Early application of an intermittent pneumatic compression device assists dilation of radiocephalic fistulas.

INTRODUCTION:: Radiocephalic fistulas are the preferred type of arteriovenous fistula due to their advantageous location. However, radiocephalic fistula maturation has historically been inferior to brachiocephalic fistulas. Research indicates that intermittent compression may aid in forearm vein dilation. Early use of a compression device may assist radiocephalic fistula maturation. OBJECTIVES:: This prospective, randomized, placebo controlled study evaluates device safety and the effect of intermittent compression on vein size of radiocephalic fistulas and brachiocephalic fistulas. METHODS:

A biomechanical comparison of transosseous-suture anchor and suture bridge rotator cuff repairs in cadavers.

BACKGROUND: Several biomechanical studies comparing open and arthroscopic rotator cuff repair methods have shown inferior performance of arthroscopic repairs. Suture anchor-augmented transosseous repairs and suture bridge repairs have shown superior biomechanical performance when compared with other methods, but these 2 repair methods have not been directly compared. HYPOTHESIS: There will be no difference in the biomechanical performance of the transosseous-suture anchor and suture bridge techniques. STUDY DESIGN: Controlled laboratory study. METHODS: Eight paired cadaveric shoulder specim

Hospital Outcomes among COVID-19 Hospitalizations with Acute Ischemic Stroke: Cross-Sectional Study Results from California State Inpatient Database.

Coronavirus disease 2019 (COVID-19) could be a risk factor for acute ischemic stroke (AIS) due to the altered coagulation process and hyperinflammation. This study examined the risk factors, clinical profile, and hospital outcomes of COVID-19 hospitalizations with AIS. This study was a retrospective analysis of data from California State Inpatient Database (SID) during 2019 and 2020. COVID-19 hospitalizations with age ≥ 18 years during 2020 and a historical cohort without COVID-19 from 2019 were included in the analysis. The primary outcomes studied were in-hospital mortality and discharge to

Long-term Effects of Androgen Deprivation in a Patient with Spinal and Bulbar Muscular Atrophy - A Case Report with 14 Years of Follow-up.

Spinal and bulbar muscular atrophy (SBMA) is a progressive hereditary neuromuscular disease caused by the testosterone-dependent accumulation of pathogenic polyglutamine-expanded androgen receptor protein. A 41-year-old man with SBMA received the androgen deprivation agent leuprorelin acetate for 7 years in clinical trials and underwent castration following the trial. Suppression of testosterone levels for 14 years resulted in a slower disease progression, as measured prospectively with quantitative measurements, than the historical control data reported in previous studies. This suggests that

[Invagination ileus--stages and progress of treatment].

Result from the historic development of the treatment of the intussusception was given an epidemiologic study of the different kinds of possible treatment of infantile intussusception from 1963 to 1988 including 246 affected children. Correlations are established between the duration of anamnesis, the general condition, form of intussusception and possible therapies. Besides indications and contraindications are recommended for hydrostatic and operative evagination as well as for treatment by resection. Furthermore, some conservative methods of treatment are briefly introduced, such as hydrost

Postoperative adjuvant mediastinal radiation in lung cancer.

Postoperative adjuvant mediastinal radiation therapy was used in a group of 18 patients with advanced (nine Stage 2, nine Stage 3) non-small cell carcinoma of the lung who had undergone curative resection. All patients had metastases to hilar and/or mediastinal nodes. The disease-free survival at 2 1/2 years was 55% for Stage 2 patients, suggesting a survival advantage compared to historical controls. No benefit was found in Stage 3 patients. A prospective randomized study of adjuvant radiation in node-positive patients is indicated.

Genetic sequencing of a 1944 Rocky Mountain spotted fever vaccine.

Rocky Mountain spotted fever (RMSF) is a rapidly progressive and often fatal tick-borne disease caused by Rickettsia rickettsii. Its discovery and characterization by Howard Ricketts has been hailed as a remarkable historical example of detection and control of an emerging infectious disease, and subsequently led to the establishment of the Rocky Mountain Laboratories (RML). Here, we examined an unopened bottle of a vaccine, labeled as containing RMSF inactivated by phenol-formalin of infected ticks, developed prior to 1944 at RML by DNA analysis using Illumina high throughput sequencing techn

Splenectomy and treatments of historical interest.

Hairy-cell leukaemia (HCL) was initially described as a distinct clinical entity in 1958. Various types of therapy have been evaluated since this initial description and several are currently important only for historical comparison. Hairy-cell leukaemia serves as an example of rapid progress in the development of effective therapeutic strategies. Splenectomy was the first effective treatment because it resulted in an improvement in cytopenias and relief from symptomatic splenomegaly. However, complete remission of the disease was observed only very rarely. Splenectomy was considered an approp
