Prolonged hypercalcemia induced by teriparatide: a case report and literature review.

Teriparatide is indicated for the treatment of osteoporosis with a high risk of fractures. While transient hypercalcemia is a commonly reported side effect, we report a case of prolonged hypercalcemia associated with teriparatide. The patient was a 76-year-old female who had postmenopausal and glucocorticoid-induced osteoporosis on the background of rheumatoid arthritis since 2005. After failing treatment with etidronate, alendronate, and denosumab in view of minimal improvement in bone density and multiple fragility fractures, she was started on teriparatide on 5 May 2023. Serial serum calciu…

Treatment of hypercalcemia related to polymethylmethacrylate injection: report of three cases.

Polymethylmethacrylate is a microsphere-based plastic used in aesthetic procedures. However, it can cause granuloma formation and severe hypercalcemia, often accompanied by renal impairment, which is a significant concern. Despite increasing reports, there is no standardized treatment approach for polymethylmethacrylate-induced hypercalcemia, and its incidence remains underreported. In this study, we present three cases of polymethylmethacrylate users who developed severe hypercalcemia that was resistant to conventional treatments, resulting in deteriorating renal function. These patients achi…

Milk-alkali syndrome (MAS) was first described as an adverse effect of the Sippy regimen, which contained large amounts of sodium bicarbonate, creating a clinical scenario in which hypercalcemia, metabolic alkalosis, and acute renal failure could develop simultaneous features characteristic of MAS. The syndrome has resurged due to widespread calcium suppleme…

A Hypercalcemic Deception: Uncovering an Unusual Case of Familial Hypocalciuric Hypercalcemia.

Although a rare cause of hypercalcemia, familial hypocalciuric hypercalcemia (FHH) is an inherited condition most often due to a missense mutation in the calcium-sensing receptor (CASR) gene, giving rise to increased calcium levels with elevated parathyroid hormone (PTH) levels and hypocalciuria. Many clinical features of FHH show a high degree of overlap with the much more common disorder of primary hyperparathyroidism (PHPT), making the correct diagnosis a challenge since surgery should be avoided in FHH. In the current case, although PHPT was initially suspected based on urine biochemistry …

A 65-year-old man with cough, hypercalcemia, and diffuse opacities.

Respiratory infections in patients with cancer are common. A low threshold for bronchoscopy with bronchoalveolar lavage (BAL) should be maintained in immunocompromised patients with pulmonary opacities on chest imaging to evaluate for opportunistic infections. In those with unresolving symptoms, transbronchial biopsies may add significant diagnostic value. Concomitant hypercalcemia in patients with cancer is typically related to underlying malignant disease. However, in those with persistent respiratory symptoms or atypical infiltrates then granulomatous inflammatory diseases should be conside…

Lithium-associated hypercalcemia and hyperparathyroidism: A systematic review and meta-analysis.

OBJECTIVES: We aimed to review and summarise the existing human literature on the association between lithium and hyperparathyroidism. METHODS: A systematic literature search was carried out according to PRISMA guidelines (last search 27 February 2024), using MEDLINE, Web of Science, Embase and the Cochrane Library. A meta-analysis was performed to determine the prevalence of lithium-associated hypercalcemia (LAHca) in lithium-treated patients. RESULTS: The pooled prevalence of LAHca based on total calcium and ionised calcium was comparable, at 3.17% and 4.23%, respectively. Calcium, and PTH…

Calcium Chaos in Sarcoidosis: A Tale of Severe Hypercalcemia's Diagnostic Challenge.

Sarcoidosis is a systemic inflammatory condition characterized by noncaseating granulomas. Lung involvement is typical, while extrapulmonary manifestations, notably lymphadenopathy, are observed in a significant proportion of cases. The etiology involves complex interactions among immune cells and mediators, resulting in granuloma formation capable of independently producing 1,25-dihydroxyvitamin D, leading to unregulated hypercalcemia and hypercalciuria. Diagnosis can be challenging, especially when hypercalcemia is the initial symptom. The presence of noncaseating granulomas on biopsy is cha…

Challenges in the management of hypercalcemia in pregnancy - Case report of two cases.

Hypercalcemia in pregnancy is rare and can pose a great diagnostic challenge due to its asymptomatic presentation. It is associated with maternal complications such as urolithiasis, pancreatitis, renal insufficiency and preeclampsia, fetal complications such as growth restriction and intrauterine fetal demise, and neonatal complications such as neonatal hypocalcemia, tetany and hypoparathyroidism. Prompt diagnosis and treatment of the underlying cause of hypercalcemia is important. Two cases of hypercalcemia in pregnancy were encountered over 12 months. Both presented asymptomatically in the f…

Malignant Hypercalcemia Due to the Ectopic Production of Calcitriol by an Abdominal Liposarcoma.

Hypercalcemia of malignancy (HM) is a common form of paraneoplastic syndrome associated with a poor prognosis of the disease. In solid tumors, HM occurs mainly due to the production of parathyroid hormone-related peptide (PTHrP). We present a case of a 60-year-old male with a 25 cm retroperitoneal liposarcoma diagnosed with severe hypercalcemia (16.8 mg/dL) by a preoperative blood sampling. Hypercalcemia workup showed suppressed parathyroid hormone (PTH), normal PTHrP, and high 1,25-dihydroxyvitamin D (1,25(OH)2D) serum levels. After surgery, hypercalcemia and calcitriol levels normalized. Imm…

Treatment of hypokalemia with amiloride unmasked hypercalcemia and hyperparathyroidism: A case report.

Colonic pseudo-obstruction, also called Ogilvie's syndrome, occurs due to impaired intestinal propulsion, and may be caused by electrolyte imbalances such as hypokalemia and some endocrine disorders such as hyperparathyroidism. Secretory diarrhea due to intestinal pseudo-obstruction can cause hypokalemia. Diuretics such as amiloride can be used to treat hypokalemia, however in this case, treatment with amiloride induced hypercalcemia and unmasked hyperparathyroidism. A 73-year-old female with a history of hypertension and parathyroid adenoma presented with recurrent colonic pseudo-obstruction …

An Unusual Case of Hypercalcemia Due to Graft-Versus-Host Disease.

BACKGROUND/OBJECTIVE: Hypercalcemia is a common disorder with a wide differential and is most commonly related to malignancy and hyperparathyroidism. Hypercalcemia is a rarely reported consequence of graft-versus-host disease (GVHD) and may be related to a granulomatous manifestation of the common stem cell transplantation procedure. CASE REPORT: A 67-year-old woman with a history of allogenic stem cell transplantation due to myelodysplastic syndrome presented to the bone marrow transplant clinic with dysphagia, muscle aches, and rash. She was found to have an extremely increased calcium and …

Denosumab-Induced Rebound Hypercalcemia Treated With Bisphosphonates in a Pediatric Patient.

Denosumab is a RANK-L inhibitor used off-label as a treatment for a variety of pediatric bone disorders, including aneurysmal bone cysts (ABC). Rebound hypercalcemia is a known side effect after denosumab therapy and is more commonly reported in pediatric patients. Although there are no established treatment guidelines, denosumab-induced rebound hypercalcemia is usually managed with a combination of intravenous fluids, diuretics, corticosteroids, denosumab, and/or bisphosphonates. We present the case of a 10-year-old female patient with history of a right sacral ABC treated with denosumab who …

A Case of Acute Adrenal Insufficiency: A Rare but Possible Cause of Hypercalcemia.

A 30-year-old woman presented with a 15-day history of epigastric pain, nausea, vomiting, asthenia, and weight loss. On admission, hypercalcemia was reported with a negative etiologic workup (including no primary hyperparathyroidism, malignancy, or vitamin D toxicity). Fluid replacement did not improve her hypercalcemia. We performed a blood ionogram and assessed the adrenocortical function, which showed that her serum cortisol was decreased, her adrenocorticotropic hormone (ACTH) was elevated, and anti-21 hydroxylase antibodies were positive. We established the diagnosis of autoimmune primary…

Use of Calcitonin for Hypercalcemia in a Premature Neonate With Congenital Mesoblastic Nephroma.

We report on a premature neonate (31 4/7 weeks' gestation) who presented with hypercalcemia secondary to congenital mesoblastic nephroma (CMN), the most common type of renal tumor in neonates. Typical presentation includes a palpable abdominal mass or swelling and may include abdominal pain, hematuria, fever, or hypertension. A less common complication of CMN is hypercalcemia of malignancy. Although the primary management strategy for hypercalcemia of malignancy is to treat the underlying disease, there are several agents that can be used as well for acute hypercalcemia including fluids, loop …
