Dying at "home" - a qualitative study of end-of-life care in rural Northern Norway from the perspective of health care professionals.

BACKGROUND: 'Most patients want to die at home' is a familiar statement in palliative care. The rate of home deaths is therefore often used as a success criterion. However, providing palliative care and enabling patients to die at home in rural and remote areas may be challenging due to limited health care resources and geographical factors. In this study we explored health care professionals' experiences and reflections on providing palliative care to patients at the end of life in rural Northern Norway. METHODS: This is a qualitative focus group and interview study in rural Northern Norway

Analysis of a general practitioner's work in a private nursing home for the elderly.

A quantitative analysis was made over a fouryear period (1984-87) of the work and time involved for one rural general practitioner in caring for 42 elderly patients living in a private nursing home. The results were compared with those for the rest of the practice. The study showed that the consultation rate for nursing home patients was 50% higher than the rate for the remaining practice patients aged 65 years or over, and more than twice that for the whole practice. The prescribing rate in the nursing home was twice that of the 65+ years group and six times the rate for the whole practice. T

Speed- and Endurance-Based Classifications of Community Ambulation Post-Stroke Revisited: The Importance of Location in Walking Performance Measurement.

BACKGROUND: Gait speed or 6-minute walk test are frequently used to project community ambulation abilities post-stroke by categorizing individuals as household ambulators, limited, or unlimited community ambulators. However, whether improved clinically-assessed gait outcomes truly translate into enhanced real-world community ambulation remains uncertain. OBJECTIVE: This cross-sectional study aimed to examine differences in home and community ambulation between established categories of speed- and endurance-based classification systems of community ambulation post-stroke and compare these with

[Prosthetic treatment needs among the elder in Germany].

INTRODUCTION: The life expectancy has significantly increased in the last 100 years. Therefore it has to be taken into account that many patients who come to the dentist are over 60. In this group of patients, you should pay attention to geriatric aspects of medical history and treatment plan. Nowadays as well as in the past, teeth loss correlates with the age especially thus with the branch of dentistry which is prosthetics. The aim of this study was to define: 1) the condition of the masticatory system of people over 60 who live with their families or at the nursing home; 2) the assessment o

Using telemedicine in the care of newborn infants after discharge from a neonatal intensive care unit reduced the need of hospital visits.

AimThis study examined the use of telemedicine as a means to follow up infants discharged from a Swedish neonatal intensive care unit to home health care.MethodsFamilies were randomised to either a control group receiving standard home health care (n = 42 families) or a telemedicine group receiving home health care with telemedicine support (n = 47 families) after discharge from the hospital. Both groups had follow-up hospital appointments with the neonatal nurse. In the telemedicine group, appointments were supplemented by the use of a specially designed web page and video calls.ResultsThe us

Add-On Use of Eplerenone Is Effective for Lowering Home and Ambulatory Blood Pressure in Drug-Resistant Hypertension.

The authors aimed to investigate the blood pressure (BP)-lowering ability of eplerenone in drug-resistant hypertensive patients. A total of 57 drug-resistant hypertensive patients whose home BP was ≥135/85 mm Hg were investigated. The patients were randomized to either an eplerenone group or a control group and followed for 12 weeks. The efficacy was evaluated by clinic, home, and ambulatory BP monitoring. Urinary albumin, pulse wave velocity, and flow-mediated vasodilation (FMD) were also evaluated. Home morning systolic BP (148±15 vs 140±15 mm Hg) and evening systolic BP (137±16 vs 130±16 mm

Diarrhea in elderly patients due to Clostridium difficile associated with Salmonella and Shigella infection.

Clostridium difficile is the main cause of nosocomial diarrhea mainly in elderly hospitalized patients and in nursing homes, but less common in the community. We report three elderly patients living in the community, one frail and two in reasonable good health who recently developed diarrhea due to C. difficile associated with Salmonella group C in two cases and Shigella flexneri in the other. The association with Shigella has not previously been reported and there have only been a few cases with Salmonella. These findings may be important due to the possibility that diarrhea caused by C. diff

A comparison of life satisfaction and mood in nursing home residents and community-dwelling elders.

This study compared life satisfaction and mood in a sample of 138 cognitively intact and ambulatory elders, including 70 who lived in nursing homes and 68 who lived independently in the community. Community-dwelling elders reported greater life satisfaction, and scored higher on the Vigor-Activity subscale of the Profile of Mood States (POMS) than those who lived in a nursing home. Nursing home residents scored higher on the Depression-Dejection, Tension-Anxiety, and Confusion-Bewilderment subscales of the POMS. No between group differences were shown on the Anger-Hostility and Fatigue-Inertia

[Rehabilitation of psychiatric patients in the halfway house--a 10-year retrospective study].

In a period of ten years 159 residents were admitted to a transitional home. 90% of them were diagnosed as chronic schizophrenics with a marked psychic and social handicap. With an average age of 28.7 years and a mean duration of illness of 6.8 years most of the patients had been hospitalized several times; 95% had no job, lived on social assistance or were invalidity pensioners. There was poor success in vocational rehabilitation (22%). Better results were seen in solving the housing problems, showing a clear-cut tendency to move from the family of origin into protected group homes or indepen…

HIV and serious mental illness: reducing the risk.

Persons with SMI residing in community mental health center group homes received an educational intervention program on HIV/AIDS. As with virtually all such approaches provided for this population, the intervention was generalized from programs used with other populations, for example, users of intravenous drugs, gay men, and adolescents. Assessment of pre- and postintervention knowledge indicated no increase in accurate information. Further, consumers were initially uncertain regarding their risk for HIV infection; this attitude, too, remained unchanged. The research design employed did not c…

The development of an adopted child recovering from severe malnutrition. Case report.

The growth and developmental level (DQ) of a child who was in hospital with severe protein-energy malnutrition (PEM) at nine months of age and was subsequently exposed to a greatly improved environment are described. The child was a subject in a longitudinal study, and his progress is compared with that of 16 other children recovering from severe PEM who continued to live in poor homes. All children had regular developmental assessments over five years and their growth was measured. The study child showed dramatic improvements in DQ, and at five years was over two standard deviations above the

Why some disabled adults in Medicaid face large out-of-pocket expenses.

Using national survey data, we found that the Medicaid program effectively limits out-of-pocket health care spending for the majority of Medicaid enrollees who are disabled but are not in institutions, such as nursing homes. However, we also found that for 10 percent of these enrollees, annual out-of-pocket spending is $1,200 or higher--a significant sum for people who are low-income. Other research has shown that such cost barriers can deprive Medicaid enrollees of cost-effective care in favor of cost-ineffective care or no care at all. As state policy makers contemplate benefit reductions th

[Prevalence of Trypanosoma cruzi infection in Nicaraguan students. Isle of Youth Special Municipality. February, 1987].

A seroepidemiologic study on American trypanosomiasis was performed to a group of Nicaraguan students in Isle of the Youth Special Municipality, during February 1987, using indirect hemagglutination and indirect immunoflorescence techniques. The prevalence obtained with both tests and the percentage of the most frequent titer of indirect immunofluorescence are pointed out. The youth living in rural zones presented higher risk of acquiring the infection than those living in urban zones. The students coming from the Atlantic Coast, Rio San Juan and Masaya, presented the highest positive rates. O

Effectiveness of interventions to promote couples-based HIV testing and family health during pregnancy and postpartum in Kenya: a multicentre, couples-based, randomised controlled trial.

BACKGROUND: Engaging couples during pregnancy can improve HIV prevention and family health. We examined the effectiveness of two couples-focused interventions on uptake of couples HIV testing and counselling (CHTC) and maternal and child health outcomes. METHODS: In this multicentre, couples-based, randomised controlled trial, HIV status-concordant and HIV status-discordant pregnant couples (aged ≥15 years) from 24 antenatal clinics across Migori and Kisumu counties in Kenya were randomly assigned (1:1:1) to one of three study groups: lay counsellors delivered five couple home visits on healt

Extended thrombotic prophylaxis in COVID-19 early discharge: A retrospective cohort study.

INTRODUCTION/BACKGROUND: Due to limits in available staff and space during the COVID-19 pandemic, home monitoring programmes were introduced, reducing strain on resources, and preventing readmissions. Several hospitals included prophylaxis for venous thrombotic events (VTE), as COVID-19 appeared to be thrombogenic. Other hospitals did not, expecting patients to be more mobile while at home. Our aim was to determine whether the administration of nadroparin has led to a difference in VTE occurrence between two groups of previously included patients. MATERIALS AND METHODS: Retrospective cohort s

[The present state and subject of home hospice for patients with non-cancer disorders].

In order to clarify the issues regarding home hospice for patients with non-cancer disorders, the patients' death at home was compared with that of terminal cancer patients treated in our clinic. The following results were obtained: In the non-cancer disorder group, the number of deaths of female patients was greater than that of male patients. In this same group, the average age of patients at death was greater by 6.7 years and the period of home care was longer than that in the terminal cancer group. The underlying diseases in the cases belonging to the non-cancer disorder group were mainly
