Longitudinal symptom severity tracking in vagus nerve stimulation patients: a 2-stage LLM-based pipeline with explainable AI.

OBJECTIVES: To produce the first structured longitudinal dataset of symptom severity trajectories in vagus nerve stimulation (VNS) patients using a 2-stage large language model pipeline for automated symptom severity extraction from clinical notes, as part of the NIH-funded research evaluating vagal excitations and anatomical linkages (U54AT012307). MATERIALS AND METHODS: We analyzed 1427 annotated clinical notes from 95 patients across 5 corpora. LLaMA models (3.2-1B, 3.2-3B, and 3.3-70B) were fine-tuned to classify 13 symptoms as present, absent, or negated (stage 1), then assessed severity

"Contraceptives are like committing a crime": a qualitative study on contraceptive access and utilization in Libya.

INTRODUCTION: Contraceptive access is a critical component of sexual and reproductive health and rights, as well as an indicator of health system performance and broader commitments to gender equity. While global frameworks like Family Planning 2030 emphasize the need for equitable and rights-based access to contraception, Libya's centralized yet fragmented health system, compounded by political instability and weak national monitoring, has contributed to gaps in contraceptive service delivery and limited understanding of how women navigate access in practice. This study examines women's contr…

Pediatric demographics and regional trends from congenital anomalies of the kidney and urinary tract: A U.S. population-based study from 1999 to 2020.

Congenital malformations of the urinary system contribute significantly to pediatric morbidity and mortality. Understanding national and subgroup-specific mortality trends can guide targeted healthcare interventions and policy responses. Mortality data for individuals aged 0 to 14 years from 1999 to 2020 were obtained from the CDC WONDER database. Deaths attributed to congenital anomalies of the urinary system were identified using the International Classification of Diseases, Tenth Revision (ICD-10) codes Q60-Q64. Age-adjusted mortality rates (AAMRs) per 100,000 population were calculated, st

Barriers and facilitators to HIV testing among African and Caribbean heritage communities: a mixed methods study.

OBJECTIVES: African and Caribbean heritage (ACH) communities in the UK face disproportionately high rates of HIV and often experience delayed diagnoses, worsening health inequities. Increasing HIV testing in these communities is essential to address these disparities and support the UK's HIV reduction targets. This study examines barriers and facilitators to HIV testing among Bristol's ACH community, a high-prevalence area with significant rates of late diagnoses, filling a critical gap in context-specific data. METHODS: Using a mixed-methods approach, this study combined 29 in-depth intervie

Resilience Mediates the Association Between Discrimination and Psychological Distress Among Rural, Black Women in South Carolina: A Structural Equation Model.

INTRODUCTION: The marginalization of intersecting racial, gender, and rural identities compounds the structural vulnerabilities (e.g., discrimination, stigma, health disparities) experienced by Black women in South Carolina's rural areas. Discriminatory experiences can deteriorate mental health; however, resilience can potentially reduce the ill effects of discrimination by decreasing associated psychological distress. This study examines the mediating role of resilience in the association between discrimination and psychological distress among rural, Black women in South Carolina in the post-

Intersecting social-ecological vulnerabilities to and lived experiences of sexually transmitted infections among Syrian refugee women in Lebanon: A qualitative study.

Conflict-affected women and girls living in protracted forced displacement settings are vulnerable to sexually transmitted infections (STIs). Yet, little is known about the risk factors for and lived experiences of STIs in complex humanitarian settings, particularly in the Middle East and North Africa, where STIs have long been understudied. This qualitative study adapts the social ecological model to characterize the multi-level risks for and lived experiences of STIs among Syrian refugee women resettled in an urban refugee camp in Beirut, Lebanon. Adopting a community-based sampling strategy

Equity in the Early Pain Management of Long Bone Fractures in Black vs White Patients: We Have Closed the Gap.

INTRODUCTION: Patients with long bone fractures often present to the emergency department (ED) with severe pain and are typically treated with opioid and non-opioid analgesics. Historical data reveals racial disparities in analgesic administration, with White patients more likely to receive analgesics. With the diversifying US population, health equity is increasingly crucial. In this study we aimed to evaluate the early administration of opioid and non-opioid analgesia among Black and White patients with long bone and femur fractures in EDs over different time frames using a substantial datab

Evaluating the Correlation Between Anteroposterior Diameter, Body Surface Area, and Height for Liver Transplant Donors and Recipients.

BACKGROUND: Small stature and female sex correlate to decreased deceased donor liver transplant (DDLT) access and higher waitlist mortality. However, efforts are being made to improve access and equity of allocation under the new continuous distribution (CD) system. Liver anteroposterior diameter (APD) is a method used by many centers to determine size compatibility for DDLT but is not recorded systematically, so it cannot be used for allocation algorithms. We therefore seek to correlate body surface area (BSA) and height to APD in donors and recipients and compare waitlist outcomes by these f

Differences in health care provider screening for tobacco use among youth in the United States: The National Youth Tobacco Survey, 2021.

OBJECTIVE: Health care providers (HCP) are encouraged to screen youth for tobacco product use as a key step in preventing such use and associated health outcomes. However, recent data examining differences in HCP tobacco screening by sociodemographic characteristics and tobacco use is scant. METHODS: Data from the 2021 National Youth Tobacco Survey (N = 14,685) were analyzed. Three types of HCP screening were examined: no screening, any e-cigarette use (e-cigarette only, e-cigarette and other tobacco), and non-e-cigarette tobacco product use. Differences by HCP screening were examined using m

Pilot evaluation of the Making Employment Needs [MEN] count intervention: addressing behavioral and structural HIV risks in heterosexual black men.

Few community-based HIV interventions exist for Black men at heterosexual risk for HIV. None focus on structural HIV risks such as unemployment and unstable housing. This study involved a pilot evaluation of the MEN (Making Employment Needs) Count HIV intervention, a three session peer counselor-delivered program of HIV risk reduction and gender-equity counseling, and employment and housing case management. A single-arm intervention trial of MEN Count was conducted with Black men recruited from a community men's clinic and social services program. Eligible men were those who reported two or mo
