Formaldehyde is but one of many chemicals capable of causing the tight building syndrome or environmentally induced illness (EI). The spectrum of symptoms it may induce includes attacks of headache, flushing, laryngitis, dizziness, nausea, extreme weakness, arthralgia, unwarranted depression, dysphonia, exhaustion, inability to think clearly, arrhythmia or muscle spasms. The nonspecificity of such symptoms can baffle physicians from many specialties. The case is reported of a 19-year-old patient with recurrent Quincke edema with systemic symptoms. Based on observations of the course, diary entries and specific skin tests (scratch), sensitization to malt extract from germinated barley was diagnosed. The allergic reactions usually occurred after consumption of malt-containing chocolate drinks and malt-containing snack products. In cooperation with Prof. S. G. O. JOHANSSON, Stockholm, we have for the first time identified specific serum IgE antibodies against malt extract.

A number of 56 serofibrinous pleurisy cases were subjected to several usual diagnostic tests, as well as to virological investigations. Significant rises in serum antibody titers to different viruses were recorded in 25 cases; the presence of virus antigens in cells from the pleural fluid could be visualized by immunofluorescence in 11 patients. In many cases there was evidence suggesting multiple viral infection. The contribution of various investigation methods to differential diagnosis is discussed and the etiological patterns specific to different territories are outlined.

Six workers with occupational asthma due to toluene diisocyanate (TDI) were studied. For each worker a detailed clinical and occupational history was taken, and lung function measurement and skin intradermal tests for common allergens were carried out. Methacholine inhalation challenge was performed before TDI inhalation, and 8 h after TDI inhalation. Lymphocyte transfer tests (intradermal inoculations of 2·5 million of allogenic lymphocytes) were done in five normal persons, five lepromatous leprosy and four tuberculoid leprosy patients. Observations were made daily for at least 10 days. In a natural focus of paragonimiasis in the northeastern region of Venezuela, 130 people were clinically, parasitologically and immunologically evaluated. A specific intradermal test was positive in 13%. Besides an index case, three other active cases of paragonimiasis were identified. Adult Paragonimus worms and metacercariae were recovered from Didelphis marsupialis and Eudaniela garmani, respectively. Previous reports of Paragonimus infection in Venezuela are discussed.

During April 2005, 636 paediatricians and 192 GP took part in a survey about BCG, practices managed by InfoVac-France, InVS and AFPA.
