Evaluation of Drug Interactions in Hospitalized Patients with Respiratory Disorders in Greece.

Background: Patients with respiratory disorders often have additional diseases and are usually treated with more than one medication to manage their respiratory conditions as well as additional comorbidities. Thus, they are frequently exposed to polypharmacy (≥5 drugs), which raises the risk for drug-drug interactions (DDIs) and adverse drug reactions (ADRs). In this work, we present the results regarding the prevalence of DDIs in hospitalized patients with respiratory disorders in Greece. Methods: A 6-month descriptive single-center retrospective observational study enrolled 102 patients with

How to get care right for people with learning disabilities in the emergency department: ask and engage.

Healthcare professionals are legally obliged to make reasonable adjustments to ensure that people, including those with learning disabilities, can access appropriate and timely care. However, although people with learning disabilities are high users of general health services, staff have little understanding of learning disability with a negative effect on patient experience and outcomes. This article explores some of the challenges experienced by people with learning disabilities in the emergency department (ED). Each section focuses on a different aspect of how to adjust the delivery of heal

Patients With Opioid Use Disorder Have Increased Readmission Rates, Emergency Room Visits, and Costs Following a Hallux Valgus Procedure.

BACKGROUND: Patients with a history of opioid use disorder (OUD) tend to have more complications, higher readmission rates, and increased costs following orthopaedic procedures. This study evaluated patients undergoing hallux valgus correction for their odds of increased (1) readmission rates, (2) emergency room (ER) visits, and (3) costs. METHODS: Patients undergoing hallux valgus corrections with OUD history were identified using a national Medicare administrative claims database of approximately 24 million orthopaedic surgery patients. OUD patients were matched to non-opioid use disorder (

[Therapeutic touch and anorexia nervosa].

An innovative practice, therapeutic touch has been used for around ten years in the treatment of eating disorders. Delivered by nurse clinicians having received specific training, this approach is based on nursing diagnoses which identify the major symptoms of this pathology. The support is built around the body and its perceptions. Through the helping relationship, it mobilises the patient's resources to favour a relationship of trust, a letting-go, physical, psychological and emotional relaxation, and improves the therapeutic alliance.

Why is Pain Still Under-Treated in the Emergency Department? Two New Hypotheses.

Across the world, pain is under-treated in emergency departments (EDs). We canvass the literature testifying to this problem, the reasons why this problem is so important, and then some of the main hypotheses that have been advanced in explanation of the problem. We then argue for the plausibility of two new hypotheses: pain's under-treatment in the ED is due partly to (1) an epistemic preference for signs over symptoms on the part of some practitioners, and (2) some ED practices that themselves worsen pain by increasing patients' anxiety and fear. Our argument includes the following logic. So

Working under threat: fear and nurse-patient interactions in a forensic psychiatric setting.

The purpose of this article is to present the results of a study conducted in a Canadian medium-security forensic psychiatric facility. The primary objective of this qualitative research was to describe and comprehend how fear influences nurse-patient interactions in a forensic psychiatric setting. Eighteen semistructured interviews with nurses were used as the primary source of data for analysis. In brief, the results from this research indicate, as other researchers have demonstrated, that within this highly regimented context, nurses are socialized to incorporate representations of the pati

The (dis)appearance of the dying patient in generalist hospital and care home nurses' talk about the patient.

This article explores interview data from a study of 50 Norwegian generalist nurses' focus group accounts of caring for dying patients in the hospital and care home. An eclectic discourse analytic approach was applied to nurses' accounts of the patient and three discursive contexts of reference to the patient were identified: the 'taken as read' patient, the patient paired with particular characteristics and the patient as psychologically present. Talk about the patient falls mainly into the first two contexts, which position the patient in relation to three closely related discursive processe

False allegations of sexual misconduct: clinical and institutional considerations.

That some therapists engage in sexual misconduct with patients has been unequivocally established. In recent years, however, some patients' allegations of sexual misconduct have been determined to be false. This paper describes four cases in which hospitalized psychiatric patients made false allegations. Such allegations may result from patients' seeking to gain monetarily or in other ways or from a desire for retaliation or revenge against a clinician who they believe has scorned, abandoned, or otherwise mistreated them. In other cases, especially among patients with a history of severe traum

An observational study of associations between nurse behaviour and violence in psychiatric hospitals.

A cyclical model of violence to psychiatric nurses is proposed in this paper and a partial test of the model is reported. The model suggests that stress induced by exposure to violence leads to impaired staff performance and adoption of behaviours which make the re-occurrence of violence more likely. We tested the proposal that certain staff behaviours (e.g. expressing verbal hostility) are associated with an increased risk of assault by observing nurse-patient interaction on 12 psychiatric wards (n = 103 nurses). The verbal and physical interaction of (1) assaulted staff, (2) staff on violent

[Indirect forms of iatrogenic violence in clinical psychiatry].

The paper tries to describe often neglected forms of iatrogenic violence in clinical psychiatry. These examples are different from those forms of violent treatment in psychiatry, which are commonly known and discussed (medication, fixation, isolation). One aim of the paper is to emphasize determinants and modalities of emotional and verbal violence used without being noticed in clinical routine work. The idea is supported that the patient may voluntarily estimate his/her closeness to the therapist, who, on the other hand, should try to abandon therapeutic overactivity. An atmosphere should be

Accountability: a right of the rape victim.

Accountability by the professional is especially important in a crisis situation. Rape is experienced by victims as a life threatening situation and it thus triggers an acute stress reaction which requires them to go through a long-term reorganization process. As a crisis situation, it makes them hypersensitive to the attitudes of those people to whom they turn for help and assistance. Clients--in this case rape victims--benefit when professionals are careful to be accountable to them. The victim counselors defined and redefined their role to the victims through their work with the victim as w

Consultation-Liaison psychiatric service delivery: results from a European study.

The reported findings of the European Consultation-Liaison Workgroup (ECLW) Collaborative Study describe consultation-liaison service delivery by 56 services from 11 European countries aggregated on a C-L service level. During the period of 1 year (1991), the participants applied a standardized, reliability tested method of patient data collection, and data were collected describing pertinent characteristics of the hospital, the C-L service, and the participating consultants. The consultation rate of 1% (median; 1.4% mean) underscores the discrepancy between epidemiology and the services deliv

A Community Hospital NICU Developmental Care Partner Program: Feasibility and Association With Decreased Nurse Burnout Without Increased Infant Infection Rates.

BACKGROUND: Volunteers can provide staff-directed sensory inputs to infants hospitalized in the NICU, but research on volunteer programs is limited. PURPOSE: To evaluate the feasibility of a developmental care partner (DCP) program in a level III NICU and determine its relationship with provider burnout and infant infection rates. METHODS: DCPs were trained to provide sensory input to infants, based on the behavioral cues observed by the occupational therapists and nursing staff, in medically stable infants. Feasibility was assessed by documenting the process of training and utilizing volunt

Hospital outbreak of Salmonella virchow possibly associated with a food handler.

A foodborne outbreak of salmonella infection at a private hospital in London in 1994 was found to be associated with eating turkey sandwiches prepared by a food handler. One patient, nine staff, and a foodhandler's baby were confirmed to have Salmonella enterica serotype virchow, phage type 26 infection. The attack rate was estimated to be 5% among the approximately 200 patients and staff at risk. A food handler reportedly became ill days after, but her baby days before, the first hospital case. Although it appeared to be a single outbreak, antibiogram analysis, supplemented by plasmid profile

Using patients' experiences to identify priorities for quality improvement in breast cancer care: patient narratives, surveys or both?

BACKGROUND: Patients' experiences have become central to assessing the performance of healthcare systems worldwide and are increasingly being used to inform quality improvement processes. This paper explores the relative value of surveys and detailed patient narratives in identifying priorities for improving breast cancer services as part of a quality improvement process. METHODS: One dataset was collected using a narrative interview approach, (n = 13) and the other using a postal survey (n = 82). Datasets were analyzed separately and then compared to determine whether similar priorities for

Consultation-Liaison Case Conference: Challenges of Managing Severe Opioid Use Disorder in the Hospital.

Consultation-liaison psychiatrists are frequently consulted to assess and manage severe opioid use disorder in hospitalized patients requiring treatment for complications related to their opioid use disorder. We describe the case of a 43-year-old male with severe opioid use disorder who recently underwent bilateral amputations for his leg wounds at another hospital and left before medically advised. He subsequently presented to our hospital for pain and was admitted for ongoing infection. An understanding of the ethical considerations surrounding patient-directed discharge (previously referred

[Suicide in hospitalized patients and medical liability].

Suicide is a complex phenomenon that has accompanied human beings throughout history. Its strong association with mental disorders led to its medicalization and psychiatrists became the physicians in charge of diagnosing and treating patients at risk of suicide. This article discusses the potential limitations that psychiatrists may face when diagnosing suicide risk and providing optimal care. Evidence of the eventual inevitability of suicide and the tension that may arise between providing optimal treatments on the one hand and preserving the rights of patient's autonomy and dignity on the ot

Toxoplasma encephalitis in HIV/AIDS patients admitted to the Douala general hospital between 2004 and 2009: a cross sectional study.

BACKGROUND: It is estimated that about a third of the world's population is chronically infected with Toxoplasma gondii. Toxoplasma encephalitis (TE), which occurs as a reactivation of quiescent chronic infection, is one of the leading causes of central nervous system (CNS) infection in AIDS. Its diagnosis in most centres still remains difficult. We opted to describe the clinical and radiological features of TE as well as in-hospital outcome and its associated factors. METHODS: We carried out a cross sectional study on the clinical case notes of adult patients admitted and treated for TE at t
