Renal osteodystrophy: role of calcimimetics.

In patients with secondary hyperparathyroidism (HPT), increased parathyroid hormone (PTH) secretion is triggered by low plasma calcitriol levels, hypocalcemia, and hyperphosphatemia. Vitamin D analogues have been used successfully to reduce PTH levels, but increases in serum calcium, phosphorus, and calcium x phosphorus ion product levels may occur. Second-generation calcimimetics have been shown to suppress PTH levels and also reduce calcium x phosphorus ion product. Potential indications are patients with secondary HPT, particularly those who respond to calcitriol therapy with an increase in…

[Hyperuricemia in hematological disorders].

Hyperuricemia is a common manifestation in various hematological disorders such as hemolytic anemias, megaloblastic anemia malignant lymphomas and leukemias. Tumor lysis syndrome (TLS) is a serious complication consists of hyperuricemia, hyperkalemia, hyperphosphatemia and hypocalcemia due to massive cell lysis, which occurs shortly after the onset of chemotherapy or radiotherapy for acute leukemias and malignant lymphomas. Acute renal failure may develop due to uric acid nephropathy. The risk factors of TLS are heavy tumor cell burden, rapid cell turn over rate and renal involvement of neopla…

How to assess the efficacy of phosphate binders.

BACKGROUND AND AIMS: The efficacy of phosphate binders is difficult to be estimated clinically. This study analyzes the changes in serum phosphate and urinary phosphate excretion after the prescription of phosphate binders (PB) in patients with chronic kidney disease stage 4-5 pre-dialysis, and the usefulness of the ratio between total urinary phosphate and protein catabolic rate (Pu/PCR) for estimating the efficacy of PB. METHODS: This retrospective observational cohort study included adult chronic kidney disease patients. Biochemical parameters were determined baseline and after 45-60 days

KLOTHO methylation is linked to uremic toxins and chronic kidney disease.

Epigenetic regulation plays a major role in uremic toxin-induced chronic kidney disease (CKD) progression. The KLOTHO protein is a key modulator of homeostasis in renal function. Uremic toxin accumulation can induce DNA methyltransferase (DNMT) protein expression, which is involved in the silencing of KLOTHO through hypermethylation. Treatment with DNMT inhibitors can induce a hypermethylated status of KLOTHO and suppress mRNA and protein expression. Epigenetic targeting of specific genes may become an effective strategy to prevent progression of uremia-related CKD.

Calciphylaxis Cutis Associated With Fibroblast Growth Factor Receptor (FGFR) Inhibitor Therapy: A New Challenge.

Specific fibroblast growth factor receptor (FGFR) inhibitors have been developed to treat malignancies harboring fusions or rearrangements in FGFR2 or FGFR3. Here, we report a case of calciphylaxis cutis in association with FGFR inhibitor therapy in a patient with FGFR2 rearranged cholangiocarcinoma. Although calciphylaxis cutis typically arises in the setting of hyperphosphatemia and end-stage renal disease, this patient had preserved renal function, normal serum calcium, and only modestly elevated serum phosphorus levels, which is similar to other recent reports of calciphylaxis in patients …

Carfilzomib-induced tumor lysis syndrome in relapsed multiple myeloma: a report of two cases.

BACKGROUND: Tumor lysis syndrome (TLS) is a potentially fatal complication of cancer therapy characterized by severe electrolyte and metabolic abnormalities such as hyperphosphatemia, hyperkalemia, and hypocalcaemia. TLS usually occurs in aggressive hematologic malignancies such as Burkitt lymphoma and acute leukemia. TLS has rarely been observed in multiple myeloma (MM). CASE REPORT: We present 2 patients with relapsed MM who developed TLS after the first cycle of carfilzomib treatment. CONCLUSION: Carfilzomib is a next-generation proteasome inhibitor with proven efficacy in relapsed/refrac…

FGF23 in skeletal modeling and remodeling.

Fibroblast growth factor 23 (FGF23), a hormone primarily produced in bone cells, targets the kidney to accelerate phosphate excretion into the urine and suppresses vitamin D synthesis, thereby inducing a negative phosphate balance. Excessive serum FGF23 due to hereditary disorders such as hypophosphatemic rickets leads to phosphate wasting and impaired bone mineralization. In contrast, deficiencies in FGF23 are associated with hyperphosphatemia, elevated 1,25(OH)(2)D(3), ectopic ossification in soft tissues, and defects in skeletal mineralization. Recent studies of human genetic disorders and …

Renal osteodystrophy.

Renal osteodystrophy is a complex disorder which can be divided into five distinct bone histologic subtypes: mild bone disease, hyperparathyroid bone disease, mixed bone disease, osteomalacia, and low-turnover bone disease. Hyperparathyroidism develops in renal failure due to two principal abnormalities: 1,25(OH)2D3 deficiency and hyperphosphatemia. Treatment of these problems is important in order to prevent hyperparathyroidism. Most cases of osteomalacia, mixed bone disease, and low-turnover bone disease are influenced by aluminum status and parathyroid hormone. Aluminum-associated bone dise…

Dystrophic calcification, cataracts, and enamel hypoplasia due to long-standing, privational vitamin D deficiency.

A 19-year-old Indian girl presenting with intermittent tetany, enamel hypoplasia, bilateral cataracts, and calcification of the basal ganglia is described. Dental evidence suggested a calcification defect had been present from the age of 2-3 years. Hypocalcemia, hyperphosphatemia, and low levels of immunoassayable parathyroid hormone (iPTH), urinary cAMP, 25-hydroxyvitamin D, and 1,25-dihydroxyvitamin D were documented, suggesting hypoparathyroidism with vitamin D deficiency. A bone biopsy showed osteomalacia. Following ultraviolet irradiation and oral calciferol therapy, a symptomatic and bio…

Impact of Serum Phosphorus on Hemoglobin: A Literature Review.

Phosphorus is a macroelement found in the body, mostly in the bones as crystals of hydroxyapatite. Higher levels are found in patients affected by chronic kidney disease (CKD). Since the early stage of CKD phosphorous excretion is impaired, but the increase of PTH and FGF23 maintains its level in the normal range. In the last decades, the role of FGF23 in erythropoiesis was studied, and now it is well known for its role in anemia genesis in patients affected by conservative CKD. Both Hyperphosphatemia and anemia are two manifestations of CKD, but many studies showed a direct association betwee…

[New Developments in CKD-MBD. Cell Biology of parathyroid in CKD].

Parathyroid monitors the calcium concentration in blood by signals from calcium-sensing receptors, adjusts secretion of parathyroid hormone to keep constant calcium concentration in the body. Although parathyroid parenchymal cells consist of chief cells which secrete PTH, and oxyphil cells which are rich in mitochondria, all hardly perform mitotic proliferation in normal status. However, in CKD, PTH hypersecretion and hyperplasia are started by hyperphosphatemia, hypocalcemia, and activated-vitamin-D deficiency, and the secondary hyperparathyroidism develops. While treatment with cinacalcet hy…

[Transitory pseudohypoparathyroidism in the newborn infant].

This is a report on two young infants suffering from transitory pseudohypoparathyroidism. Therapy with vitamin D in high doses was not helpful but metabolic changes were promptly corrected by the administration of 1.25-(OH)2-D3 (Rocaltrol). The typical pretreatment findings were: hypocalcemia, hyperphosphatemia, reduced clearance of phosphate. Parathormone in serum was normal. Treatment with 0.25 and 0.5 micro-Gram 1.25-(OH)2-D3 respectively resulted in prompt correction of serum calcium- and serum-phosphate-changes. The observation of the urinary-cAMP-excretion during the course of the diseas…
