Does Helicobacter pylori affect portal hypertensive gastropathy?

Helicobacter pylori (H. pylori) is a major etiological factor of peptic ulcer disease (PUD). It is supposed to be a risk factor for the more frequently encountered PUD in patients with liver cirrhosis. Several investigators have evaluated the effect of H. pylori on liver cirrhosis, portal hypertensive gastropathy (PHG) and encephalopathy with controversial results. Some reports have shown a higher seroprevalence and suggested a synergistic effect of H. pylori on liver cirrhosis and PHG. However, this increased prevalence is associated with a negative histology and is not influenced by the caus…

Helicobacter pylori infection and related gastrointestinal diseases.

Helicobacter pylori has been implicated in the pathogenesis of a number of digestive tract disorders, such as chronic active gastritis, peptic ulceration, gastric cancer, and mucosa-associated lymphoid tissue lymphoma. Disease outcome is dependent on many factors, including bacterial genotype, host physiology and genetics, and environmental factors such as diet. Researchers continue to explore the complexities of H. pylori infection, seeking to explain why some individuals have asymptomatic infection, whereas others experience clinical disease. The importance of treating H. pylori infection in…

[Pseudomembranous colitis after Helicobacter pylori eradication therapy].

A 69-year-old woman with gastritis was prescribed a 1-week triple regimen therapy of Amoxicillin, Clarithromycin, and Lansoprazole to eradicate Helicobacter pylori (H. pylori) starting on March 7, 2005. H. pylori was detected on the gastric mucosa by the urease test. Twenty days after eradication therapy, she began to suffer from profuse watery diarrhea. Colonoscopy on April 12 showed multiple pseudomembranes in the cecum and the transverse colon, leading to a diagnosis of pseudomembranous colitis. Because she had not taken Vancomycin (VCM) (500 mg/day) as directed, she had a relapse of watery…

Pharmacoeconomic comparison of Helicobacter pylori eradication regimens.

BACKGROUND: Helicobacter pylori is the most important etiologic agent for development of peptic ulcer, chronic gastritis and gastric carcinomas. It is now well established that H. pylori eradication treatment is more cost-effective than acid suppressing therapies alone for the treatment of peptic ulcer disease. However, the comparative cost-effectiveness of various H. pylori eradication regimens is still not clear. OBJECTIVE: This study was designed to make a pharmacoeconomic comparison of different H. pylori eradication regimens in patients with peptic ulcer disease or chronic gastritis, usi…

Volatile sulfur compounds produced by Helicobacter pylori.

GOALS: To assess the volatile sulfur compounds produced by three strains of Helicobacter pylori in broth cultures mixed with sulfur-containing amino acids. BACKGROUND: Halitosis has been reported in H. pylori-positive patients, and volatile sulfur compounds such as hydrogen sulfide and methyl mercaptan are known to be responsible for inducing oral malodor. Whether H. pylori produces these volatile sulfur compounds has yet to be established. STUDY: Three strains of H. pylori (ATCC 43504, SS 1, DSM 4867) were cultured with 5 mM cysteine and methionine. After 72 hours of incubation, the headspa…

Extragastric diseases associated with Helicobacter pylori infection.

This paper critically reviews the current literature on extragastric diseases associated with Helicobacter pylori infection, with an emphasis on methodologic issues that complicate interpretation of study findings. This review reveals common study limitations and overall uncertainty that H. pylori infection plays a role in extragastric diseases, although such a role has not been clearly ruled out for specific diseases of relevance. Evidence suggests that anti-H. pylori therapy may lead to improvement of a few extragastric diseases, in particular, idiopathic thrombocytopenic purpura, iron defic…

Helicobacter pylori infection and gastric MALT lymphoma.

Helicobacter pylori infection is implicated in the development of two different gastric cancers: gastric adenocarcinoma and gastric MALT lymphoma. The association with the gastric MALT lymphoma is strong and causal. It is currently the only cancer which can be treated by a simple antibiotic treatment. However, the evolution of an H. pylori infection towards lymphoma is exceptional. Host susceptibility factors and environmental factors predisposing a patient to lymphoma have not yet been determined. The bacterial factors are currently being identified.

Cytokine expression in pediatric Helicobacter pylori infection.

Helicobacter pylori infection is one of the most common gastrointestinal infections worldwide and almost invariably causes chronic gastritis in the infected host. A predominant Th1 profile has been demonstrated in H. pylori-infected mucosa from adults, but no previous study has evaluated in situ cytokine expression in children. We therefore examined expression of proinflammatory, anti-inflammatory, and regulatory cytokines by immunohistochemistry in cryopreserved antral biopsy specimens from 10 H. pylori-infected and 10 uninfected children and correlated expression of cytokines with histology …

Duodenal ulcer promoting gene of Helicobacter pylori.

BACKGROUND & AIMS: Identification of a disease-specific H pylori virulence factors predictive of the outcome of infection remains unachieved. METHODS: We used the polymerase chain reaction and Southern blot to compare the presence of 14 vir homologue genes with clinical presentation of H pylori infection, mucosal histology, and mucosal interleukin (IL)-8 levels. RESULTS: We examined 500 H pylori strains from East Asia and South America, including 120 with gastritis, 140 with duodenal ulcer (DU), 110 with gastric ulcer (GU), and 130 with gastric cancer. Only 1 gene that encompassed both jhp09

Helicobacter pylori infection and idiopathic thrombocytopenic purpura.

A treatment strategy for idiopathic thrombocytopenic purpura (ITP) is considered with the aim of cure or management of the bleeding tendency. In 1998, Gasbarrini et al reported a high prevalence of Helicobacter pylori infection in patients with ITP and showed that platelet recovery occurred after eradication therapy in most cases. Since then, many studies were performed to evaluate eradication therapy. This article discusses the incidence of H pylori infection in ITP, characteristic clinical features in H pylori-positive ITP, the effectiveness of eradication on platelet count increase, and the…

[Helicobacter pylori infection and idiopathic thrombocytopenic purpura].

There are increasing data on the association between Helicobacter pylori infection and idiopathic thrombocytopenic purpura and the significant increase in platelet count after bacterial eradication. The aim of this review was to consider the studies so far published on Helicobacter pylori infection and idiopathic thrombocytopenic purpura in order to evaluate a possible correlation between these two conditions. A review of the literature showed that 306 out of the 524 patients investigated (54.8%) were positive for Helicobacter pylori infection and that the bacterium was eradicated in 83% of ca…

Gastroesophageal reflux disease and Helicobacter pylori infection.

The prevalence of Helicobacter pylori infection has been decreasing while the prevalence of gastroesophageal reflux disease and esophageal adenocarcinoma has been increasing in developed countries since the 1930s. This has raised concerns that H. pylori infection may protect against esophageal disease and that the disappearance of H. pylori from the population might lead to a further increase in gastroesophageal reflux disease. Some studies have suggested that eradication of H. pylori in patients with duodenal ulcer disease results in an increase in the incidence of erosive esophagitis, wherea…

Gastro-oesophageal reflux disease and Helicobacter pylori.

The interaction between gastro-oesophageal reflux disease (GERD) and Helicobacter pylori (H. pylori) infection has been the subject of intense scrutiny in recent years. Although the evidence base is incomplete it is now sufficient to clarify a number of key questions. H. pylori is not a risk factor for reflux disease. Similarly, H. pylori infection, in most patients, is not ''protective'' against the risk of developing reflux and oesophagitis. Furthermore, reflux and oesophagitis are not more likely to develop or to worsen after H. pylori eradication therapy and eradication does not make contr…

[Helicobacter pylori infection and idiopathic thrombocytopenic purpura].

The prevalence of Helicobacter pylori(H. pylori) infection and the effect of its eradication on platelet count in 51 chronic idiopathic thrombocytopenic purpura(ITP) and 9 secondary autoimmune thrombocytopenic purpura(SAITP) patients, were investigated. H. pylori infection was found in 31 ITP patients(60.8%) and in two SAITP(22.2%). H. pylori eradication was obtained in 24 of 26 infected ITP patients. 14 of 24 H. pylori-eradicated patients(58.3%) showed a significant increase in platelet count 6 months after eradication, however two infected patients with SAITP did not show platelet increase. …
