On the way to deprescribing levothyroxine in subclinical hypothyroidism in general practice - the patients' perspective of enablers and barriers.

BACKGROUND: Recent studies indicate that the synthetic thyroid hormone levothyroxine (LTX) in many cases has no beneficial effects on patients with subclinical hypothyroidism. Still, prescriptions are increasing worldwide. If there is no clear indication for treatment, patients treated with LTX should be offered a deprescribing trial according to current guidelines. However, there is currently no protocol for deprescribing LTX in primary care. OBJECTIVES: We aimed to explore patients' enablers and barriers towards deprescribing levothyroxine in primary care to inform the further participatory

"A creature inside me": perceptions and representations of HIV among adolescents living with HIV in Malawi.

Malawi is among the countries with the highest HIV prevalence worldwide. Adolescents living with HIV (ALHIV) face diverse challenges, which influence their emotional wellbeing and long-term health, in addition to impacting HIV onward transmission. HIV education, especially the use of fear-based animation, but also the figurative language used for HIV, contribute to how ALHIV perceive and respond to their HIV status. The aim of the study was to explore how ALHIV in Malawi describe, perceive, and represent HIV, with a particular focus on the role of language in shaping these perceptions and its

Gap Analysis Investigating Healthcare Professionals' Skills and Knowledge for Pain Management in Haemophilia-A Qualitative Study.

INTRODUCTION: People with haemophilia continue to experience pain impacting activities of daily living despite the advances in haemostatic treatments. Although guidelines exist, pain management in the clinical setting remains inadequate. It is unclear why this gap in practice persists. This study aimed to explore the practice and experience of pain management by haemophilia healthcare professionals (HCPs) to better understand perceived competency and barriers/facilitators to appropriate care. METHODS: European HCPs working ≥ 3 years in haemophilia care were invited to participate (haematologi

Calling time on low-risk drinking guidelines: An evaluation of alternative methods to communicate risks of alcohol use to consumers.

BACKGROUND: Current epidemiological analyses suggest there is no risk-free level of alcohol consumption. Awareness remains low and consumption levels high. There is limited research on the types of alcohol warning label (AWL) messages to address this concern. AIMS: We tested alternative AWL messages with young adults to identify effective methods of communicating Canada's 2023 Guidance on Alcohol and Health. METHODS: We recruited 332 young adults attending two Canadian universities who drank at least monthly. Participants' knowledge of health risks from alcohol was assessed. Six AWLs were de

"Disease requires medication, right? And children don't, they're for life": HIV prevention according to low-income youth from five cities in Brazil.

The increase in HIV incidence rates among young people contrasts with the discontinuity of educational actions and the invisibility of AIDS in public spaces. Based on socio-anthropological research, this article analyzes conceptions and practices regarding HIV prevention of 139 men and women, aged 15 to 24 years, mostly heterosexual and cisgender, from low-income Brazilian communities in Porto Alegre, São Paulo, Rio de Janeiro, Manaus, and Salvador. The study involved ethnographic observation, interviews, and focus groups. According to the findings, the knowledge about diagnosis and new techno

Evaluation of Educational Activity with Guatemalan Birth Attendants About Helping Babies Breathe.

Introduction: Guatemala's Infant Mortality Rate was 20th highest in the world at 18 deaths per 1,000 live births in 2022. Part of the problem is lay midwives (LMs) who attend most births at home lack training in helping babies breathe (HBB). LMs transfer babies struggling to breathe to firefighters (FF). The aims of this mixed-methods pilot study were to evaluate a HBB educational activity, explore LM and FF's lived experience HBB, and to assess if the HBB educational activity changed participants' knowledge. Methods: Twelve LMs and six FFs (N = 18) participated in focus group discussions, com

Multilevel Factors Shaping Awareness of and Attitudes Toward Pre-exposure Prophylaxis for HIV Prevention among Criminal Justice-Involved Women.

Although pre-exposure prophylaxis (PrEP) is a key tool in HIV prevention efforts, little is known about PrEP as a prevention strategy for criminal justice-involved (CJI) women. The purpose of this study was to examine multilevel factors shaping PrEP awareness and acceptability among CJI women. Between January 2017 and December 2017, we conducted 52 interviews with CJI women at high risk for HIV and stakeholders from the criminal justice (CJ) and public health (PH) systems. Interviews explored awareness of PrEP and the multilevel factors shaping PrEP acceptability. Data were analyzed using indu

Critical Time Intervention for People Leaving Shelters in the Netherlands: Assessing Fidelity and Exploring Facilitators and Barriers.

International dissemination of evidence-based interventions calls for rigorous evaluation. As part of an evaluation of critical time intervention (CTI) for homeless people and abused women leaving Dutch shelters, this study assessed fidelity in two service delivery systems and explored factors influencing model adherence. Data collection entailed chart review (n = 70) and two focus groups with CTI workers (n = 11). The intervention obtained an overall score of three out of five (fairly implemented) for compliance fidelity and chart quality combined. Fidelity did not differ significantly betwee

Knowledge, Attitudes and Perceptions About Routine Childhood Vaccinations Among Jewish Ultra-Orthodox Mothers Residing in Communities with Low Vaccination Coverage in the Jerusalem District.

Background and aims Childhood vaccinations are an important component of primary prevention. Maternal and Child Health (MCH) clinics in Israel provide routine vaccinations without charge. We aimed to study attitudes and knowledge on vaccinations among mothers, in communities with low immunization coverage. Methods A qualitative study including focus groups and semi-structured interviews. Results Low immunization coverage was defined below the district's mean (age 2 years, 2013) for measles-
