challenge due to its biological heterogeneity and high rates of recurrence and progression. Urothelial carcinoma represents the predominant histological subtype, ranging from non-muscle-invasive disease with relatively favorable outcomes to aggressive muscle-invasive and metastatic forms associated with poor prognosis. Accurate diagnosis, staging, prognostic stratification, and assessment of treatment response are therefore essential for optimal patient managem

18F-FDG PET/CT for post-cardiac-arrest brain injury: current evidence and future directions.

Post-cardiac-arrest brain injury is a major cause of death and long-term neurological disability after cardiac arrest, yet current prognostic tools provide limited information on regional cerebral metabolism and functional recovery. Fluorine-18 fluorodeoxyglucose positron emission tomography and positron emission tomography/computed tomography (18F-FDG PET and PET/CT) provide a molecular imaging approach for evaluating cerebral glucose metabolism after global ischemia-reperfusion injury. This narrative review summarizes the limited preclinical and clinical evidence on the use of 18F-FDG PET an

Increased Activity of the Pancreatic Infarcts Mimicking Malignancy on FDG PET/CT.

Parenchymal infarction in organs may cause increased FDG activity mimicking malignancy. We describe contrast-enhanced CT and FDG PET/CT findings in a case of multiple iatrogenic pancreatic infarcts after transcatheter bronchial artery embolization with gelatin sponge particles for the treatment of hemoptysis caused by lung cancer. The pancreatic infarcts were located at the periphery of the pancreas and showed hypovasculariy on postcontrast CT and hypermetabolism on FDG PET/CT, mimicking malignancy. This case indicates that pancreatic infarcts should be included in the differential diagnosis o

Persistent Müllerian Duct Syndrome on 18 F-FDG PET/CT.

A 32-year-old man presenting with limb weakness was diagnosed with Guillain-Barré syndrome. Abdominal distension was noted, and imaging revealed a large, heterogeneously enhancing abdominopelvic mass with bilateral cryptorchidism and uterine-like tissue. The mass showed high FDG uptake on 18 F-FDG PET/CT, while the uterine-like tissue did not. Following surgical biopsy, histopathologic examination demonstrated seminoma and rudimentary uterine tissue, confirming persistent Müllerian duct syndrome (PMDS). PMDS is a rare autosomal recessive disorder characterized by male phenotype with cryptorchi

Deep learning-guided late-phase dynamic frame prediction for accelerated Patlak analysis in 18F-FDG-PET brain imaging.

PURPOSE: Dynamic 18F-FDG-PET enables the quantitative assessment of cerebral glucose metabolism but requires prolonged acquisition times, which pose challenges to patients presenting with neurodegenerative disorders. This study aims to use deep learning to reduce dynamic FDG-PET scan duration while preserving quantitative kinetic modeling accuracy by predicting late-phase PET frames from intermediate-phase data. METHODS: A deep learning framework based on a 3D U-Net was developed to generate the final six late-phase dynamic PET frames (frames 47-52) using intermediate-phase frames (frames 34-

Semi-Automated Quantification of 18F-FDG PET-CT in Pulmonary TB Contacts and Its Association with Prospective Outcomes.

Positron emission tomography-computed tomography (PET-CT) has shown potential as a research tool to characterize heterogeneity in tuberculosis (TB) infection. Prospective utilisation of this technology will require standardization of imaging protocols, interpretation, and analysis of PET-CT images to improve generalizability and data assimilation between discrete cohorts. We evaluated a semi-automated approach using open-source software to minimize inter-operator variability of imaging interpretation and use reference organ normalization to lower variability in physiological uptake. We quantif

PET/CT: Interpretative Pitfalls in the Thorax.

In PET/CT evaluation of thoracic malignancies, knowledge of common pitfalls and artifacts that can mimic pathology is important to avoid misinterpretation. These pitfalls include physiologic uptake in striated muscle or brown fat, malignancies that are not [18F]-fluoro-2-deoxy-d-glucose (FDG) avid, and FDG avid benign processes. Artifacts include misregistration of PET and CT images due to cardiac or respiratory motion, and high attenuation materials on CT. Specific protocols for patient preparation and imaging acquisition, review of both the attenuation-corrected and nonattenuation-corrected

18 F-FDG PET/CT Findings Mimicking Gallbladder Metastasis From a Rectal Adenocarcinoma: Rare Synchronous Gallbladder and Rectal Cancer.

Metastasis from rectal adenocarcinoma (RAC) to the gallbladder (GB) is rare. Differentiating GB metastasis from primary gallbladder cancer (GBC) is important for treatment planning. A patient diagnosed RAC with 18 F-FDG PET/CT found suspicious findings of GB metastasis. However, after surgery was found to be primary GBC. The incidence of multiple primary tumors (MPTs) varies 2.4%-8%. In this case report, we would like to present 18 F-FDG PET/CT findings of rare synchronous coexistence of GBC and RAC, and also that, although rare, metastasis from the RAC to the GB should definitely be kept in m

A rare case of follicular thyroid carcinoma with unknown primary tumor location.

The most common follicular cell-derived malignant neoplasm of the thyroid is papillary thyroid carcinoma (PTC), which typically demonstrates early lymphatic dissemination. The second most common - follicular thyroid carcinoma (FTC), primarily spreads via hematogenous routes, often resulting in distant metastases, most commonly in the lungs and bones. The standard diagnostic and therapeutic process of patients with differentiated thyroid carcinoma (DTC) includes ultrasound examination followed by fine needle biopsy (FNB), subsequent thyroidectomy, and postoperative radioiodine (RAI) treatment.

Primary Prostatic Synovial Sarcoma With Intravenous Tumor Thrombus Presented on 18 F-FDG and Al 18 F-Thretide PET/CT.

A 35-year-old man with painful and difficult urination underwent 18 F-FDG and Al 18 F-Thretide PET/CT scans after a pelvic MRI suggested a prostate mass. The 18 F-FDG and Al 18 F-Thretide PET/CT scans revealed a prostate mass accompanied by an intravenous tumor thrombus. Both the prostate mass and the intravenous tumor thrombus demonstrated increased FDG uptake. Intense PSMA activity was observed in both lesions, with the intravenous tumor thrombus showing considerably higher PSMA activity compared with its FDG uptake. Histologic analysis of the biopsy confirmed the diagnosis of prostatic syno

Ascites in Breast Cancer: Progression from Negative Cytology to Avid Peritoneal Involvement.

Malignant ascites is caused by various factors such as lymphatic obstruction, increased vascular permeability and immune response. It presents a significant clinical challenge to the treating physicians considering limited treatment options resulting in increased morbidity and mortality. It is mostly associated with gastrointestinal, gynaecological, breast, hepatobiliary, and lung carcinomas. Paracentesis is usually important in determining the quantity and etiology of ascites. However, unlike conventional radiological imaging, 18F-FDG PET-CT can act as an early pointer towards its diagnosis.

Synchronous Isolated Ovarian Metastasis From Endometrioid Endometrial Carcinoma on FDG PET/CT.

Women with ovarian metastasis from endometrial cancer are more likely to have >50% myometrial invasion, high-grade non-endometrioid tumors, and lymphovascular space invasion. Low-grade endometrioid endometrial cancers with <50% myometrial invasion and negative lymphovascular space invasion have a very low rate (0.5%) of ovarian metastasis. We describe FDG PET/CT findings in a case of low-grade endometrioid endometrial cancer with <50% myometrial invasion, negative lymphovascular space invasion, negative lymph node metastasis, and synchronous isolated unilateral small ovarian metastasis. Imaging Features of Anal Carcinoma after Chemoradiation.

Anal cancer is a rare malignancy that is primarily treated with chemoradiation therapy (CRT). Clinical examination of the anal canal after CRT is often limited owing to the patient's discomfort. Therefore, radiologic surveillance plays a fundamental role in treatment response assessment. Currently recommended imaging modalities for posttreatment follow-up include pelvic MRI for local response evaluation and CT for evaluation of possible distant metastases. Patients who demonstrate a complete treatment response undergo regular clinical and imaging surveillance. Cases demonstrating an equivocal

The role of 18F-FDG PET/CT in adrenocortical carcinoma: a systematic review.

Adrenocortical carcinoma (ACC) is a rare and highly aggressive carcinoma with a poor prognosis. The aim of our study is to summarize existing evidence on the potential usefulness of fluorine-18-fluorodeoxyglucose positron/computed tomography (18F-FDG PET/CT) in the management of patients affected with ACC. The current systematic review was registered to the PROSPERO registry (ID1103377). A comprehensive search of the PubMed/MEDLINE, Embase, and Cochrane Library databases was conducted until July 2025. A total of 19 studies that evaluated the role of 18F-FDG PET/CT in ACC were included. One of

Top 7 Diagnostic Dilemmas in Cardiac Sarcoidosis: Lessons from Clinical Practice.

Cardiac sarcoidosis (CS) is a potentially fatal but often underrecognized manifestation of systemic sarcoidosis. Its diagnosis remains a major clinical challenge due to nonspecific symptoms, overlapping features with other cardiac and genetic diseases, and the lack of a universal diagnostic gold standard. This review outlines seven key diagnostic dilemmas frequently encountered in clinical practice: (1) failure to consider CS, (2) premature attribution of findings to CS, (3) limitations and misinterpretation of echocardiogram and cardiac magnetic resonance imaging, (4) limitations of 18F-FDG p

FDG PET/CT in a Case of Caplan Syndrome.

Caplan syndrome is a rare disease characterized by pulmonary nodules in patients with rheumatoid arthritis with a history of exposure to inhaled inorganic dust particles. We describe FDG PET/CT findings and evolution of CT findings in a case of Caplan syndrome. FDG PET/CT showed multiple consolidations with moderate activity in both upper lobes, numerous small nodules with mild activity in both lungs, and multiple FDG-avid lymph nodes in the mediastinum and bilateral hila and axillae. Follow-up CT showed confluence of the consolidations in both upper lobes and emergence of multiple peripherall

Cerebral metabolic covariance in delirium: pattern response to symptomatic changes.

BACKGROUND: Delirium is an acute neuropsychiatric condition linked to increased dementia risk, yet its mechanisms remain unclear. Previous studies reported cerebral hypometabolism. Spatial covariance of 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET) was applied to acutely unwell inpatients with and without delirium (Delirium, Conunwell) to derive a delirium-specific metabolic pattern (DP). DP expression was evaluated in healthy older adults (Conhealthy) and patients with delirium superimposed on dementia (DSD) and tracked longitudinally with symptom resolution. METHODS: Seven

Demonstration of in-vivo simultaneous 3D imaging with 18F-FDG and Na131I using Compton-PET system.

Simultaneous imaging of the SPECT tracer 131I and PET tracer 18F is important in the diagnosis of high- and low-grade thyroid cancers because high-grade thyroid cancers have high 18F-FDG and low 131I uptake, while low-grade thyroid cancers have high 131I and low 18F-FDG uptake. In this study, Na131I and 18F-FDG were simultaneously imaged using the Compton-PET system, in vivo. The angular resolution and sensitivity of the Compton camera with 356 keV gamma ray measured using a 133Ba point source were 12.3° and 2 × 10-5, respectively. The spatial resolution and sensitivity of PET were measured wi
