Melatonin's role as a co-adjuvant treatment in colonic diseases: A review.

Melatonin is produced in the pineal gland as well as many other organs, including the enterochromaffin cells of the digestive mucosa. Melatonin is a powerful antioxidant that resists oxidative stress due to its capacity to directly scavenge reactive species, to modulate the antioxidant defense system by increasing the activities of antioxidant enzymes, and to stimulate the innate immune response through its direct and indirect actions. In addition, the dysregulation of the circadian system is observed to be related with alterations in colonic motility and cell disruptions due to the modificati

Disturbed bowel habits in patients with non-ulcer dyspepsia.

UNLABELLED: BACKGROUND; Emerging medications for non-ulcer-dyspepsia, such as the serotonin-receptor modulators, also affect bowel habits by altering colonic transit. If drugs that alter colonic function were to prove useful in non-ulcer dyspepsia, knowledge of baseline bowel habit disturbances would be potentially critical. AIM: To estimate the rate of non-ulcer dyspepsia patients with clinically relevant constipation or diarrhoea potentially precluding use of motility agents. METHODS: Consecutive patients with non-ulcer dyspepsia (n = 79), gastro-oesophageal reflux disease (n = 135) and or

The pathophysiology of gastric electrical activity and emptying.

A good understanding of upper gastrointestinal physiology is required to properly understand the pathophysiological events in various diseases or after operations on the upper gastrointestinal tract. It is evident that the electrical activity in infancy does not have the orderly pattern of that in adults and that severe disorder of this activity may produce abnormalities of gastric emptying. Surgical procedures on the gastrointestinal tract can cause alterations in the electrical and motility patterns of the stomach. The influence of various surgical manoeuvres on upper gastrointestinal motili

Antimitochondrial antibodies in systemic sclerosis target enteric neurons and are associated with GI dysmotility.

OBJECTIVES: Most patients with systemic sclerosis (SSc) experience gastrointestinal (GI) dysmotility. The enteric nervous system (ENS) regulates GI motility, and its dysfunction causes dysmotility. A subset of SSc patients harbour antimitochondrial M2 autoantibodies (AM2A). Here, we investigate whether M2 is expressed by specific ENS cells, and if AM2A are associated with GI dysmotility in SSc patients. METHODS: Sera from 154 well-characterised patients with SSc were screened for AM2A by enzyme-linked immunosorbent assay. Clinical features and GI transit data were compared between AM2A-positi

Delayed orocecal transit in pediatric gut-brain interaction disorders: A comparative study using the lactulose breath test.

BACKGROUND: Functional abdominal pain disorders (FAPDs) are common gut-brain interaction disorders with unclear pathophysiology. While impaired gastrointestinal motility is thought to play a key role, small intestinal dysmotility remains largely unexplored. Orocecal transit time (OCTT), an indirect indicator of small intestinal transit, offers an insight into its potential contribution to FAPD's pathophysiology. AIM: To assess OCTT in children with FAPDs compared with healthy children using the lactulose breath hydrogen test. METHODS: Thirty-four children (44.1% males, age 5-12 years, mean 7

Electroacupuncture Alleviates Functional Constipation in Mice by Activating Enteric Glial Cell Autophagy via PI3K/AKT/mTOR Signaling.

OBJECTIVE: To investigate autophagy-related mechanisms of electroacupuncture (EA) action in improving gastrointestinal motility in mice with functional constipation (FC). METHODS: According to a random number table, the Kunming mice were divided into the normal control, FC and EA groups in Experiment I. The autophagy inhibitor 3-methyladenine (3-MA) was used to observe whether it antagonized the effects of EA in Experiment II. An FC model was established by diphenoxylate gavage. Then the mice were treated with EA stimulation at Tianshu (ST 25) and Shangjuxu (ST 37) acupoints. The first black

Orocecal transit time in patients in the chronic phase of corrosive injury.

UNLABELLED: Accidental/suicidal ingestion of corrosive substances is common in North India. Decreased gastric secretion and delayed gastric emptying in the chronic phase of corrosive injury has been documented at our center. We hypothesize that patients in the chronic phase of corrosive injury may have delayed orocecal transit time (OCTT). OBJECTIVE: To measure the orocecal transit time (using the noninvasive method of lactulose hydrogen breath test) in patients in the chronic phase of corrosive injury. METHODS: Thirty patients with corrosive injury to their gastrointestinal tract with its s

A novel surgical approach to slow-transit constipation: report of two cases.

INTRODUCTION: Slow-transit constipation after proper diagnosis and extensive medical therapy may have a surgical solution. Total abdominal colectomy and ileorectal anastomosis, at our institution, is the surgical procedure of choice. Nonetheless, patients may reject this alternative because of morbidity. DISCUSSION: We report two cases of slow-transit constipation diagnosed after a thorough investigation with two colonic transit tests showing slow-transit constipation, a normal anorectal manometry, balloon expulsion test, small-bowel follow-through, defecography, laboratory and psychologic te

[Colonic transit time in patient with liver cirrhosis].

BACKGROUND/AIMS: The colonic transit time in patients with liver cirrhosis has not been studied extensively in South Korea. Thus, the authors investigated the differences of colonic transit time between cirrhotic patients and normal controls with attention to factors that affect this change. METHODS: Fifteen cirrhotic patients and 15 controls were included in this study. To exclude any organic diseases, colonoscopy was preceded. The colonic transit time was measured by taking plain abdominal films on the 4th and 7th days after ingestion of radiographic non-absorbable colon markers for 3 days.

General geriatrics and gastroenterology: constipation and faecal incontinence.

The incidence of constipation increases with age but no consistent changes of colonic or anorectal motility have been shown in elderly people. Instead, neurological diseases, constipating drugs, bedriddenness and weak straining ability may explain this increased prevalence of constipation. The amount of dietary fibre in the diet may be reduced because of poor chewing ability. Parkinson's disease is accompanied by both slow colonic transit and impaired relaxation of the anal sphincter. Drug-induced constipation is particularly likely with anti-parkinsonism drugs (either anti-cholinergic or dopa

'Four-position method' makes beginner endoscopists aware of spatial positioning of the left hand to master upper gastrointestinal endoscopy.

Background and study aims  We developed a novel "four-position method" for facilitating endoscopy of the upper gastrointestinal tract, and this study aimed to verify its usefulness for beginner endoscopists. Methods  Medical students (n = 121) were divided into three groups. A simulator subsequently was used to perform endoscopy. Group A was taught how to hold the endoscope and move the dial; Group B was taught the "four-position method"; and Group C, in addition to being taught the "four-position method," was permitted to use an endoscope before introduction to the simulator. In each group, t

Novel mixed NOP/MOP agonist BU08070 alleviates pain and inhibits gastrointestinal motility in mouse models mimicking diarrhea-predominant irritable bowel syndrome symptoms.

The opioid and nociceptin systems play a crucial role in the maintenance of homeostasis in the gastrointestinal (GI) tract. The aim of this study was to characterize the effect of BU08070, a novel mixed MOP/NOP agonist, on mouse intestinal contractility in vitro and GI motility in vivo in physiological conditions and in animal models mimicking symptoms of irritable bowel syndrome (IBS), including diarrhea and abdominal pain. The effect of BU08070 on muscle contractility in vitro was characterized in the ileum and colon. To assess the effect of BU08070 in vivo, the following parameters were ass

Neurogenic colonic motility patterns and gut transit disrupted in a mouse model of Rett Syndrome.

Gastrointestinal dysfunction is a prominent feature of neurodevelopmental disorders and contributes significantly to visceral discomfort, but the specific etiology is unclear. In particular, it is unknown which cell types within central, autonomic or enteric nervous systems, gut muscle, epithelium or immune system are the principal drivers of gut dysfunction. We report that the Mecp2 mouse model of Rett syndrome, a neurodevelopmental disorder with profound gut dysmotility, exhibits impaired whole gut transit times and constipation in vivo as well as striking alterations to propagating neurogen
