Diagnoses and management of adult cough: An Indian Environmental Medical Association (EMA) position paper.

BACKGROUND: Cough is a common yet distressing symptom that results in significant health care costs from outpatient visits and related consultations. OBJECTIVE: The understanding of the pathobiology of cough in recent times has undergone an evolution with Cough hypersensitivity syndrome (CHS) being suggested in most cases of dry cough. However, in the case of productive cough, ancillary mechanisms including impaired Mucociliary clearance, in addition to hypermucosecretory bronchospastic conditions of Smoker's cough, asthma-COPD overlap, bronchiectasis, and allergic bronchopulmonary aspergillo

The experiences and adaptations of abortion providers practicing under a new TRAP law: a qualitative study.

OBJECTIVE: Abortion laws are proliferating in the United States, but little is known about their impact on abortion providers. In 2011, North Carolina instituted the Woman's Right to Know (WRTK) Act, which mandates a 24-h waiting period and counseling with state-prescribed information prior to abortion. We performed a qualitative study to explore the experiences of abortion providers practicing under this law. STUDY DESIGN: We conducted semistructured interviews with 31 abortion providers (17 physicians, 9 nurses, 1 physician assistant, 1 counselor and 3 clinic administrators) in North Caroli

Hyperemesis gravidarum: a holistic overview and approach to clinical assessment and management.

Hyperemesis gravidarum (HG) is a rare and severe form of nausea and vomiting of pregnancy associated with significant costs and psychosocial impacts. The etiology of HG remains largely unknown, although maternal genetics and placental factors are suspected. Prompt recognition and treatment of HG are essential to minimize associated maternal and fetal morbidity. Diagnosis is made on the basis of typical presentation, with exclusion of other causes of severe nausea and vomiting of pregnancy. Validated clinical tools are available to assess severity of symptoms and guide plans of care. Evidence t

Classification of eating disturbance in children and adolescents: proposed changes for the DSM-V.

Childhood and adolescence are critical periods of neural development and physical growth. The malnutrition and related medical complications resulting from eating disorders such as anorexia nervosa (AN), bulimia nervosa (BN) and eating disorder not otherwise specified may have more severe and potentially more protracted consequences during youth than during other age periods. The consensus opinion of an international workgroup of experts on the diagnosis and treatment of child and adolescent eating disorders is that (a) lower and more developmentally sensitive thresholds of symptom severity (e

Policy Recommendations From Transmission Modeling for the Elimination of Visceral Leishmaniasis in the Indian Subcontinent.

BACKGROUND: Visceral leishmaniasis (VL) has been targeted by the World Health Organization (WHO) and 5 countries in the Indian subcontinent for elimination as a public health problem. To achieve this target, the WHO has developed guidelines consisting of 4 phases of different levels of interventions, based on vector control through indoor residual spraying of insecticide (IRS) and active case detection (ACD). Mathematical transmission models of VL are increasingly used for planning and assessing the efficacy of interventions and evaluating the intensity and timescale required to achieve the el

Impact of universal infectious diseases consultation on the management of Staphylococcus aureus bloodstream infection in a Swiss community hospital.

We compared two periods, before and after systematic implementation of infectious diseases consultation for each Staphylococcus aureus bacteremia. Comparing these periods, we showed a significant increase in follow-up blood cultures (from 38% to 85%), transthoracic echocardiography (from 25% to 79%), and administration of appropriate antistaphylococcal agent (from 77% to 96%).

Consensus guidelines for implementation of quality processes to prevent invasive fungal disease and enhanced surveillance measures during hospital building works, 2014.

Healthcare-associated fungal outbreaks impose a substantial economic burden on the health system and typically result in high patient morbidity and mortality, particularly in the immunocompromised host. As the population at risk of invasive fungal infection continues to grow due to the increased burden of cancer and related factors, the need for hospitals to employ preventative measures has become increasingly important. These guidelines outline the standard quality processes hospitals need to accommodate into everyday practice and at times of healthcare-associated outbreak, including the role

Contraceptive options for women with preexisting medical conditions.

Most healthy women have normal pregnancies; however, even healthy women may experience serious morbidity during pregnancy. Women with chronic medical problems face increased pregnancy-related risks compared with their healthy peers. Planning pregnancy improves maternal and fetal outcomes; medical conditions can be stabilized, teratogens can be avoided, and early antenatal intervention and surveillance can be instituted. The safest and most effective forms of contraception should be offered to women with medical conditions. Overestimatation of risk associated with the use of contraception among

Contraception in systemic lupus erythematosus and antiphospholipid antibody syndrome.

PURPOSE OF REVIEW: Family planning and contraception are central to disease management in people with rheumatic musculoskeletal diseases (RMD), particularly systemic lupus erythematosus (SLE) and antiphospholipid syndrome (APS), in whom unplanned pregnancy can carry substantial maternal and fetal risk. This review summarizes current evidence and guideline-based recommendations regarding contraceptive choices in women with SLE and APS, with a focus on thrombotic risk, bone health, and cardiovascular considerations. RECENT FINDINGS: Estrogen-containing hormonal contraception is well established

Landscape analysis of the Kenyan policy on the treatment and prevention of diarrheal disease among under-5 children.

OBJECTIVE: Diarrhoea remains a leading cause of morbidity and death among under-5 children in Kenya, despite multipronged policy and programme initiatives to increase access to treatment. This study interrogates the comprehensiveness and adequacy of Kenya's policies, frameworks and action plans for diarrheal management and prevention. The study seeks to identify policy and practice gaps that need to be filled to strengthen diarrhoea treatment and prevention among under-5 children in Kenya. DESIGN: Our study is a landscape analysis, which seeks to identify the gaps in the current Kenya diarrhe

Sports Pre-Participation Evaluation in Healthy Adults-A Survey-Based Assessment of Guideline Acceptability and Feasibility Among Sports Physicians and Participants.

INTRODUCTION: Sports pre-participation evaluations (PPEs) aim to identify people at increased risk of acute adverse events during physical activity. Little is known about their acceptability and feasibility among physicians and participants ('interest holders'). During the development of a consensus-based PPE guideline in Germany targeted at healthy adults who plan to take up or resume intense physical activity, we used surveys to assess interest holder perceptions of the acceptability and feasibility of the PPE, and to obtain their feedback on draft guideline recommendations. METHODS: Cross-

Trends in the Treatment of Pediatric Hydrocephalus Since 2014: Understanding the Role of the 2014 Hydrocephalus Guidelines.

BACKGROUND AND OBJECTIVES: The management of hydrocephalus has significantly changed over the past decade. Endoscopic third ventriculostomy has become more prevalent, and shunt surgery has become more protocolized through research efforts by the Hydrocephalus Clinical Research Network. In 2014, the Pediatric Hydrocephalus Guidelines were first published, providing a cohesive source for clinical guidance. We planned to investigate national trends in the management of pediatric hydrocephalus after the publication of the 2014 Pediatric Hydrocephalus Guidelines as guidelines act as a compilation o

Perspective: Using Bronchiectasis Action Management Plans for Children With Bronchiectasis-Can It Improve Clinical Care?

While once thought to be rare, bronchiectasis has been increasing globally over the last 15 years. Bronchiectasis is a major contributor to chronic lung morbidity and mortality but remains a neglected disease in respiratory health globally. Currently, few high-level evidence-based management strategies are available for children with bronchiectasis. Strategies to improve clinical outcomes associated with exacerbations are important. In other respiratory conditions such as asthma and chronic obstructive pulmonary disease, use of personalized written management plans have been shown to improve c

Two Sides to Every Conversation: Communication Strategies and Appropriate Interpreter Utilization in Pediatric Orthopaedics.

Marginalized patients including those with limited English proficiency (LEP) are at risk of seeing graver health outcomes across many specialties. Communication barriers contribute to impaired shared-decision making and missed education opportunities. Tools are available to clinicians to allow for more accurate and effective education and to facilitate shared decision-making. This article reviews existing guidelines for utilization of professional interpreters. A framework is provided for pre-visit planning, facilitating appropriate introductions, and setting up a clinic room to optimize workf

Artificial intelligence predicts healthcare workers' antibiotic use intentions from psychological and behavioral measures across multiple theories.

Antimicrobial resistance (AMR) remains a global health priority, partly driven by non-guideline-concordant antibiotic prescribing among healthcare workers. Existing interventions often overlook the psychological and contextual factors that shape prescribing behaviour. This study integrated behavioural theory with explainable machine learning to identify psychological predictors of intention to use antimicrobials appropriately among clinicians. A cross-sectional survey was conducted among 1135 healthcare workers from four public hospitals in China. Participants completed questionnaires based on
