High-energy phosphate metabolism in the frontal lobes of patients with panic disorder detected by phase-encoded 31P-MRS.

Using phosphorus-31 magnetic resonance spectroscopy (31P-MRS), we analyzed the brain phosphorous metabolism in 18 patients with panic disorder (PD) and in 18 sex-, age-, and handedness-matched normal controls (NC). All patients were receiving ongoing drug treatments for PD. The evaluation of phosphorous metabolism in the whole frontal lobes revealed no significant differences between the patients and controls in 31P metabolite levels, although the PD patients showed slightly decreased inorganic phosphate (Pi) concentration of the frontal lobes. Moreover, we found a significant asymmetry (left …

The effectiveness of computerized cognitive training on the attention and working memory of children with frontal lobe epilepsy: A single-case study.

Children with frontal lobe epilepsy (FLE) are at elevated risk for deficits in working memory (WM) and attention that contribute to academic and psychosocial difficulties. Computerized cognitive training (CCT) programs such as Cogmed show promise in pediatric populations, but their effects in FLE remain largely unexplored. This single-case experimental study evaluated the impact of CCT on WM and attention in three children (ages 9-11) with FLE. A non-concurrent multiple-baseline design across participants was used, with staggered intervention onset to control for practice and maturational effe…

Sexual assault by a gynecologist with two frontal lobe meningiomas: implications and challenges for forensic psychiatric assessment: a case report.

Brain diseases may determine the development of focal neurological deficits and behavioral anomalies. In some cases, behavioral alterations may lead to illicit demeanours which, in turn, may have potential serious legal consequences as well. We report the case of a male gynecologist in his seventies sued by a young female patient for sexual violence. The man had no significant premorbid medical history and was a respected medical doctor with several decades of professional activity. Months after the sexual violence episode, he began to manifest symptoms indicative of cognitive impairment, whic

Clinical characteristics and predictors of seizure recurrence in children and adults undergoing frontal lobe epilepsy surgery: A comparative study.

Frontal lobe epilepsy (FLE) is the second most common surgically treated focal epilepsy. Our study aims to compare the clinical characteristics, seizure outcomes, and their predictors between pediatric (<18 years) and adult patients undergoing FLE surgery. Clinical data from consecutive pediatric and adult patients with FLE were retrospectively analyzed and compared at a tertiary hospital. Univariate and multivariate analyses were used to identify the predictors of seizure recurrence. A total of 234 patients with FLE (67 children) were included. Of them, 61.1 % (143/234) achieved Engel I outco…

[Surgical treatment of glial tumors of the frontal lobe. Current state of the problem. Part I: motor functional system].

In everyday clinical practice, neurosurgeons usually deal with glial tumors localized precisely in the frontal lobe. There is a widespread opinion that almost any neurosurgeon can resect glioma of the frontal lobe regardless of experience and microsurgical skills. However, working at the federal center, we observe how employees often have to operate on patients with frontal lobe gliomas after previous surgery in other hospitals. Therefore, treatment of these patients seems to be quite difficult problem of neurosurgery for glial tumors, and incidence of postoperative complications is still high…

Short-term changes in brain networks and cognition in children with frontal lobe lesions treated solely with neurosurgical procedures.

Intracranial lesions in children often have good prognoses, allowing long-term survival. Cognitive functions, crucial for life quality, need more attention. Previous research has focused on adults, with pediatric studies limited by varied lesions and complex treatments. This study aims to evaluate cognitive and brain network changes in children with frontal lobe lesions, which significantly impact cognitive function, using a before-and-after comparison. The study enrolled 20 children with frontal lesions who underwent fMRI and cognitive tests before and after surgery, with only surgical treatm…

Frontal lobe epilepsy and mild malformation with oligodendroglial hyperplasia: Further observations on electroclinical and imaging phenotypes, and surgical perspectives.

OBJECTIVE: Mild malformation with oligodendroglial hyperplasia (MOGHE) is a recently described clinicopathologic entity, associated with drug-resistant epilepsy and extensive epileptogenic networks. Knowledge is accumulating about particular electroclinical phenotypes, correlations with imaging, and potential prognostic significance for surgical outcomes. The study adds relevant information by documenting the presence of a hyperkinetic frontal lobe seizure phenotype in adolescents and an epileptic encephalopathy phenotype in young children. METHODS: Five cases were subjected to a structured p…

Application of the International Classification of Cognitive Disorders in Epilepsy (IC-CoDE) to frontal lobe epilepsy using multicenter data.

RATIONALE: The International Classification of Cognitive Disorders in Epilepsy (IC-CoDE) was recently introduced as a consensus-based, empirically-driven taxonomy of cognitive disorders in epilepsy and has been effectively applied to patients with temporal lobe epilepsy (TLE). The purpose of this study was to apply the IC-CoDE to patients with frontal lobe epilepsy (FLE) using national multicenter data. METHODS: Neuropsychological data of 455 patients with FLE aged 16 years or older were available across four US-based sites. First, we examined test-specific impairment rates across sites using…

Cortical and white matter anatomy relevant for the lateral and superior approaches to resect intraaxial lesions within the frontal lobe.

Despite being associated with high-order neurocognitive functions, the frontal lobe plays an important role in core neurological functions, such as motor and language functions. The aim of this study was to present a neurosurgical perspective of the cortical and subcortical anatomy of the frontal lobe in terms of surgical treatment of intraaxial frontal lobe lesions. We also discuss the results of direct brain mapping when awake craniotomy is performed. Ten adult cerebral hemispheres were prepared for white matter dissection according to the Klingler technique. Intraaxial frontal lobe lesions …

Frontal Lobe Dysfunction in a Depressed Patient Who Survived a Suicide Attempt by Jumping from the Bridge on the Han River.

Suicide attempts at the Han river are rapidly increasing, which are 4.11 times from 2005 to 2015, whereas the rate of completed suicide in South Korea increased 1.07 times during the same period. However, few studies have been conducted on the issue because many suicide attempters were seriously injured after a fall in the Han river. We present a case of a patient with major depressive disorder (MDD) who attempted suicide and minimally injured after jumping from the bridge at the Han river. We could assess his psychological and neurocognitive functions before and immediately after his attempt.

Anesthetic challenges of decompressive craniotomy for the right frontal lobe abscess in a patient with Ebstein's anomaly: A rare case report.

Ebstein's anomaly is a rare congenital heart disease characterized by downward displacement of the tricuspid valve, atrialization of right ventricle, enlarged right atrium with tricuspid regurgitation, or stenosis. It is often associated with atrial septal defect (ASD) or patent foramen ovale with right to left shunt. It has a varied clinical presentation and can manifest as cyanosis, ventricular, or supraventricular arrhythmias and recurrent episodes of congestive heart failure. We describe the anesthetic management of a female patient with Ebstein's anomaly who had right frontal lobe abscess…

What bone part is important to remove in accessing the suprachiasmatic region with less frontal lobe retraction in frontotemporal craniotomies.

BACKGROUND: The anterolateral approach is one of the main routes for accessing suprachiasmatic lesions involving the anterior communicating artery (AComA) complex. Pterional (PT) craniotomy and its alternatives, including orbitozygomatic, orbitopterional, and mini-supraorbital craniotomies, have been developed as tailored frontotemporal craniotomies. One of the main differences between PT craniotomy and its alternatives is the removal of the orbital bone along with the sphenoid wing. However, which bone part is the most important to remove has not been discussed in relation to frontal lobe ret…

Cognitive deficits are associated with frontal and temporal lobe white matter lesions in middle-aged adults living in the community.

BACKGROUND: The association between brain white matter lesions and cognitive impairment in old age is well established. However, little is known about this association in midlife. As this information will inform policy for early preventative healthcare initiatives, we investigated non-periventricular frontal, temporal, parietal and occipital lobe white matter hyperintensities (WMH) in relation to cognitive function in 428 (232 women) community-dwelling adults aged 44 to 48 years. RESULTS: Frontal white matter lesions were significantly associated with greater intraindividual RT variability in

[Quantitative study of the dendritic spins of lamina V pyramidal neurons of the frontal lobe in children with severe mental retardation].

An altered morphology and number of dendritic spines on neurons of the human cortex has been shown to be associated with aging and neuropsychiatric diseases. Proceeding on this assumption specimens from the area 10 of the frontal cortex collected out of 9 children and adolescents with severe mental retardation, aged 6 to 24 years and also 7 non-neuropsychiatric age-matched controls were processed according to the Golgi-Kopsch method. The number of dendritic spines on the apical dendrite of layer V pyramidal neurons was counted on consecutive segments 50 microns long from the cellular body. Mus

Protocol for postmortem bedside endoscopic procedure to sample human respiratory and olfactory cleft mucosa, olfactory bulbs, and frontal lobe.

We present a protocol for the rapid postmortem bedside procurement of selected tissue samples using an endoscopic endonasal surgical technique that we adapted from skull base surgery. We describe steps for the postmortem collection of blood, cerebrospinal fluid, a nasopharyngeal swab, and tissue samples; the clean-up procedure; and the initial processing and storage of the samples. This protocol was validated with tissue samples procured postmortem from COVID-19 patients and can be applied in another emerging infectious disease.
