Rigid internal fixation of displaced distal radius fractures.

Distal radius fractures, the most common long bone fracture, are treated in several ways, including closed reduction, percutaneous pinning, external fixation, and open reduction and internal fixation. This article presents a surgical technique and a series of patients treated with a novel minimally invasive intramedullary fixation technique. The implant is a partially flexible intramedullary rod that can be locked in a rigid position once it is implanted in the bone. An awl and reamer are passed through a starter hole in the radial styloid using a 2-cm incision between the 1st and 2nd dorsal c

Intramedullary nailing of lateral malleolus in ankle fractures - surgical technique and literature review.

Our objective is to describe the technique of intramedullary (IM) nailing of lateral malleolus in the surgical management of ankle fractures. Fracture reduction is performed either percutaneously with a small pointed reduction clamp in simple oblique fractures or using longitudinal traction and rotation for comminuted fractures, thus reducing complications related to open reduction and internal fixation with a plate. The technique has been shown to be simple and reproducible. In addition, the technique allows early weight bearing, which accelerates rehabilitation and potentially fasten fractur

Intrafocal pin plate fixation of distal ulna fractures associated with distal radius fractures.

Subcapital ulnar fractures in association with distal radius fractures in elderly patients increase instability and pose a treatment challenge. Fixation of the ulnar fracture with traditional implants is difficult due to the subcutaneous location, comminution, and osteoporosis. We describe an intrafocal pin plate that provides fixation by a locking plate on the distal ulna and intramedullary fixation within the shaft. The low profile and percutaneous technique make this device a useful alternative for treatment of subcapital ulna fractures in the elderly.

The Treatment of Periprosthetic Distal Femoral Fractures After Total Knee Replacement: A Critical Analysis Review.

Periprosthetic distal femoral fracture after total knee arthroplasty carries substantial morbidity and mortality regardless of fixation technique. Surgical treatment is favored in most patients compared with conservative therapy because of high rates of nonunion, malunion, and reoperation after casting or bracing. Internal fixation techniques including retrograde intramedullary nailing and locked plating are favored for surgical treatment in most fractures when bone stock in the distal fragment allows for appropriate fixation. In the setting of deficient distal femoral bone stock or femoral co

Single-Bone Intramedullary Nailing of Pediatric Both-Bone Forearm Fractures A Systematic Review.

Traditional operative management of unstable, pediatric both-bone forearm fractures is fixation of both ulna and radius. Literature suggests single-bone fixation with intramedullary nailing obtains good results and is less technically demanding and invasive. This systematic review evaluates the efficacy of single-bone intramedullary nailing of pediatric both-bone forearm fractures. Medline and Embase were searched for English-language primary studies published in peer-reviewed journals. Two independent investigators extracted data. Eleven papers met the criteria for inclusion. Overall, studies

Modified plate-nail fixation for periprosthetic distal femur fractures following total knee arthroplasty in elderly patients - A technical note.

The global rise in total knee arthroplasty (TKA), driven by an aging population, has led to an increased incidence of periprosthetic fractures (PPFs). Dual implants for distal femur periprosthetic fractures (PDFFs) are a growing area of interest for these challenging fractures with dual plating (DP) and plate-retrograde femoral intramedullary nail (PN) emerging as viable constructs for these injuries. However, dual implants have inherent limitations. Herein we focus on describing a modified PN fixation-retrograde tibial intramedullary nail (RTN) combined with a less invasive stabilization syst

Plantar Plating for the Treatment of Proximal Fifth Metatarsal Fractures in Elite Athletes.

Proximal fifth metatarsal fractures, zones II and III, are commonly treated surgically, especially in elite athletes. Intramedullary screw fixation remains the most used construct despite nonunion and refracture. High tensile forces on the plantar-lateral aspect of the fifth metatarsal are difficult to control, and intramedullary screw fixation depends on ideal screw position, length, and width. The authors present a plantar plating technique with cancellous bone autograft for zones II and III proximal fifth metatarsal fractures. Rotational instability and plantar-lateral gapping are resisted

[How many severely injured multiple-trauma patients can benefit from the biomechanical advantage of early mobilization following femoral intramedullary nailing?].

The benefit of early operative stabilization of femoral fractures is established in patients with multiple injuries. In the last few years the unreamed femoral nail is favoured for internal fixation of femoral fractures despite pathophysiological concerns. The foremost advantage of femoral nails compared with plate fixation is the possibility of early full weight bearing. The aim of this retrospective study was to investigate, under consideration of the severity of injury, the extent of injury, and the clinical course, if multiple injured patients with concomitant femoral fractures benefit fro

Medial migration of the intramedullary Gamma 3 nail - a case report.

Intertrochanteric femur fractures are very common in patients over 65 years old, and are often associated with osteoporosis. Proximal femoral nails are preferred because of their biomechanical advantages in the treatment of these fractures, especially if the fracture is unstable. However, many complications associated with intramedullary fracture fixation have been described. The medial migration of the intramedullary gamma nail is a rare complication. The authors report an uncommon but potentially fatal complication, medial and intrapelvic migration of the intramedullary Gamma 3 nails, record

Flexible intramedullary nails for unstable fractures of the tibia in children. An eight-year experience.

Flexible intramedullary nailing is gaining popularity as an effective method of treating long-bone fractures in children. We retrospectively reviewed the records and radiographs of 56 unstable fractures of the tibia in 54 children treated between March 1997 and May 2005. All were followed up for at least two months after the removal of the nails. Of the 56 tibial fractures, 13 were open. There were no nonunions. The mean time to clinical and radiological union was ten weeks. Complications included residual angulation of the tibia, leg-length discrepancy, deep infection and failures of fixation

Internal fixation after fracture or osteotomy of the femur in young children with polyostotic fibrous dysplasia.

Children from 4 to 7 years of age with polyostotic fibrous dysplasia (PFD) may need internal fixation of the femur for either fracture or osteotomy. At that age, the small size of the femur allows only the use of small intramedullary nails. However, titanium elastic nails and rigid intramedullary pediatric interlocking femoral nails - good for fracture or osteotomy fixation in the normal femur - are not indicated in PFD. From 2009 to 2011, we treated eight cases of PFD femoral fracture and deformity by internal fixation with a custom-modified adult humeral nail to which a spiral blade was conn

Treatment of Symptomatic Medial Clavicle Nonunion with Intramedullary Tendon Allograft: A Case Report.

CASE: A 67-year-old patient sustained a medial clavicle fracture (MCF) treated with open reduction and fixation with cerclage sutures. A year later, he presented with symptomatic nonunion and skin prominence, which was revised with an intramedullary tendon allograft fixation including the sternoclavicular joint. At 31 months after the procedure, the patient showed no pain or clinical instability. CONCLUSION: MCF is an uncommon injury. Symptomatic nonunion after the failure of fixation is rarely described. Although the best way to treat this complication is debatable, revision fixation using a

The Role of Arthroplasty in Acute and Failed Fracture Surgery Around the Knee.

Total knee arthroplasty (TKA) has become an increasingly valuable solution for managing acute periarticular fractures and failed fixation, particularly in elderly patients with severe comminution or underlying arthritis. Its key advantage lies in enabling immediate weight bearing and potentially reducing the need for reoperation compared with traditional fixation methods such as open reduction and internal fixation or intramedullary nailing. Although open reduction and internal fixation and intramedullary nailing remain standard fixation methods for many fracture types, primary TKA is gaining

Refractures of the paediatric forearm with the intramedullary nail in situ.

Forearm fractures in children are common. When conservative treatment fails, internal fixation with Elastic Stable Intramedullary Nailing (ESIN) become the first choice in the operative treatment of diaphyseal forearm shaft fractures. Refractures with the intramedullary nail in situ are known to occur but formal guidelines to guide management in such fractures are lacking. We present a well-documented case of a radius midshaft refracture in a 12-year-old boy with the intramedullary nail in situ, managed by closed reduction. Literature is reviewed for this type of complication, the treatment of

Enhancing Stability of Pediatric Femoral Fractures Treated with Elastic Nail Using an External Fixator.

Background: Diaphyseal femoral fractures in children older than 5 years and before adolescence are usually treated surgically. The literature describes several surgical techniques; however, we present an additional minimally invasive technique that combines the use of elastic intramedullary nails and a uniplanar external fixator as an optional solution for managing more complex cases. Method: This was a retrospective review of four children aged 9-12 years who suffered from unstable diaphyseal femoral fractures and were admitted to our institution. Results: We treated four children between the

Double level fractures of the femur treated with closed intramedullary nailing.

The treatment of segmental multilevel fractures of the femur is problematic. Open methods of fracture fixation which strip the soft tissue attachments from the bone cause devascularisation of the middle segment. Mechanical fixation using 2 separate plates results in a stress riser. The use of closed intramedullary nail addresses some of the problems in this difficult group of fractures, preserving the soft tissue envelope and eliminating stress risers. However, vascularity of the middle segment may still be compromised. Six cases of fractures of the femur that were segmental in nature or assoc
