Computerised tomography evaluation of the frontal recess in inflammatory diseases of the frontal sinus: standardisation of a new technique.

The authors present a new investigation technique by means of CT of the frontal sinus drainage pathway, the frontal recess, which could be of considerable help in defining its potential role in determining a chronic or recurrent inflammatory process of this cavity. Among the main characteristics of this technique are: (1) a clear presentation of the course and conformation of the recess and its relations with surrounding structures; (2) speedy, and therefore economical, operation (12 min for a complete examination); and (3) tolerability, because this examination starts off with axial-scans, wh

Pott puffy tumor in a 4-year-old boy presenting in status epilepticus.

Pott puffy tumor is an osteomyelitis of the frontal bone with the development of a subperiosteal abscess manifesting as a puffy swelling of the forehead or scalp. It is believed to occur as a complication of frontal sinusitis. The modern antibiotic era has made it a rarely encountered entity. This case describes a 4-year-old boy who presented in status epilepticus secondary to Pott puffy tumor.

The effect of expandable polyvinyl acetate packing for preventing stenosis of the frontal sinus ostium.

BACKGROUND: The aim of this study was to evaluate the effect of repeated expandable polyvinyl acetate (EPA) packing for preventing stenosis of the frontal sinus ostium after patients undergo functional endoscopic sinus surgery (FESS). METHODS: A nonrandomized, prospective study of 20 patients with chronic frontal sinusitis was conducted based on the Lund-Mackay scoring system. EPA was packed, for 3 days/week, in total, three times for 3 weeks, into the left frontal sinus ostium right after FESS. For the control, the right side frontal sinus ostium was left unpacked. After 6 months, the patenc…

[Update on levofloxacin in the management of acute sinusitis with risk of complications].

THE DIAGNOSIS OF ACUTE BACTERIAL MAXILLARY SINUSITIS: Is based on at least two of three major criteria: sinus pain, unilateral signs, increasingly voluminous and purulent rhinorrhea. Minor criteria can also be retained for diagnosis if they persist for three days. THE NEED FOR ANTIBIOTIC THERAPY: In this diagnostic context is undeniable, similar to the situation after failure of symptomatic treatment or complication. Likewise for unilateral maxillary sinusitis due to homolateral infection of the superior dental archade or for frontal, ethmoidal, or sphemoïdal sinusities. THE MOST FREQUENTLY CA

Frontal sinus as an identity and sex indicator.

UNLABELLED: The present study explores the variability of frontal sinuses volume in modern Greeks and their potential use in the forensic identification process. METHODS: The sample consisted of 102 CT scan images from a modern Greek population. 50 (49%) were male and 52 (51%) were female. Ages ranged from 19 to 101 (mean age 70.6 for males and 69.6 for females). 3D models of the skull were constructed, and volume calculations of the frontal sinus were done with the software Amira 5.4. 10 CT scans were used to extract 2D frontal and lateral images that were used as proxies of antemortem X-ray

Frontal skull base surgery combined with endonasal endoscopic sinus surgery.

BACKGROUND: Postoperative infection remains a serious complication after radical resection of anterior skull base lesions because intracranial, nasal, and paranasal cavities are opened during surgery. To prevent complications from postoperative infection, we combined endonasal endoscopic sinus surgery (ESS) with the frontal transbasal approach in patients with skull base lesions. METHODS: Patients (n = 16) with anterior skull base lesions extending to the paranasal or nasal cavity underwent surgical resection via the frontal transbasal approach. After removal of the lesion via the transcrania

Frontal sinus obliteration with the pericranial flap.

BACKGROUND: Frontal sinus obliteration is often accomplished by autologous grafts such as fat, muscle, or bone. These avascular grafts carry an increased risk of resorption and infection as well as donor site morbidity. Vascular regional flaps may be used to obliterate small sinuses with less morbidity. OBJECTIVES: To review our experience with the use of the pericranial flap for obliteration of the frontal sinus. METHODS: The records of 10 patients who underwent obliteration of the frontal sinus with the pericranial flap were reviewed. Demographics, indications for frontal sinus obliteratio

Limitations of balloon sinuplasty in frontal sinus surgery.

Balloon sinuplasty is a tool that is used to treat selected patients with paranasal sinus pathologies. No studies have investigated the aetiology of failed access to the frontal sinus. The aim of our study was to specify the intraoperative technical failure rate and to analyse the aetiology of the failed access to predict potential technical difficulties before surgery. We retrospectively analysed the charts of patients who underwent balloon sinuplasty from November 2007 to July 2010 at three different ENT-Centres. CT-analysis of the patients with failed access was performed. Of the 104 fronta

The formation of the human paranasal sinuses.

The paranasal sinuses, which formed in mammals as turbinates and air spaces to perform or aid olfaction, eventually came to participate in the formation and maintenance of the entire cranium and to have concomitant functions as well. Evolving through primates to humans, they were influenced by the retraction of the maxillo-facial cranium and the enlargement of the cerebral cranium and thus reached the state unique to humans. The evolutionary phenomena involved in the formation and functions of the paranasal sinuses include adaptation to olfaction, respiration, and cranial formation. These were

The abnormally large frontal sinus. I. A practical method for its determination based upon an analysis of 100 normal patients.

One hundred normal Caldwell view radiographs were examined on patients who had no signs or symptoms referable to the frontal sinuses. Magnification factors were determined for Caldwell views obtained on a dedicated head unit and as part of a skull series. The areas of the frontal sinuses were actually measured with a Zeiss Videoplan 2 computer and corrected for magnification. The area variation in this normal population was determined. Four linear measurements were individually correlated with these areas. Two of these lines had excellent correlation with the sinus area and if either of these

Phrenology's frontal sinus problem: An insurmountable obstruction?

Whereas some of Gall's critics were quick to assail his organology as materialistic and fatalistic, others questioned his methods and scientific assumptions, especially his craniological tenets. The idea that the skull does not faithfully reflect the features of small, underlying brain areas was repeatedly brought up in the scientific debates. Critics pointed to the frontal sinuses above the eye orbits as evidence for the interior and exterior plates of the cranium not being in parallel-hence, for several or many phrenological organs being unknowable. This article traces the origins of the fro

Free mucosal grafts and anterior pedicled flaps to prevent ostium restenosis after endoscopic modified Lothrop (frontal drillout) procedure: a randomized, controlled study.

BACKGROUND: The endoscopic modified Lothrop procedure (EMLP) is used to manage ongoing refractory frontal sinusitis after failed previous endoscopic sinus surgery (ESS), but this approach has a significant restenosis rate. We evaluated the potential benefits of mucosal grafts and pedicled flaps on the opening of the newly formed frontal ostium. METHODS: Fifty patients with refractory frontal sinusitis or mucoceles after ESS were randomized to undergo EMLP, either with (n = 27) or without (n = 23) mucosal grafts and pedicled flap reconstruction of the neo-ostium. The frontal neo-ostium was mea…
