Massive spontaneous haemoperitoneum in a post-partum patient with neurofibromatosis type 1.

Neurofibromatosis type 1 (NF-1) or von Recklinghausen's disease is a genetic, progressive, multi-system disease that predominantly affects the skin and nervous system. Vascular involvement is rare, but can have catastrophic results. Pregnant patients with this disease need careful, multidisciplinary follow up in order to control possible vascular alterations, which usually affect the kidneys. We present the case of a patient diagnosed with NF-1 who debuted with massive spontaneous hemoperitoneum in the late postoperative period of an elective Cesarean section. Spontaneous bleeding from large v…

Maternal and fetal death at 22 weeks following uterine rupture at the site of the placenta percreta in a C-section scar.

Placenta percreta is the abnormal invasion of the placenta through the myometrium and serosa of the uterus. It is the most invasive of the placenta accreta spectrum followed by placenta increta. This paper presents a case of a maternal and fetal death in the second trimester due to rupture of the uterus at the site of placenta percreta in a C-section scar. Postmortem MRI showed a large hemoperitoneum and thinning of the anterolateral uterine wall. Internal examination revealed two liters of blood in the abdomen and rupture of the anterolateral uterine wall at the site of placenta percreta in a…

Spontaneous liver rupture in a patient with peliosis hepatis: a case report.

Peliosis hepatis is a rare pathological entity and may cause fatal hepatic hemorrhage and liver failure. Here, we present a young male patient with aplastic anemia, who had received long-term treatment with oxymetholone. The patient suffered from sudden onset of intra-abdominal hemorrhage with profuse hemoperitoneum. The patient was treated successfully with a right hemihepatectomy and is in good health after 13 postoperative months. We suggest that peliosis hepatis be considered in patients with hepatic parenchymal hematoma, especially in patients under prolonged synthetic anabolic steroid me…

[Rupture of non-communicating rudimentary pregnant uterine horn in a pseudo-unicornuate uterus at 23 weeks of amenorrhea. Case report].

Pregnancy in the rudimentary horn of a unicornuate uterus is an extremely rare form of ectopic gestation associated with a high risk of uterine rupture. We report the case of a pregnancy developed in a non communicating rudimentary horn of a unicornuate uterus complicated by horn rupture at 23 weeks of amenorrhea showing as an acute abdominal pain and massive hemoperitoneum. This patient's uterine abnormality was known before, as this woman has delivered two years before at term a healthy boy by cesarean section. This past pregnancy was located in the normal horn and the non communicating rudi…

Spontaneous liver rupture in pre-eclampsia.

Spontaneous liver rupture is a rare complication of pre-eclampsia. A booked, 30-year old woman with pre-eclampsia and twin gestation developed severe abdominal pains 10 hours after a supervised, vaginal delivery. On examination she was in hypovolemic shock with abdominal distension from hemoperitoneum. Uterine rupture was suspected and she had a laparotomy after resuscitation. But at surgery the uterus was intact and instead liver rupture was found which was managed by omental packing after evacuating the clots. Postoperatively, the patient developed acute renal failure that responded well to …

Ruptured hepatoblastoma with massive internal bleeding in an adult.

Hepatoblastoma is the most common primary hepatic tumor of children. However, only a very few cases have been reported in adults. Most studies support treatment with chemotherapy followed by surgical resection. We present the first reported case of adult hepatoblastoma in Taiwan. A 52-year-old female suffered from sudden onset of abdominal pain and general weakness for days. Internal bleeding with hemorrhagic shock was suspected and two massive lesions in both lobes of the liver with hemoperitoneum were noted from imaging studies. Surgical resection of the larger left lobe tumor and radio-freq…

Right Middle Cerebral Artery Occlusion Following Ovarian Hyperstimulation Syndrome: A Case Study.

Ovarian hyperstimulation syndrome (OHSS) is one of the most serious complications occurring from the growing field of artificial reproductive technology. The expanding literature points to a link between OHSS and thromboembolism events. We present a case of ischemic stroke secondary to a right middle cerebral artery occlusion in a female after a fresh embryo transplant. A CT of the abdomen demonstrated significant free fluid in the abdomen, in which hemoperitoneum from an ectopic pregnancy could not be excluded. A paracentesis was completed and demonstrated clear fluid, thus supporting a presu…

Ultrasonography Facilitates the Diagnosis of Traumatic Iliopsoas Hemorrhage: A Report of Two Cases with Different Patterns.

We report two cases of traumatic iliopsoas hemorrhage, without hemoperitoneum, initially detected by ultrasound. Flexion hip contracture in the first case and incomplete femoral nerve palsy in the second case alerted the sonographer to the possibility of traumatic iliopsoas hemorrhage. The first case involved a 54-year-old man who complained of progressive right flank pain and difficulty in walking after falling to the ground. The second case involved a 34-year-old man who complained of severe lower back pain and numbness and weakness of the left leg after a motorcycle accident. In both cases,…

Diagnosis and management of rudimentary horn pregnancy rupture, misinterpreted as bicornuate uterus in the 14th week of pregnancy.

We present a 26-year-old primigravida with rudimentary horn pregnancy rupture at 14 weeks of pregnancy. Uterine anomaly was first diagnosed at the time of nuchal translucency scan and was presumed to be a bicornuate uterus with normal intrauterine pregnancy in the right horn. One day later, she was admitted to our department with abdominal pain, shortly leading to massive hemoperitoneum and hypovolemic shock. Uterine rupture was confirmed ultrasonically, followed by immediate laparotomy. Ruptured rudimentary horn with already expulsed pregnancy was encountered during surgery. Despite significa…

Focused assessment with sonography in trauma: a review of concepts and considerations for anesthesiology.

The use of point-of-care ultrasound in trauma provides diagnostic clarity and routinely influences management. A scanning protocol known as the Focused Assessment with Sonography in Trauma (FAST) has been widely adopted by trauma providers of all specialties. The FAST exam addresses a broad array of pathologic conditions capable of causing instability, including hemoperitoneum, hemopericardium, hemothorax, and pneumothorax. The exam is an integral component to the primary assessment of injured patients and an iconic application of point-of-care ultrasound. This review article aims to summarize …

Massive intra-abdominal bleeding caused by nontraumatic rupture of a vein in the colorenal ligament: report of a case.

We report a case of acute abdomen caused by nontraumatic intra-abdominal bleeding in a 38-year-old man. Emergency laparotomy revealed the source of bleeding as a vein in the right colorenal ligament. The rupture of this vein may have been attributable to shear stress after severe vomiting on the day before admission. Although the patient had a coagulation disorder secondary to early-stage liver cirrhosis, there were no signs of portal hypertension intraoperatively. This report describes an unusual cause of acute hemoperitoneum, highlighting the importance of including this life-threatening dis…

Spontaneous hematoma of the mesocolon.

Spontaneous hematoma of the mesocolon is a rare condition, mainly due to the rupture of a colic artery aneurysm. The authors report on two cases of spontaneous hematoma of the mesocolon and examine the relevant literature. The clinical presentation in our patients was, as indicated in the literature, non-specific, with the occurrence of acute abdomen (case 1) or mild abdominal pain (case 2). Only diagnostic imaging (contrast-enhanced CT scan) is capable of yielding an accurate diagnosis, specifying the size and location of the hematoma. A palpable mass or hemoperitoneum due to rupture should b…

Spontaneous uterine rupture at an unusual site due to placenta percreta in a 21-week twin pregnancy with previous cesarean section.

Spontaneous uterine rupture is a rare, but serious complication of placenta percreta. This case report is about a spontaneous uterine rupture at an unusual site due to placenta percreta in a 21-week twin pregnancy with previous cesarean section. A 30-year-old, G3, P2 woman was referred to our unit in the 21st week of a twin pregnancy with acute abdomen. An emergency laparotomy was performed with the diagnosis of uterine rupture and intra-abdominal hemorrhage. A significant hemoperitoneum was found, with both fetuses freely floating in the peritoneal cavity. A large transverse rupture at the po…

[Spontaneous splenic rupture: report of three new cases and review of the literature].

This study reports 3 new cases of spontaneous (or atraumatic) splenic rupture of the spleen, including two with massive hemoperitoneum and one with a secondary rupture of splenic infarct, and reviews the literature about this rare disease. These spontaneous ruptures are rare and potentially fatal. They result from infectious diseases (mainly mononucleosis and and paludism) and hematological diseases (mainly malignant hemopathies) in more than 50% of cases. Mortality is close to 20%, and includes some deaths occurring before diagnosis was established and postoperatives deaths, which can result …

Unexpected major venous injury in the setting of percutaneous arterial access for GORE TAG Thoracic Branch Endoprosthesis.

Inferior vena cava (IVC) injury during thoracic endovascular aortic repair is exceedingly rare. We present the case of an 80-year-old woman who developed a fatal IVC perforation at the iliac confluence during large-bore (24F) sheath insertion for thoracic endovascular aortic repair. Despite successful thoracic endograft deployment, the patient developed acute hypotension and hemorrhagic shock. Emergent exploratory laparotomy revealed hemoperitoneum from an IVC tear, likely caused by shearing forces transmitted through heavily calcified iliac arteries. This case highlights the importance of pre…

Splenic rupture after video-assisted thoracoscopic lobectomy: a case report.

This report details a rare splenic rupture following left video-assisted thoracoscopic lobectomy in a 50-year-old male. Intraoperative tachycardia (78 → 110 bpm) and postoperative refractory hypotension (56/33 mmHg) progressed to hemorrhagic shock (Hb 57 g/L) at 90 min post-op. E-FAST confirmed splenic rupture with hemoperitoneum, leading to emergent splenectomy for a 3-cm diaphragmatic surface laceration (3000 mL blood loss). Unique features include: (i) Traction-mediated injury despite intact diaphragm; (ii) Diagnostic delay from nonspecific early signs. This case underscores the need for vi…

Spontaneous splenic rupture from indeterminate dendritic cell proliferation: a case report.

Spontaneous splenic rupture is a rare and life-threatening phenomenon, usually associated with an underlying infectious, inflammatory, hemat … splenic subcapsular hematoma. Intraoperative findings from an emergency laparotomy revealed a large hemoperitoneum and a ruptured spleen. Microscopic sections identified numerous, mostly poorly formed, small nodules classified as a proliferation of indeterminate dendritic cell tumors.

Hemorrhagic shock revealing a spontaneous rupture of a splenic artery aneurysm in a young patient without prior medical history: A rare case and literature review.

Splenic artery aneurysm (SAA) is a rare condition, and its spontaneous rupture is life-threatening. It usually occurs in patients with known risk factors such as pregnancy or portal hypertension. We report the case of a 29-year-old male with no prior medical history who presented with sudden abdominal pain and hemorrhagic shock. Contrast-enhanced CT angiography revealed massive hemoperitoneum due to spontaneous rupture of a splenic artery aneurysm. Despite emergency surgical management, the patient died. This case is important because it demonstrates that splenic artery aneurysm rupture can oc…
