Diagnosis and treatment of hepatic hydatid cysts with the aid of echo-guided percutaneous cyst puncture.

The usefulness of echo-guided percutaneous cyst puncture in the diagnosis and treatment of hepatic hydatid cysts was assessed in a study of 45 patients referred to the Department of Tropical Medicine at Kasr El Aini Hospital in Cairo. All subjects had at least one hepatic cyst detected by ultrasonography (54 cysts in all), and 42 patients had positive results in serological tests for antibodies to Echinococcus granulosus. The majority of cysts were in the right hepatic lobe (40 cysts), were anechoic (51), and were rounded and smooth (49). All 45 active cysts (44 anechoic, rounded, and smooth;

The effect of reconstruction after subtotal gastrectomy on release of vasoactive intestinal peptide.

To avoid dumping after Billroth gastrectomy, we designed pylorus and antroseromuscular-flap preserving subtotal gastrectomy (PAFPG). The mean maximal plasma level of VIP (vasoactive intestinal peptide) in PAFPG after oral hypertonic glucose was close to that in the control. Compared with PAFPG group, the plasma VIP concentrations after hypertonic glucose ingestion in BI and BII group were significantly higher, and the concentration of plasma VIP in dumpers was significantly higher than that in non-dumpers after BI or BII gastrectomy. The results suggest that the amount of VIP release is relate

Hypovolemic shock and intestinal ischemia: a preventable complication of incomplete formula labeling.

A 19-day-old male infant was seen with full-thickness gangrene of the transverse colon. Hyperosmolar formula feedings caused by inadequate "directions for use" was the probable cause. Improved formula labeling might prevent further cases.

Impairment of osmotically stimulated AVP release in patients with primary polydipsia.

The secretion of arginine vasopressin (AVP) from the posterior pituitary is primarily and finely regulated by the osmolality of plasma. Even though a number of factors alter osmolality-induced release of AVP, there are no published data in humans that have addressed the role of chronic overhydration on this phenomenon. To address this problem we have identified eight patients with primary polydipsia using criteria not involving measurement of AVP, and have subjected them to standardized infusions of hypertonic saline. These patients had less AVP in both plasma and urine in relation to plasma o

Hypertonic saline/dextran resuscitation of dogs with experimentally induced gastric dilatation-volvulus shock.

We investigated small-volume (5 ml/kg) 7% NaCl in 6% dextran 70 (HS/D70) as an alternative to large-volume (60 ml/kg) 0.9% NaCl for treatment of experimentally induced canine gastric dilatation-volvulus (GDV) shock. The stomach was surgically displaced and then distended with an intragastric balloon in 11 dogs anesthetized with pentobarbital. All dogs were subjected to GDV for 180 minutes before partial decompression and resuscitation. Hemodynamic values, blood gas values, and plasma volume were measured during control, shock, and resuscitation periods. Resuscitation started with 1 group (n =

[Carbohydrates in parenteral nutrition solutions in pediatrics--a critical evaluation].

Application of carbohydrates in pediatric infusion therapy has recently been limited to glucose and xylitol. Fructose and sorbitol, which formerly had been used widely as energy sources in parenteral nutrition, have meanwhile been banned in order to prevent fatal complications in patients with undiscovered hereditary disturbances in fructose metabolism. The aim of this review is to focus the attention on potential side effects and limitations of glucose administration in pediatric infusion therapy. With special regard to total parenteral nutrition in preterm infants, sufficient glucose convers

[Effect of NIK-242 inj. (20% erythritol) on intracranial pressure in dogs with acute obstructive hydrocephalus].

The effects of NIK-242 inj. (20% erythritol) on intracranial pressure (ICP) and serum osmotic pressure (Osm. P) were investigated in dogs with acute obstructive hydrocephalus, and they were compared with those of 20% mannitol or 10% glycerol in 5% fructose and 0.9% saline. Animals were divided into 5 groups (n = 6 in each group) and treated with either NIK-242 inj. (0.5 g/kg, 1.0 g/kg, 2.0 g/kg), mannitol (1.0 g/kg) or glycerol (0.5 g/kg). These drugs were administrated intravenously for 10 min. NIK-242 inj. rapidly reduced ICP and increased Osm. P. There was correlation between changes of ICP

Altered drinking responses in dogs with chronic metabolic alkalosis.

Chronic chloride depletion alkalosis in dogs causes a lowered osmotic threshold and increased sensitivity for vasopressin (AVP) release. Since AVP release and drinking behavior normally are closely associated over a narrow range of changes in plasma osmolality (Posm), we investigated whether alkalotic dogs would also show an altered responsiveness to the dipsogenic effects of angiotensin II (ANG II) and osmotic stimuli. Dogs made chronically alkalotic by a combination of chloride-free diet and furosemide injections developed polydipsia in the absence of any increase in solute intake and in the

Human platelet arginine vasopressin.

Approximately 90% of arginine vasopressin circulating in blood is associated with the platelets. Studies in six patients with cranial diabetes insipidus indicate that the platelet vasopressin originates in the hypothalamus and is taken up from plasma. Extracted platelet vasopressin is chromatographically identical to the posterior pituitary peptide and biologically active. Infusion of low-doses of exogenous vasopressin into 10 normal male subjects caused a marked rise in both plasma and platelet hormone concentrations. Stimulation of endogenous vasopressin release by hypertonic saline (0.51 mo

[Central pontine and extra-pontine myelinolysis following rapid correction of hyponatremia--a case report].

An autopsy case of central pontine and extrapontine myelinolysis following rapid correction of hyponatremia is reported. Hyponatremia was induced by massive devastation of adrenal cortex due to tuberculosis. Severe hyponatremia (approximately 100 mEq/l of serum sodium level) was rapidly corrected by infusion of hypertonic saline solution within two days. After transient regression of her consciousness, the patient developed decorticated state and then complete paralysis of all limbs. She died of pneumonia approximately one month after the last illness. The autopsy revealed myelynolysis in the

Factors influencing the secretion of vasopressin into cerebrospinal fluid.

Vasopressin (VP) has been found in the cerebrospinal fluid (CSF) of several species of animals. Although it is known that hemorrhage, hypertonicity of body fluid, hypoxia, and hypercapnia all increase VP in plasma, little is known regarding the stimuli that cause the secretion of VP into the CSF. We therefore performed several studies to examine whether stimuli capable of increasing plasma levels of VP can also increase VP in the CSF of anesthetized dogs. We found that hemorrhage, intracerebroventricular infusion of hypertonic artificial CSF, hypoxia, and hypercapnia all produced increases in

Arginine-vasopressin in postpartum panhypopituitarism: urinary excretion and kidney response to osmolar load.

The response to an osmolar load (750 ml 2.5% NaCl solution iv preceded by 500 ml water by mouth) was studied in 20 patients with Sheehan's syndrome and 12 normal women. Sodium and osmolality were determined in plasma and urine and arginine-vasopressin (AVP) was measured by RIA in urine. The test was performed in each patient when untreated (group P), after hydrocortisone replacement alone (group F), and combined hydrocortisone and thyroid hormone replacement (group F+T). After the osmolar loading, maximum urinary osmolality in the patients was lower than in the normal women and remained unaffe

Treatment of 242 neonates with dehydrating diarrhea with an oral glucose-electrolyte solution.

Two hundred thirty-four of 242 neonates (96.7%) with a mean dehydration of 5.1% body weight were rehydrated with a glucose-electrolyte solution given orally in an average time of 7 44 hours. Eight (3.3%) neonates required intravenous fluid therapy. Emesis was not an obstacle for complete oral rehydration. Hypernatremia, hyponatremia, and acidosis present on admission were corrected within a few hours. Oral rehydration is an entirely satisfactory choice for treatment of uncomplicated dehydration in neonates.

Action of cortisol introduced into the supraoptic nucleus, on vasopressin release and antidiuresis during hypertonic saline infusion in conscious rhesus monkeys.

In human subjects cortisol raises the osmotic threshold for vasopressin release. The presumption that this results from a direct inhibitory action of cortisol on osmotically stimulated vasopressin release has been tested in conscious rhesus monkeys. Preparatory surgical procedures included chronic bladder catheterization, placement of cannulae in the right atrium, the left jugular vein and the stomach and stereotaxic insertion of metal cannulae in both supraoptic nuclei. The osmotic threshold for vasopressin release was defined as the plasma osmolality at the beginning of the 3 min urine sampl

[Effects of uric acid on the B cell in the isolated perfused rat pancreas (author's transl)].

Methylxanthines, such as caffeine and theophylline, show effects increasing the secretion of insulin. Perfusion experiments were intended to find out whether insulin secretion is influenced by uric acid, which is chemically closely related. Besides, it was to be demonstrated that uric acid causes no damages to islet cell structures for it is well established that alloxan, which is chemically related to uric acid, produces necroses in B cells. Isolated rat pancreata were stimulated by glucose at concentrations of 100 mg/100 ml and 300 mg/100 ml. In part of the experiments uric acid was added to

Treatment of refractory hypovolaemic shock by 7.5% sodium chloride injections.

Injections of hyperosmotic (7.5%) sodium chloride (100-400 ml) were given to 12 patients in terminal hypovolaemic shock who had not responded to vigorous volume replacement and corticosteroid and dopamine infusions. Hyperosmotic sodium chloride promptly reversed the shock in 11 of these patients. The immediate effects of the NaCl injections were a moderate rise in arterial pressure, the resumption of urine flow, and recovery of consciousness. These effects tended to persist for a few hours. The hyperosmotic infusion also reduced isosmotic fluid requirement by 90%.

Antidiuretic hormone excess.

Known physiologic mechanisms explain the elevated blood ADH levels observed in most patients with the syndrome of inappropriate ADH. Therefore the word "inappropriate" is a misnomer. It implies that the mechanisms that regulate ADH release are not functioning normally--which is not true. The term misleads the physician who, ideally, should determine why a patient has an excessive blood ADH level and initiate appropriate treatment. Patients with ectopic production of ADH and hyponatremia should be so labeled: "Hyponatremia due to ectopic ADH production." The term SIADH, if used at all, should b
