Hip and groin pain in the child athlete.

An increasing number of children are taking up sporting activities and at more competitive levels. For this reason (pediatric) radiologists should expect to receive greater numbers of requests from their orthopedic colleagues to image the athletic child who presents with hip or groin pain: "athletic pubalgia."Lower limb sports-related pathology is particularly common in sports such as ballet, football, hockey, rugby, and running. Injuries to the hip and groin may account for up to a quarter of injuries seen in athletic children and may be acute or chronic, osseous, cartilaginous, ligamentous,

A new technique for closed reduction of traumatic posterior dislocations of the hip: the 'PGI technique'.

Many techniques have been described for closed reduction of posterior hip dislocations, but most require forceful and sustained traction, assistants to stabilise the pelvis and awkward positioning of the surgeon and/or the patient. We describe a new technique of closed reduction that does not need traction-countertraction or special positioning and can be safely and effectively performed by one surgeon.Fifteen patients, mean age 36.4 years, were subjected to reduction. There were five type I, four type II and six type III dislocations; ipsilateral injuries included one knee dislocation with pa

Rising Volume of Periprosthetic Fractures After Primary Total Hip and Knee Arthroplasty: A Contemporary Epidemiologic Analysis.

BACKGROUND: Periprosthetic fractures following total hip arthroplasty and total knee arthroplasty are complications associated with substantial morbidity and mortality. As the volume of primary arthroplasty continues to increase in the United States, the incidence and overall burden of periprosthetic fractures are expected to rise accordingly. However, contemporary trends and future projections in emergency department (ED) visits and operative management for these injuries remain incompletely characterized. METHODS: There were three national databases that were queried to identify the nationa

Editorial Commentary: Postless Hip Arthroscopy May Help Optimize Outcomes, but Use of a Post Is Acceptable and Is Indicated in Select Cases.

Compared with postless arthroscopy, hip arthroscopy using a perineal post (post-assisted traction) is associated with increased risk of complications in 1% to 30% of patients, including pudendal neuropraxia (sexual dysfunction, dyspareunia, perineal pain, and/or numbness) and perineal soft-tissue injuries (vulvar bruising, tears, or scrotal swelling). Although these groin-related complications are usually transient in nature, they are significant and should be prevented if at all possible. Because of the sensitive nature of groin-related injuries after surgery, these complications also can go

Traumatic hip dislocation--a review.

Hip dislocations are uncommon injuries that result from high-energy mechanisms. These patients require careful trauma evaluation to rule out concomitant injuries. Early closed or open reduction that is performed within 6 hours and close radiological follow-up is recommended to obtain the best possible results. It is also essential to educate the patient regarding the potential sequelae and follow them for evidence of osteonecrosis and posttraumatic arthritis.

Development and verification of a protocol to quantify hip joint kinematics: an evaluation of ice hockey goaltender pads on hip motion.

BACKGROUND: The butterfly save technique is commonly used by ice hockey goaltenders and has recently been identified as a potential mechanis … ICAL RELEVANCE: Exceeding internal rotation range of hip motion may make goaltenders susceptible to hip injuries such as femoral acetabular impingement.

The Lumbopelvic-Hip Complex Contribution During Lower Extremity Screening Tests in Elite Figure Skaters.

CONTEXT: Figure skating requires power and stability for takeoff and landing from multirotational jumps and various on-ice skills. Repetitive forces may cause overuse injuries distally, making lumbopelvic-hip endurance, strength, and neuromuscular control imperative. OBJECTIVE: To compare lumbopelvic-hip endurance and neuromuscular control in elite figure skaters between sexes and landing and nonlanding limbs using common screening tests. DESIGN: Cross-sectional study. SETTING: US Olympic and Paralympic Training Center. PATIENTS OR OTHER PARTICIPANTS: Forty elite figure skaters (20 women,

Editorial Commentary: Flat Earth-Why 2-Dimensional Imaging Reveals Acetabular Retroversion's Minimal Impact of Arthroscopic Outcomes.

Acetabular retroversion (AR) has been found to be associated with the development of multiple symptomatic pathologies of the hip joint including femoroacetabular impingement syndrome, labral tears, chondral injuries, posterior instability, and the development of hip osteoarthritis. Many studies have showed good results with hip arthroscopy in patients with 2-dimensional imaging preoperatively utilized to diagnose AR. While plain radiographs can be helpful in preliminary evaluation for AR, its diagnosis can be more accurately made using 3-dimensional imaging. Surgeons should utilize 3-dimension

Periprosthetic Femur Fracture Around the Hip.

Periprosthetic femur fractures around the hip are one of the most common aseptic complications following total hip arthroplasty. Understanding the risk factors of periprosthetic femur fracture can aid surgeons in the prevention of these injuries. The Vancouver classification provides a reproducible description of the factors that should be considered in the treatment of patients with periprosthetic femur fractures: fracture site, implant stability, and bone stock. It is important for the surgeon to review the epidemiology and risk factors for periprosthetic femur fractures around the hip along

Arthroscopic Removal of a Free-Floating Bullet From the Hip Joint.

Gunshot injuries and bullet removal are extremely rare indications for hip arthroscopy. We present the case of a 22-year-old male with a free-floating bullet in the hip joint after a gunshot injury. A thorough imaging investigation was used to demonstrate the exact location of the foreign body. The bullet was removed by arthroscopic means under fluoroscopic guidance. The patient has been symptom-free for two years postoperatively. The tips and tricks of the technique are discussed. Hip arthroscopy is a minimally invasive technique to remove a free-floating bullet and avoid potential long-term
