[Significance of shunt efficacy decision of SPECT on idiopathic normal pressure hydrocephalus].

PURPOSE: The purpose of this study was to investigate the significance of the shunt-effect evaluation of SPECT in idiopathic normal pressure hydrocephalus (iNPH). SUBJECTS AND METHODS: The subjects were 15 patients with possible iNPH, aged 62-83 (mean 75.3, the ratio of males to females to 6:9), who were treated at our department during the period from June to September, 2004. All patients received the lumbar tap test (LTT) at the outpatient section before surgery. An L-P shunt was conducted on patients whose the LTT positive or negative with cerebrospinal fluid outflow resistance value (Ro)

Does the ventricle size change after shunt operation of normal-pressure hydrocephalus?

The aim of this study was to determine whether ventricular size correlates with a positive clinical outcome following shunt placement. Hydrostatic valves (Dual-Switch-Valves) were implanted in 80 patients with NPH at Unfallkrankenhaus Berlin between September 1997 and January 2002. One year postoperatively, these patients underwent computerized tomography scanning, and their ventricular size was ascertained using the Evans-Index. Among 80% of the patients who showed no postoperative change in ventricular volume, 59% nonetheless had good to excellent clinical improvements, 17% satisfactory clin

[Shunt operation versus endoscopic ventriculostomy in normal pressure hydrocephalus: diagnostics and outcome].

In contrast to shunt operation the indication for an endoscopic ventriculostomy in patients diagnosed for normal pressure hydrocephalus is not scientifically established. From September 1997 to October 2001 we operated on 79 patients diagnosed for normal pressure hydrocephalus. Diagnosis was established by means of the intrathecal lumbal or ventricular infusion test, the cerebrospinal fluid tap test and MRI-CSF flow studies pre- and post-operatively. In 60 patients (76 %) we implanted a ventriculo-peritoneal shunt (Miethke Dual-Switch valve), and in 15 patients (19 %) we performed the endoscop

Idiopathic intracranial hypertension and idiopathic normal pressure hydrocephalus: diseases with opposite pathogenesis?

Idiopathic intracranial hypertension (IDICH) and idiopathic normal pressure hydrocephalus (IDNPH) are disorders of the circulation of the cerebrospinal fluid (CSF) with opposite diagnostic features, obscure etiology but the same treatment: CSF drainage. In both diseases a large quantity of CSF exists: equilibrated by the brain edema in IdICH with high ICP and not equilibrated by brain pressure, and the ventricles are enlarged in IdNPH. The same anatomical structure involved in both diseases is the ventricular wall and this represents a true barrier, a glial-ependymal barrier and it acts as a b

Adjustable valves in normal-pressure hydrocephalus: a retrospective study of 218 patients.

OBJECTIVE: We sought to assess the value of adjusting shunt valve opening pressure, complications, and outcomes with the use of an adjustable shunt valve in the treatment of patients with normal-pressure hydrocephalus (NPH). METHODS: In a single-center retrospective study, 231 adjustable valves (range, 30-200 mm H(2)O) were the first shunt implantations in 147 patients with idiopathic NPH (INPH) and 71 patients with secondary NPH (SNPH). The effect of adjustment on gait disturbance, cognitive impairment, urinary incontinence and other symptoms were evaluated, and an improvement index was crea

Intracranial cerebrospinal fluid distribution and its postoperative changes in normal pressure hydrocephalus.

BACKGROUND: This study was conducted to investigate the usefulness of intracranial cerebrospinal fluid (CSF) volume measurement using MR-based methods in the management of patients with normal pressure hydrocephalus (NPH). METHODS: The study group comprised 19 patients with NPH who showed a favorable outcome after ventricular shunting, 15 normal volunteers (NV), and 15 patients with cerebrovascular disease (CVD). A 3D-fast asymmetric spin echo MR imaging sequence and the region-growing method were used to extract the CSF space from MR images. Ventricular volume (VV) and intracranial CSF volum

In contrast to shunt operation the indication for an endoscopic ventriculostomy in patients diagnosed for normal pressure hydrocephalus is not scientifically established. From September 1997 to March 1999 we operated on 36 patients diagnosed for normal pressure hydrocephalus. Diagnosis was established by means of the intrathecal lumbar or ventricular infusion test, the cerebrospinal fluid tap test and MRI-CSF flow studies pre- and post-operatively. In 30 patients (83%) we implanted a ventriculo-peritoneal shunt, and in 6 patients (17%) we performed the endoscopic assisted third ventriculostomy

Comparison of Pcsf monitoring and controlled CSF drainage diagnose normal pressure hydrocephalus.

We evaluated 86 patients for possible normal pressure hydrocephalus (NPH) by: 1) CSF pressure (Pcsf) monitoring and analysis for percent of time with A or B-waves, and 2) controlled CSF drainage for 3 days via a lumbar subarachnoid catheter. Clinical outcome after CSF drainage and shunt surgery was assessed as change of clinical exam, with grades of none, minor, moderate, or marked change. For outcome analysis in 47 patients after shunt surgery, NPH was defined as moderate or marked clinical improvement. We assessed the diagnostic discrimination of percent-of-time thresholds of A and B-waves f

[Clinically suspected normal-pressure hydrocephalus diagnosis--current status of diagnosis and therapy].

Since the first description of the disease normal pressure hydrocephalus (NPH), there has been a great progress in the diagnosis of this disease. Taking into account that the age of the population is rising the incidence and social medical importance is increasing. Modern neuroradiological techniques are visualising a ventricular enlargement but a definitive diagnosis cannot be drawn. The intrathecal infusion tests represent a save and valid tool while measuring the cerebrospinal fluid dynamics. Shunting is the only possible therapy in NPH. Complications from shunting are still a great problem

The predictive value of cerebrospinal fluid tap-test in normal pressure hydrocephalus.

Eighteen patients (mean age of 66.5 years) with normal pressure hydrocephalus (NPH) underwent a ventriculo-peritoneal shunt surgery. Prior to operation a cerebrospinal fluid tap-test (CSF-TT) was performed with measurements of gait pattern and psychometric functions (memory, visuo-motor speed and visuo-constructive skills) before and after the removal of 50 ml CSF by lumbar puncture (LP). Fifteen patients improved and 3 were unchanged after surgery. Short duration of disease, gait disturbance preceding mental deterioration, wide temporal horns and small sulci on CT-scan were associated with go

Psychiatric manifestations of normal-pressure hydrocephalus: a short review and unusual case.

Normal-pressure hydrocephalus (NPH) is a common cause of potentially reversible dementia. It can present with psychiatric manifestations that may hinder its diagnosis. A 68-year-old man presented with a paranoid psychosis and mild cognitive impairment, but not neurological signs or classic "triad." Gait disturbance and urinary incontinence developed later in the course of illness after the diagnosis of NPH had already been made on computed tomographic (CT) scanning. A lumbo-peritoneal shunt was performed, followed by full remission of psychotic symptoms, as well as considerable improvement in

Dutch normal pressure hydrocephalus study: baseline characteristics with emphasis on clinical findings.

We present the baseline characteristics of 101 patients with normal pressure hydrocephalus (NPH), entering a study that evaluates the diagnostic reliability of CSF outflow resistance. Patients were assessed by a gait scale consisting of 10 features of walking and the number of steps and seconds necessary for 10 m, a dementia scale comprising the 10 word test, trail making, digit span and finger tapping, the modified Mini Mental State Examination (3MSE) and the modified Rankin scale (MRS). Inclusion criteria were a gait and dementia scale ≥ 12 (range 2-40), a MRS ≥ 2 and a communicating hydroce

Normal-pressure hydrocephalus with misleading features of irreversible dementias: a case report.

An 85-year-old man presented with the clinical triad (gait instability, dementia, and bladder and bowel incontinence), the ventriculomegaly, and the normal CSF pressure that characterize normal-pressure hydrocephalus (NPH). Diagnostic uncertainty was raised by an unusually rapid onset and a lack of initial response to CSF tap tests. Additionally, periodic sharp waves on EEG suggested the possibility of Creutzfeldt-Jakob disease, and positron emission tomography (PET) demonstrated a pattern of cerebral hypometabolism typical of Alzheimer's disease. Nevertheless, the diagnosis of NPH was support

Normal-pressure hydrocephalus: evaluation with cerebrospinal fluid flow measurements at MR imaging.

PURPOSE: To evaluate magnetic resonance (MR) imaging-based quantitative phase-contrast cerebrospinal fluid (CSF) velocity imaging for prediction of successful shunting in patients with normal-pressure hydrocephalus (NPH). MATERIALS AND METHODS: Eighteen patients (mean age, 73 years) with NPH underwent routine MR imaging and CSF velocity MR imaging before ventriculoperitoneal (VP) shunting. The calculated CSF stroke volume and the aqueductal CSF flow void score were compared with the surgical results. RESULTS: All 12 patients with CSF stroke volumes greater than 42 microL responded favorably

[Pure red cell aplasia with thymoma, myasthenia gravis and normal pressure hydrocephalus].

Cases of pure red cell aplasia with thymoma and myasthenia gravis are rare. We described a patient who had concomitant pure red cell aplasia, thymoma, myasthenia gravis and a normal pressure hydrocephalus. A 63-year-old man with disturbances of gait, left blepharoptosis and anemia was presented to our hospital. Laboratory examination on admission revealed severe anemia. Bone marrow aspirates showed erythroid hypoplasia of marked degree. Chest x-ray and chest CT revealed a tumor to the right of cardiac wall. The tensilon chloride test and antiacetylcholine receptor antibody were positive. A hyd

Motor disturbances in normal-pressure hydrocephalus. Special reference to stance and gait.

Motor disturbances in 16 patients with normal-pressure hydrocephalus were assessed by quantitative registration of handwriting, fine movements of the hand, hand tremor, postural instability, and gait. Tremor intensity was measured using an accelerometer and electronic integration of the accelerometer curves. Postural instability was measured on a computer-assisted force-plate, and computerized analysis of gait was made using an instrumented treadmill. Severe disturbances in motor performance in the upper extremities and postural stability were found. The gait of the hydrocephalic patients was

Gait apraxia in normal-pressure hydrocephalus: patterns of movement and muscle activation.

We made gait recordings in 11 patients with normal-pressure hydrocephalus. Sagittal rotations in hip, knee, and ankle joints were determined with intermittent light photography or with polarized light goniometry. In eight patients, gait muscle activation was determined with integrated surface EMG from six leg muscles on one side. We assessed the patterns of movement and muscle activity by averaging data from 20 gait cycles. Reduced speed and range of movements, short steps, small foot-floor clearance, and low swing-to-stance ratio were typical. The activation of the calf muscles was regularly

Differential diagnosis between normal pressure hydrocephalus and cerebral atrophy on computed tomography.

A brain computed tomography--cisternographic correlation of 48 cases suspected of having normal pressure hydrocephalus (NPH) was undertaken, observing that the enlargement of temporal horns correlates with abnormal cisternograms compatible with NPH, and appears to be a reliable differential feature from cases of cerebral atrophy with "ex-vacuo hydrocephalus". The results of this correlation and typical images are presented.
