Standardizing default electronic health record tools to improve safety for hospitalized patients with Parkinson's disease.

Electronic Health Record (EHR) systems are often configured to address challenges and improve patient safety for persons with Parkinson's disease (PWP). For example, EHR systems can help identify Parkinson's disease (PD) patients across the hospital by flagging a patient's diagnosis in their chart, preventing errors in medication and dosing through the use of clinical decision support, and supplementing staff education through care plans that provide step-by-step road maps for disease-based care of a specific patient population. However, most EHR-based solutions are locally developed and, thus

Implementation of wastewater-based epidemiology in a Northern California acute care hospital: a pilot study, June-July 2024.

AIM: The aim of this study was to evaluate the feasibility and performance of wastewater-based healthcare epidemiology for monitoring respiratory viruses and antimicrobial resistance genes. METHODS: We conducted a pilot study at an 800-bed academic quaternary care hospital in Northern California. Composite wastewater samples were collected weekly over three timepoints (June-July 2024) from the main hospital outflow. Droplet digital polymerase chain reaction was performed to quantify nucleic acids from respiratory viruses (SARS-CoV-2, influenza A, and respiratory syncytial virus) and antimicro

Evaluating the process of care for persons admitted to Toronto area hospitals with acute severe ulcerative colitis.

BACKGROUND: Acute severe ulcerative colitis (ASUC) is associated with significant morbidity. In patients with ulcerative colitis (UC), the estimated lifetime risk of developing severe colitis is 25%. Several gastrointestinal societies have provided recommendations on pathways of care for managing ASUC. The degree to which they are adhered to in different care settings remains unclear. METHODS: We conducted a retrospective review using data from 7 acute-care hospitals collected through the general medicine inpatient initiative (GEMINI), a hospital research collaborative that collects administr

Respectful disposition in early pregnancy loss.

This article discusses an issue rarely seen in the professional literature: the tangible ways nurses can respect a woman's needs following miscarriage by ensuring the safe handling and disposition of fetal tissue or remains. Concepts of personhood, place, and protection are important for nurses to understand within the context of a woman's response to miscarriage. Hospitals or clinics that foster a culture of respectful fetal disposition should have a system in place to bury tissue or fetal remains in a designated area; in fact, several states have enacted laws that regulate what hospitals and

Cost of allogeneic bone marrow transplantation in four diseases.

The cost of bone-marrow transplantation is compared in 4 diseases: acute myelogenous leukaemia, severe combined immunodeficiency, severe aplastic anaemia and chronic granulocytic leukaemia. Hospital cost components directly related to the clinical protocols applied are valorized. Results confirm the well-known fact that bone-marrow transplantation is a costly technique. The unit cost of a transplantation can vary from 1 to 2 between departments for the sole reason that patients treated are not suffering from the same illness. For one disease, the unit cost may vary from 1 to 2.7 when post-graf

Privatized Health Care System in Times of Crisis: South Korea's Health Care System Response to the COVID-19 Pandemic.

The COVID-19 pandemic was a global public health crisis that demanded a "whole-of-society" response. In many countries, coordinating efforts across public and private health care sectors proved challenging. Yet South Korea maintained one of the world's lowest excess mortality rates despite having a predominantly privatized health care system, with 90.3% of hospital beds privately owned. This outcome was enabled in part by the government's strategy of disproportionately mobilizing and repurposing public hospitals as dedicated COVID-19 treatment facilities. These hospitals, historically few in n

Influence of socioeconomic status on community-acquired pneumonia outcomes in elderly patients requiring hospitalization: a multicenter observational study.

BACKGROUND: The associations between socioeconomic status and community-acquired pneumonia outcomes in adults have been studied although studies did not always document a relationship. The aim of this multicenter observational study was to determine the association between socioeconomic status and community-acquired pneumonia outcomes in the elderly, in the context of a public health system providing universal free care to the whole population. METHODS: A total of 651 patients aged > or =65 years hospitalized due to community-acquired pneumonia through the emergency departments of five Spanish

COVID-19 host genetic risk study conducted at community pharmacies: Implications for public health, research and pharmacists' scope of practice.

Community pharmacists serve a large, diverse population of patients, resulting in the potential to utilize community pharmacies as recruitment sites for clinical research. Beyond traditional roles as one of the most accessible health care professionals in the US healthcare system, pharmacists have played a major role in the response to the COVID-19 pandemic, administering hundreds of thousands of vaccines and tests. However, less emphasis is placed on the ability to leverage community pharmacies as research-focused partners for clinical studies. In this study, we demonstrate the feasibility an

Humanization of nursing care in hospitalized patients with non-communicable diseases: a phenomenological qualitative study.

BACKGROUND: The inpatient care of patients with non-communicable chronic diseases (NCDs) often involves extended stays, where the human quality of nursing care is as relevant as the technical interventions. However, dehumanization in daily practice, manifested in impersonal routines, deficient communication, and inattention to the patient’s values, erodes trust and wellbeing. Guided by Jean Watson’s theory of human caring, this study explored the lived experiences of hospitalized patients in relation to nursing care in Mexican public hospitals. METHODS: A qualitative study using a hermeneutic-

After neonatal care, what next? A qualitative study of mothers' post-discharge experiences after premature birth in Kenya.

BACKGROUND: Approximately 15 million babies are born prematurely every year worldwide. Sub-Saharan Africa (SSA) and Asia account for more than half of the global preterm deliveries. Prominent healthcare structural and socio-economic factors in SSA, for example poverty and weak health systems, amplify vulnerabilities for mothers and premature babies; often leading to poor outcomes. Post-discharge mortality rates are high, and readmission is common. For mothers of premature babies, the transition home from hospital is marked by challenges and uncertainties. This study explored the post-discharge

Effect of electronic medication reconciliation at the time of hospital discharge on inappropriate medication use in the community: an interrupted time-series analysis.

BACKGROUND: It is unclear if enhanced electronic medication reconciliation systems can reduce inappropriate medication use and improve patient care. We evaluated trends in potentially inappropriate medication use after hospital discharge before and after adoption of an electronic medication reconciliation system. METHODS: We conducted an interrupted time-series analysis in 3 tertiary care hospitals in London, Ontario, using linked health care data (2011-2019). We included patients aged 66 years and older who were discharged from hospital. Starting between Apr. 13 and May 21, 2014, physicians

Artificial Intelligence in Pediatric Imaging: A Primer for Pediatric Clinicians.

Artificial intelligence (AI) is reshaping healthcare, and radiology is at the forefront for adoption. Increasing demand for imaging, complex protocols, quantitative image analysis, and workforce shortages drive AI development. However, clinical translation in pediatric imaging still lags its use in general radiology. Only a small number of approved tools carry explicit pediatric indications, and these focus on narrow tasks. In chest imaging, deep learning models can detect pneumonia, tuberculosis, and misplaced lines or tubes, though performance depends on pediatric-specific training and local

How do we teach blood donor center operations without a blood donor center?

BACKGROUND: It is increasingly difficult to maintain a hospital-based blood donor center. Cost pressures, shifting healthcare priorities, and a dwindling donor base have shifted blood collection toward larger economies of scale achieved by larger blood suppliers. Many transfusion medicine fellowships are within healthcare systems without a dedicated donor center. Yet, the management of blood donors and a blood product collection program is essential knowledge for transfusion medicine professionals. We describe an approach to teaching blood donor center operations and management in a fellowship
