Central cholinergic control of cerebral blood flow in the baboon. Effect of cholinesterase inhibition with neostigmine on autoregulation and CO2 responsiveness.

Cerebral autoregulation and vastomotor responsiveness to carbon dioxide (CO2) were measured quantitatively by the use of the autoregulation index and chemical index, respectively, in normal baboons before and after intravertebral and intracarotid infusion of the anticholinesterase agent, neostigmine methylsufate (Prostigmin). Continuous measurements were made of cerebral blood flow (measured as bilateral internal jugular venous outflow), arterial and cerebral venous pO2 and pCO2, cerebral arteriovenous oxygen differences, and endotracheal CO2. The effect of intravertebral infusion of neostigmi

Iatrogenic Vertebral Artery Dissection with Posterior Fossa and Lateral Medullary Stroke After Uncomplicated Cervical Nerve Ablation.

BACKGROUND: A cervical radiofrequency ablation is a procedure that can be performed to treat arthritis-related pain in the neck and upper back. There have been no large studies reporting complications after this procedure. We report a case of a 55-year-old woman with iatrogenic vertebral artery dissection of C3-C4 with segmental occlusion leading to a posterior fossa stroke and lateral medullary stroke after a high-grade cervical nerve ablation. CASE REPORT: A 55-year-old woman presented to the Emergency Department with vomiting, neck pain, temperature changes, dizziness, and dysarthria after

Fibrinolytic therapy for acute embolic stroke: intravenous, intracarotid, and intra-arterial local approaches.

To clarify the efficacy and limitations of the intra-arterial local infusion of a high-dose fibrinolytic agent for acute embolic stroke, we analyzed the results of 44 patients and compared them with those of 51 patients treated with intracarotid (18 patients) or intravenous (33 patients) infusion therapy. Ten megaunits of recombinant tissue plasminogen activator or 24 x 10(4) IU of urokinase were administered through a microcatheter placed into or proximal to an embolus for 20 minutes. When arterial recanalization was not achieved, a second or third infusion was performed. The rates of complet

Modulating effect of adenosine on the hypertensive response to physostigmine.

The purpose of this work was to study the neuromodulatory role of adenosine on the hypertensive response to physostigmine in the rat. It was found that increasing doses of adenosine, injected intravenously, produced a biphasic blood pressure response, consisting of an initial and short-lasting hypotension, followed by a prolonged hypertension. The hypertensive phase of this response was significantly depressed or even abolished by phentolamine, whereas the hypotensive phase was depressed but not abolished by atropine. If injected intravenously, but immediately before physostigmine, adenosine p

Presynaptic neurotransmitter and chemosensory responses to natural stimuli.

We investigated the hypothesis that release of acetylcholine from presynaptic nerve terminals in the carotid body may be responsible for the excitation of carotid body chemoreceptors by hypoxia and hypercapnia and central ventilatory stimulation by hypercapnia. 4-Aminopyridine, an agent known to release presynaptic transmitters including acetylcholine, was administered intravenously (1 mg X kg-1) or by close intra-arterial injection to the carotid body (200 micrograms) in anesthetized cats. 4-Aminopyridine did not change the carotid chemosensory responses to any arterial PO2 or PCO2 levels stu

Aetiology of convulsions due to stellate ganglion block: a review and report of two cases.

Stellate ganglion block is a selective sympathetic block that affects the ipsilateral head, neck, upper extremity and upper part of the thorax. Convulsions are a recognized complication of intra-arterial injection during stellate ganglion block. As central nervous system toxicity depends ultimately on the concentration of the local anaesthetics presented to the brain, the likely causative factors are discussed as well as the types of toxic symptoms and their onset times. The paper considers the aetiological factors of such convulsions resulting from stellate ganglion block in two patients.

Middle meningeal artery coil embolization after intra-arterial lidocaine response for refractory side-locked post-flow-diverter headache: A neuro-interventional video.

Middle meningeal artery (MMA)-directed interventions have been described for refractory headache syndromes, including migraine and aneurysmal subarachnoid hemorrhage-associated headache. We present a Neuro-Interventional Video demonstrating intra-arterial lidocaine-guided MMA coil embolization for severe refractory side-locked headache after ipsilateral internal carotid artery flow-diverter placement. The patient denied active headache or migraine immediately before flow-diverter treatment, but postprocedural headaches were documented at 1-month follow-up. The patient subsequently developed se

Splanchnic blood flows in a rat model of antibiotic-controlled intra-abdominal abscess during normoxia and hyperoxia.

We have characterized splanchnic blood flows in a rat model of antibiotic-controlled, intra-abdominal abscess (ACIA) in normoxic and hyperoxic environments. Abscesses (1.5 ml average volume) were established by implanting a fecal-agar pellet containing known numbers and strains of live Escherichia coli and Bacteroides fragilis into the peritoneal cavity of rats via a midline incision. Sham-operated rats (SR) served as controls. Cannulas (PE-50) were placed into the left ventricle of both SR and ACIA rats for injection of tracer microspheres (MS) and into the tail artery for a reference withdra

What We Can Learn from Cases of Synchronous Acute Mesenteric Obstruction and Nonocclusive Mesenteric Ischemia: How to Reduce the Acute Mesenteric Ischemia-Related Mortality Rate.

Although the survival rate of patients with ischemic heart disease has recently increased, it remains unknown why the mortality rate of acute mesenteric ischemia (AMI) remains high. Here, we report a possible method of improving the survival rate of patients with AMI obtained through 2 cases of simultaneous acute mesenteric obstruction (AMO) and nonocclusive mesenteric ischemia (NOMI). Case 1 was a 74-year-old woman with atrial fibrillation, hypertension, and dyslipidemia as underlying diseases who developed NOMI immediately after undergoing SMA thrombolysis. Case 2 was a 69-year-old man with

Intertumor heterogeneity in vascularity and invasiveness of artificial melanoma brain metastases.

BACKGROUND: Patients diagnosed with melanoma brain metastases have few treatment options and poor prognosis, and improved treatment strategies for these patients require detailed understanding of the underlying pathobiology. In this investigation we studied the vascularity and invasiveness of artificial brain metastases established from four human melanoma cell lines. METHODS: A-07, D-12, R-18, and U-25 cells transfected with GFP were injected intracerebrally and intra-arterially in nude mice. Moribund mice were killed and autopsied, and the brain was evaluated by fluorescence imaging or by h

Pulmonary eosinophilia and granulomatous pulmonary arteritis induced in rats by intravenous Sephadex.

Airway eosinophilia has been proposed as a major pathogenetic event in bronchial asthma and airway hyperresponsiveness. Intravenous injection of Sephadex G200 in rats induces pulmonary and blood eosinophilia and alters pulmonary responsiveness to 5-hydroxytryptamine. To characterize the early pulmonary inflammatory responses following Sephadex administration, and to determine the timing of the onset of pulmonary eosinophilia relative to blood eosinophilia, Sprague-Dawley rats were injected intravenously with Sephadex G200 beads. Lungs and other tissues were examined by light and transmission e

Endovascular treatment of basilar artery occlusion by manual aspiration thrombectomy.

BACKGROUND AND PURPOSE: Basilar artery occlusion remains one of the most devastating subtypes of stroke. Intravenous and intra-arterial therapy have altered the natural history of this disease; however, clinical results remain poor. Therefore, exploring more aggressive and innovative management is warranted. METHODS: Six consecutive patients presenting with a basilar artery occlusion were treated with the same general algorithm of intra-arterial tissue plasminogen activator and mechanical thrombectomy with the Merci retrieval system. If complete recanalization was not achieved after two passe

[Right subclavian catheterization. Misplaced insertion into the subclavian artery and the ascending aorta].

The accidental subclavian artery puncture is usually obvious. We report a case of unrecognized arterial catheterisation. The catheter had been inserted during anaesthesia after return of dark and non pulsatile blood, and not controlled by a chest radiograph. During surgery, the injection of 40 mL isotonic saline containing 4 g of piperacillin for antibiotic prophylaxis resulted in a transient circulatory collapse associated with ECG tracing of myocardial ischaemia. Postoperative chest radiograph showed that the catheter was in a midsternal position, at the level of the ascending aorta. The int

Digital subtraction angiography in the evaluation of chemodectomas.

During the last 2 years eleven patients with surgically confirmed chemodectomas have been investigated by means of digital subtraction angiography (DSA). Seven patients underwent i.v. DSA, which revealed eight carotid body tumours, and in the remaining four with jugulotympanic chemodectomas intra-arterial (i.a.) DSA was performed. We found i.V. DSA an easy and satisfactory method for the investigation of carotid body tumours but when glomus intravagale, tympanicum or jugulare is suspected an i.a. selective injection is required.

Effect of acute hepatic failure on epirubicin pharmacokinetics after intrahepatic arterial injection in rats.

In the case of cancer chemotherapy for hepatocellular carcinoma, anthracycline anticancer agents such as epirubicin are widely used, and have typically been given by intrahepatic arterial (i.a.) infusion to increase treatment efficacy and to reduce systemic toxicity. The anthracyclines are eliminated primarily by the liver, and the use of these drugs in patients with hepatic failure can be difficult. In this study, we investigated the effect of acute hepatic failure on the pharmacokinetics of epirubicin after i.a. injection in rats. Experimental acute hepatic failure was induced by carbon tetr

Myophosphorylase Knock Out Prevents the Exaggerated Exercise Pressor Reflex in Rats With Simulated Peripheral Artery Disease.

AIM: Controversy exists on which metabolites determine the exaggerated exercise pressor reflex (EPR) in peripheral artery disease (PAD). In decerebrated rats, we investigated the role played by lactate and hydrogen ions in a model of PAD, which was simulated by ligating the femoral artery for 72 h before the start of the experiment. METHODS: Production of lactate and hydrogen ions by the contracting hindlimb muscles was manipulated by knocking out the myophosphorylase gene (pygm). In both knockout (pygm-/-; n = 13; 6-females) and wild-type rats (pygm+/+; n = 14; 7-females), the EPR was evoked

Less Invasive Surgery for Remnant Stomach Cancer After Esophago-proximal Gastrectomy with ICG-guided Blood Flow Evaluation: A Case Report.

The standard procedure for remnant gastric cancer after esophago-proximal gastrectomy is total resection of the remnant stomach considering blood supply. However, sometimes surgery may be too invasive due to severe adhesion in the thoracic and mediastinal cavity. The blood supply to the remnant stomach depends on the right gastroepiploic artery and the right gastric artery. Therefore, preservation of the proximal region of the remnant stomach is thought to be anatomically impossible. We report a case of remnant gastric cancer that developed more than 12 years after lower thoracic esophagectomy
