Cases of gallstone ileus (108) were analyzed retrospectively over 30 years. Even today, when laparoscopic cholecystectomy is done, the incidence of this rare disease remains the same. History, clinical and X-ray findings are non-specific. Aerobilia was seen in 17% of the patients. Since 1992 sonography has provided an exact diagnosis in 10 of 15 cases. Review of complications of the ileal conduit in patients undergoing radical cystectomy for bladder cancer showed a statistically significant increase in renal calculi, ureteral obstruction, acute and chronic pyelonephritis and deterioration of renal function by 60 months postoperatively. A new case of appendicovesical fistula is reported. Usually a known cause is ulcerative colitis, morbus Crohn, malignancy of the appendix or as complication to perforated appendicitis, but in the described case there was no history of such previous illness. The only pathological finding was the presence of an abundant amount of nervous tissue in the appendix. It is concluded that a history of pneumaturia and/or recurrent urinary-tract infections must rise the suspicion of the presence of a fistula between the small or large intestine and the bladder.

Hirai and colleagues reported delayed gastroduodenal fistula formation 6 months after EUS-guided gallbladder drainage with a double-pigtail plastic stent. Persistent stent pressure from gallbladder contraction may cause bowel injury even with double-pigtail stents. Careful stent selection and positioning, together with regular imaging follow-up, may reduce injury and detect delayed complications.
