Hemophagocytic lymphohistiocytosis (HLH) is a life- threatening hyperinflammatory disease that causes extensive organ damage. It is generally triggered by viral, fungal, or parasitic infections in the setting of hematologic disease-induced immune deficiency. Occurrences in rheumatologic disease are less frequent, with the syndrome developing most often in patients with systemic lupus erythematosus and adult-onset Still disease. It is believed that the immunosuppression induced by rheumatologic disease itself and exacerbation by immunomodulatory therapies predispose to infection and subsequentl

The identification of CD30 has been known since 1985. Trials exploring the targeted therapy focusing on this antigen have led to the successful development and approval of brentuximab vedotin (Adcetris®) for treatment of relapsed refractory systemic anaplastic large-cell lymphoma. Brentuximab vedotin has a high-level of response with generally durable remission. Common side effects of brentuximab vedotin include peripheral neuropathy, fatigue, nausea, arthralgia, and pyrexia. Grade 3-4 neutropenia, thrombocytopenia, and hyperglycemia have also been reported. Development of progressive multifoc

Pulmonary disease is a major source of morbidity and mortality in rheumatoid arthritis, manifesting most commonly as interstitial lung disease, airways disease, rheumatoid nodules, and pleural effusions. The diagnostic assessment of respiratory abnormalities is complicated by underlying risk for infection, the use of drugs with known pulmonary toxicity, and the frequency of lung disease related to rheumatoid arthritis itself. Evaluation and management of rheumatoid arthritis-associated pulmonary disease frequently necessitates a multidisciplinary approach.

BACKGROUND: Manipulation of the graft with immunosuppressive genes represents a novel approach to overcome the toxicity of immunosuppressants currently used to prevent acute rejection. Here we compared the efficiency of a non-viral versus a viral technique of gene transfer to the kidney in the setting of isotransplantation and evaluated whether transfection with adenovirus encoding CTLA4Ig prolonged allograft survival.
METHODS: Donor rat kidneys were perfused with a medium containing the cationic polymer polyethylenimine PEI 25k complexed to a vector coding for the beta-galactosidase (beta-ga

Monoclonal antibodies, specific for antigens expressed on lymphoid malignancies, which have been conjugated to toxins such as ricin, hold promise in the therapy of childhood leukemia and lymphoma. Anti-B4-blocked ricin (anti-B4-bR) is such an agent, and a phase I study of this agent was conducted in children with relapsed or refractory B-lineage leukemia and lymphoma. Anti-B4-bR was given as two 7-day continuous infusions separated by 7 days. Twenty patients were enrolled and 19 received the drug. Two dosage levels (30 and 40 microg/kg per day) were evaluated. Forty micrograms per kilogram per

Pancreatic ductal adenocarcinoma (PDAC) is the most lethal form of pancreatic cancer, with poor prognosis driven by late diagnosis, therapeutic resistance, and an immunosuppressive tumor microenvironment. Interactions between tumor cells and immune cells promote immune evasion and tumor progression, limiting the efficacy of immune checkpoint blockade and other immunotherapies. Given the high expression of trophoblast cell surface antigen 2 (TROP2) in PDAC, we developed a TROP2-targeted Toll-like receptor 7 (TLR7) agonist (E104) designed for selective accumulation within the tumor microenvironm

BACKGROUND: Folate receptor alpha (FRα), commonly expressed in epithelial ovarian cancers, is a clinically actionable biomarker following approval of the antibody-drug conjugate mirvetuximab soravtansine (MIRV). Pivotal trials showed that high FRα expression predicts MIRV benefit, creating the need for standardized testing to ensure timely, equitable access.
METHODS: To address the need for guidance on FRα testing, the Society of Gynecologic Oncology of Canada convened a multidisciplinary Expert Panel to review the evidence and integrate Canadian clinical, pathology, laboratory, and patient p

BACKGROUND: Clinical trials have demonstrated favorable outcomes with sacituzumab govitecan (SG) in metastatic triple-negative breast cancer (mTNBC). However, significant differences between trial settings and routine clinical practice raise concerns about the real-world prognosis of patients with mTNBC. To address this gap, we conducted the first comprehensive single-arm meta-analysis integrating clinical trials and real-world studies (RWSs) to evaluate clinical outcomes and treatment experiences in mTNBC patients receiving SG.
METHODS: Relevant studies were searched systematically in seven

BACKGROUND: Sacituzumab govitecan (SG), an antibody-drug conjugate targeting TROP-2, demonstrated superior efficacy over standard chemotherapy in heavily pretreated metastatic triple-negative breast cancer (mTNBC) in the ASCENT trial. However, real-world data remain limited. This study evaluated the effectiveness and safety of SG in an unselected multinational cohort of patients with mTNBC.
METHODS: This retrospective analysis included 303 women who initiated SG treatment between August 2021 and April 2025 across 18 oncology centers in Poland, the Czech Republic and Slovakia, within the Centr

Antibody-drug conjugate (ADC) efficacy in solid tumors frequently fails to track with target abundance or binding affinity, reflecting delivery bottlenecks that remain difficult to quantify during lead selection. Here, we develop a discovery-to-function workflow for carcinoembryonic antigen-related cell adhesion molecule 6 (CEACAM6), a glycosylphosphatidylinositol (GPI)-anchored antigen with context-dependent trafficking and show that intracellular delivery-related parameters can better organize ADC potency than antigen abundance or binding affinity alone in the CEACAM6 model system. Using mul

BACKGROUND: Although trastuzumab-deruxtecan (T-DXd) has demonstrated significant efficacy in advanced breast cancer, treatment discontinuation due to adverse events occurs in ∼20% of patients. Pharmacogenetic variants in uridine diphosphate glucuronosyltransferase 1A1 (UGT1A1) increase the toxicity risk for other antibody-drug conjugates using topoisomerase I inhibitors, but they remain unexplored for T-DXd.
MATERIAL AND METHODS: The PROCURE Project is a translational study involving 26 Spanish institutions, investigating the association of germline pharmacogenomic variants with T-DXd toxicit

Antibody-drug conjugates (ADCs) have revolutionized the treatment landscape for metastatic urothelial carcinoma. Encouraging clinical activity of HER2-directed ADCs, including disitamab vedotin, has been observed in early-phase trials. Recent promising results from the RC48-C016 trial raise important questions. We discuss issues that include optimal frontline treatment selection, subsequent-line treatment sequencing, and the generalizability of the findings.

The use of mRNA therapies has innovated the clinical progress of cancer immunotherapy. However, current immunotherapeutic approaches are unable to achieve site- or immune-cell-specific delivery, resulting in adverse immune responses in off-target tissues. In addition, the commercial lipid nanoparticle (LNP) formulations with a poly(ethylene glycol) coating generally undergo significant hepatic accumulation during clearance. To promote site- and immune-cell-specific delivery of therapeutic mRNA-LNPs, we investigated several bioconjugation approaches to attach targeting antibodies onto the surfa

Antibody-drug conjugates (ADCs) are important cancer therapeutics, yet efficacy in solid tumors is limited by poor penetration: the large size (∼150 kDa) restricts vascular extravasation and stromal diffusion, and strong bivalent IgG-antigen binding creates a binding-site barrier. Antibody fragment-drug conjugates (FDCs) penetrate better but clear rapidly, compromising retention. We developed a programmable Antibody Fragment-Cleavable PEGylated Drug Conjugate (AFCDC) platform enabling site-specific dual conjugation through genetically encoded bifunctional noncanonical amino acids, pTAF to atta

BACKGROUND: Sex-based differences in treatment-related adverse events (TRAEs) have been described across several anticancer therapies. However, sex-disaggregated safety data for antibody-drug conjugates (ADCs) in non-small-cell lung cancer (NSCLC) is lacking.
PATIENTS AND METHODS: We conducted a monocentric cohort study including patients with advanced NSCLC treated with developmental ADCs within phase I-II clinical trials at Gustave Roussy (2018-2023). Clinically significant TRAEs (grade ≥2, Common Terminology Criteria for Adverse Events version 5.0), dose modifications, hospitalisations, an

Patients with advanced TNBC with PD-L1 CPS < 10 are typically treated with chemotherapy and experience poor outcomes. Sacituzumab tirumotecan (sac-TMT; MK-2870/SKB264) is a trophoblast cell-surface antigen 2 (TROP2)-directed antibody-drug conjugate with a unique, bifunctional linker that maximizes payload delivery to tumor cells. Combining sac-TMT with immunotherapy may improve outcomes given their complementary respective direct cytotoxic and immune-mediated antitumor effects regardless of tumor PD-L1 expression. The TroFuse-011 study evaluates sac-TMT with/without pembrolizumab versus treatm

PURPOSE: Vobramitamab duocarmazine (vobra duo) is a duocarmycin-based, humanized antibody-drug conjugate (ADC) targeting B7-H3, with a drug-to-antibody ratio of ∼2.7. Vobra duo has demonstrated robust antitumor activity in multiple adult cancer models, along with favorable pharmacokinetic and safety profiles in cynomolgus monkeys. Early results from phase I/II clinical trials (NCT03729596) have shown manageable toxicity and promising objective responses in patients with metastatic castration-resistant prostate cancer. Given the high expression of B7-H3 in pediatric solid tumors, this target is

Antibody-drug conjugates (ADCs) targeting programmed death-1 (PD-1) represent a novel class of cancer immunotherapies that combine immune checkpoint blockade with targeted delivery of cytotoxic agents. By selectively binding PD-1-expressing cells within the tumor microenvironment, PD-1 ADCs aim to reverse T cell exhaustion and eliminate immunosuppressive cells while directly inducing tumor cell death. This dual mechanism enhances antitumor immunity and addresses resistance associated with conventional PD-1 inhibitors. Despite promising preclinical results and emerging clinical data, challenges
