Nijmegen breakage syndrome (NBS) is a rare autosomal recessive DNA repair disorder within the spectrum of inborn errors of immunity (IEI), characterized by immunodeficiency and cancer predisposition and typically diagnosed in childhood. We report a pair of 21-year-old monozygotic twins presenting with recurrent infections, hypogammaglobulinemia, ovarian dysfunction, and, in one twin, peripheral T-cell lymphoma. Both carried two novel compound heterozygous variants in the NBN gene (c.657dupA and c.1064_1124 + 9del). Immunophenotyping revealed reduced memory B cells, impaired vaccine responses,

Good's syndrome is an immunodeficiency disease involving thymoma accompanied by hypogammaglobulinemia. We encountered a case of Good's syndrome accompanied by agranulocytosis that followed a rapid clinical course. A 72-year-old man visited our hospital with a two-week history of a sore throat. Candida albicans was detected in the pharynx, and hypogammaglobulinemia was detected in addition to granulocytopenia. The patient subsequently developed septic shock and followed a rapid clinical course which ended in death. Good's syndrome with agranulocytosis was diagnosed at autopsy. Good's syndrome a

Rothmund-Thomson syndrome (RTS)(OMIM 268400) is a rare autosomal recessive genodermatosis characterized by poikiloderma, small stature, skeletal and dental abnormalities, cataract and an increased risk of cancer. It is caused by mutations in RECQL4 at 8q24. Immune deficiency is not described as a classical feature of the disease. Here we report the appearance of granulomatous skin lesions complicating primary Varicella Zoster Virus infection in a toddler with Rothmund Thomson syndrome and immune deficiency. Although granulomatous disorders are sometimes seen after Herpes zoster, they are even

Genetic defects in T-cell function lead to susceptibility to infections or to other clinical problems that are more grave than those seen in disorders resulting in antibody deficiency alone. Those affected usually present during infancy with either common or opportunistic infections and rarely survive beyond infancy or childhood. The spectrum of T-cell defects ranges from the syndrome of severe combined immunodeficiency, in which T-cell function is absent, to combined immunodeficiency disorders in which there is some, but not adequate, T-cell function for a normal life span. Recent discoveries

Clinically, primary immunodeficiencies can be grouped into several well-defined syndromes. This consistent clinical picture, however, belies the enormous complexity of lymphocyte maturation and activation, and it has long been suspected that numerous distinct underlying defects give rise to primary immunodeficiencies. At a recent workshop, the molecular and genetic characterization of a handful of such defects provided tacit support for such suspicions.

A family including three children with DiGeorge syndrome is described. One child died in the neonatal period from cardiac anomalies accompanying complete DiGeorge syndrome. The two surviving siblings shared a common set of pharyngeal pouch anomalies and immunodeficiency consistent with partial DiGeorge syndrome, and other morphologic anomalies characteristic of the velocardiofacial syndrome with which familial DiGeorge syndrome is associated (reviewed in reference 1). Both had normal karyotypes. Both presented with recurrent otitis media and sinopulmonary infections, CD4+ T cell lymphopenia, a

The observation of a 21 year-old man with primary common variable hypogammaglobulinemia and total villous atrophy is reported. Gluten withdrawal induced considerable improvement in malabsorption and mucosal damage. However, protein-losing enteropathy, nodular lymphoid hyperplasia and immunoglobulin deficiency were persistent. To our knowledge, improvement on gluten-free diet has been reported until now in 18 previous cases. This report illustrates the links between immunodeficiency and malabsorption, which are discussed.

We describe a 14-year-old female with systemic lupus erythematosus (SLE) involving the skin, joints and central nervous system who developed hypertrichosis of the upper eyelashes. This clinical finding has been observed in immunocompromised patients with acquired immune deficiency syndrome (AIDS), malnutrition, cancer or kala-azar. Although the pathogenic mechanism for this type of hypertrichosis is unknown, we believe the immunological defects seen in SLE may be responsible for such manifestation in our patient.

A case of disseminated Kaposi's sarcoma (KS) in a 21-yr-old white heterosexual male with cryptogenic cirrhosis and no serological or immunological evidence of acquired immune deficiency syndrome (AIDS) is reported. The patient died 2 wk after diagnosis. Postmortem examination showed involvement of lymph nodes, liver, spleen, gastrointestinal tract, and bone marrow. This case demonstrates that Kaposi's sarcoma can occur in a young heterosexual male with normal immune function and in the absence of HIV infection.

The role of procalcitonin (PCT) in immunocompromised patients is still under investigation. This study evaluated the influence of immune deficiency on the value of PCT concentrations in the diagnosis of early stages of bacterial infections in human immunodeficiency virus (HIV)-infected patients compared with other inflammatory markers, such as C-reactive protein and white blood cell count. We analyzed major immunologic markers including CD4, CD8, and HIV-1 viral load. PCT concentrations in the early stages of bacterial infections correlated negatively with CD4 count in HIV-infected patients. H

Although the pathophysiology of acquired immune deficiency syndrome (AIDS) is not completely understood, we know a great deal about its epidemiology, risk factors, clinical manifestations, course, and immunologic features. Clinicians caring for high-risk individuals, particularly those in endemic urban areas, should remain alert for signs and symptoms of opportunistic infections or neoplasms associated with immunosuppression. It is clear that research on AIDS has increased exponentially; both physicians and patients should be encouraged by the fact that the apparent cause of this syndrome has

Clinical observations suggest that lipoid nephrosis is produced by a systemic abnormality of T-cell function resulting in the secretion of a circulating chemical mediator toxic to an immunologically innocent glomerular basement membrane. The lack of evidence of a humoral antibody response, remission induced by measles which modifies cell-mediated immunity, the therapeutic benefits of steroids and cyclophosphamide which also abate cell-mediated responses, and the occurrence of this syndrome in Hodgkin's disease support this hypothesis. The susceptibility of untreated patients to pneumococcal in

Good syndrome, characterized by hypogammaglobulinemia and acquired immunodeficiency, is a rare condition associated with thymoma. A 67-year-old woman, who 4 months previously had a thymoma resected, presented with generalized hypogammaglobulinemia with a severely decreased B cell population as demonstrated by flow cytometry. She was diagnosed as having bacterial mediastinitis associated with Good syndrome. For the subsequent 6 years, she suffered from repeated serious bacterial infections. As this paraneoplastic syndrome is not resolved by tumor removal, careful management with intensive infec
