Four-part displaced proximal humeral fractures: operative treatment using Kirschner wires and a tension band.

Thirty-five patients with four-part displaced proximal humeral fractures and fracture-dislocations were retrospectively reviewed from the years 1977-1990. Thirty-three patients were available for study. All fractures were treated by open reduction and internal fixation allowing early motion. Surgical technique was as atraumatic as possible with the goal of restoring normal anatomy of the proximal humerus with a minimal fixation system. The fixation device consisted of two modified Kirschner wires introduced through the tuberosities and reinforced by a tension band wiring. Neer's criteria were …

[Importance of initial fracture crack width in minimally dislocated fractures of humeral lateral condyle in children for evaluation of fracture stability and treatment choice].

INTRODUCTION: The fracture of the external condyle is the most common fracture of the distal end of the humerus. This is an intraarticular fracture, which, if not properly treated, can cause serious complications, difficult to treat. OBJECTIVE: To define the importance of the initial width of the fracture crack for the evaluation of stability of the minimally dislocated fractures of the humeral lateral condyle and for the selection of the method of treatment. METHODS: The target group included the children with minimally dislocated fractures or fractures of uncertain stability. The number of

The Fracture Anatomy, Separation, and Tuberosity involvement (FAST) classification of displaced proximal humeral fractures using 3D-CT.

AIMS: The aim of this study was to assess displaced proximal humeral fractures using 3D-CT. The hypothesis was that it would be possible to classify these fractures systematically based upon recurring anatomical patterns of injury, and that this would be useful in predicting the outcomes. METHODS: CT scanning was performed in 993 patients who sustained a proximal humeral fracture between September 2014 and April 2020, with anonymized measurements of the anatomical features and displacement of the head-shaft fracture, and the extent of the involvement of the tuberosities in 3D reconstructed vi…

Does helical plating for proximal humeral shaft fractures benefit bone healing? - an in silico analysis in fracture healing.

OBJECTIVES: Helical plating is an established alternative to straight plating for humeral shaft fractures in order to prevent iatrogenic radial nerve damage. However, a previous biomechanical investigation indicated differences in fracture healing for helical plating due to a potential shift of interfragmentary movements compared to straight plating. Therefore, fracture healing simulations were performed to assess any differences in bone healing of helical vs. straight plating. METHODS: A systematic workflow for fracture healing analytics was created, covering essential steps of bone modellin

High Median Nerve Entrapment by Fracture Callus in Surgically Treated Pediatric Supracondylar Humeral Fracture: A Case Report.

CASE: We describe a patient with incomplete high median nerve palsy after surgical treatment of pediatric supracondylar humeral fracture (SCHF). Preoperative images after 11 months after the surgical treatment of the SCHF showed rotational deformity and an isolated median nerve entrapped between the proximal humerus anteriorly and callus posteriorly in the bony tunnel of the distal humerus. Two years after neurolysis, he showed clinical recovery. Four cases similar to our case were reported previously. CONCLUSION: Common pediatric fractures may cause unusual neuropathy. Rotational deformity o

[Proximal humeral fractures in the elderly : Quality of life, clinical results and institutionalization following primary reverse fracture arthroplasty].

BACKGROUND: Treatment of humeral head fractures in the elderly remains a challenge. Within the framework of demographic change the incidence as well as the direct and indirect consequences of injuries will rise. The analysis of an optimal treatment should include functional parameters as well as global health parameters, e.g. quality of life. OBJECTIVE: The aim of this study was the evaluation of functional outcome, institutionalization (necessity of placement in a residential care home for the elderly, even if temporary), the influence of cognitive deficits, quality of life and the pattern o

Fracture displacement and screw cutout after open reduction and locked plate fixation of proximal humeral fractures [corrected].

BACKGROUND: Fixation of proximal humeral fractures is challenging. Locking plate technology offers mechanical advantages for treating unstable fractures in weak bone. In this study, we assessed the radiographic and clinical results of a single surgeon's experience treating proximal humeral fractures with a locked proximal humeral plate. METHODS: Fifty-three adult patients with a displaced proximal humeral fracture were treated with a proximal humeral locking plate over a forty-five-month period. A standard postoperative rehabilitation regimen was followed. Radiographs were made at two weeks, …

An Acta Orthopaedica educational article: Treatment of adult patients with a humeral shaft fracture.

This educational review outlines the core principles of humeral shaft fracture (HSF) management and is designed for orthopedic trainees, general orthopedic surgeons, emergency physicians, and allied health professionals who participate in the acute or postoperative care of patients with HSFs. The content integrates the authors' expert opinion with the current evidence. Humeral shaft fractures account for 1-3% of adult fractures, most often resulting from low-energy falls in older adults or high-energy trauma in younger patients. Although open fractures and neurovascular injuries are rare, prim

A Rare Case of Intramediastinal Humeral Head Fracture Dislocation Through the Thoracic Inlet.

Intramediastinal fracture dislocation of the humeral head due to a blunt trauma is very rare. It may be accompanied by local and systemic injuries associated with high-energy trauma. Only one case has been reported in the literature; therefore, an appropriate treatment modality remains unclear. We present a case of a 71-year-old woman involved in an accidental fall who sustained an intramediastinal humeral head fracture dislocation through the thoracic inlet, without damaging the chest wall, causing the thoracic duct rupture and requiring three consecutive surgical procedures as treatment.

Computed tomography versus radiography for assessment of canine humeral condylar fracture malreductions.

AIMS: To compare the accuracy of radiography and CT to diagnose anatomical surgical reduction of lateral humeral condylar fractures using an ex vivo canine model. METHODS: Ten right and left pairs of thoracic limbs were obtained from fresh canine cadavers. Lateral humeral condylar fractures were created and reduced, with or without an articular step-off (0-, 1-, 2-, 3-mm in a caudal or distal direction), and stabilised using a transcondylar screw and a supracondylar pin. Mediolateral and craniocaudal radiographs of each limb were taken, followed by CT imaging in a sagittal, dorsal, and transv

Non-operative management of humeral periprosthetic fracture after stemless shoulder arthroplasty.

PURPOSE: Periprosthetic fractures around a stemless implant often involve lesser and greater tuberosities with a well-fixed implant in the metaphysis. This exposes the surgeon to unique questions and challenges as no surgical option (open reduction and internal fixation or revision to a stem) appears satisfactory to address them. Purpose of this study was to evaluate the clinical outcomes after non-operative management of periprosthetic fractures after stemless shoulder arthroplasty. METHODS: A retrospective multicenter study was conducted to identify all patients who had sustained non-operat

Preoperative vascular assessment of patients with a supracondylar humeral fracture and a perfused, pulseless limb.

The 'pink, pulseless hand' is often used to describe the clinical situation in which a child with a supracondylar fracture of the humerus has normal distal perfusion in the absence of a palpable peripheral pulse. The management guidelines are based on the assessment of perfusion, which is difficult to undertake and poorly evaluated objectively. The aim of this study was to review the available literature in order to explore the techniques available for the preoperative clinical assessment of perfusion in these patients and to evaluate the clinical implications. A systematic literature review w

Osteoporosis is the cause of spontaneous humeral fracture in dairy cows from New Zealand.

Outbreaks of humeral fractures in dairy cows have been reported in New Zealand for several years. Gross, histologic, and histomorphometric findings in the humerus from primiparous cows with spontaneous humeral fracture were compared to age-matched control cows. Affected cows had a complete nonarticular spiral fracture of the humerus. Histologically affected humeri had a thicker growth plate with abnormal architecture, thinner cortex with increased abnormal resorption, increased resorption in the distal humerus, decreased trabecular density, abnormal trabecular architecture, presence of growth …
