Impact of Iron Deficiency on Patients Undergoing Cardiac Surgery.

Iron deficiency (ID), with or without anemia, is a frequent and underrecognized condition among patients undergoing cardiac surgery, and it is associated with worse perioperative outcomes, including higher mortality, longer ICU and hospital stays, and increased transfusion requirements. This review summarizes current evidence on the prognostic role of ID and the effectiveness of supplementation strategies. While intravenous iron therapy has shown potential to improve hemoglobin levels and reduce transfusion needs, study results remain inconsistent, partly due to differences in timing, dosage,

FIT Testing in Iron Deficiency Anemia: A Retrospective Analysis.

OBJECTIVE: In low-risk populations, the faecal immunochemical FIT test is excellent at excluding colonic malignancy, with a negative predictive value (NPV) of 99.8%. However, effectiveness in a high-risk iron deficiency anaemia (IDA) population is unclear. This has significant resource implications for health care services worldwide. This study aims to establish the NPV of FIT for colorectal cancer in patients with IDA. DESIGN/METHOD: Single-centre retrospective study. Over 12 months, paired cases of FIT <10 µg Hb/g faeces and IDA were identified. Those not completing colonoscopy or CT pneumo

Selenoprotein I deficiency disrupts iron homeostasis in brain.

Selenoprotein I (SELENOI) is an essential enzyme for phospholipid synthesis that catalyzes the production of plasmenyl-phosphatidylethanolamine (plasmenyl-PE), a key component of myelin that protects and insulates axons. We recently generated a mouse model of central nervous system-restricted SELENOI deficiency that recapitulates features observed in humans with rare loss-of-function SELENOI gene mutations such as motor deficits, diminished plasmenyl-PE, and hypomyelination. Crucially, myelin-producing oligodendrocytes have the highest concentration of iron among cell types in brain. Iron is a

Beyond deficiency: The emerging health risks of excess iron.

ObjectiveTo examine the systemic biological effects of excess dietary iron and elevated hemoglobin in the context of the 2024 World Health Organization revision of anemia thresholds. This study integrates experimental evidence from BALB/c mice with previously proposed diagnostic indices (SF/TSAT and SF×TSAT) to distinguish iron overload from inflammation.MethodsThis study is based on an experimental murine model, with human evidence incorporated to provide contextual interpretation. BALB/c mice were exposed to graded oral iron supplementation, and hematocrit and glycemia were evaluated across

Plant physiology: Coumarin as a regulaTOR of root iron deficiency.

Coumarins have long been recognized for their roles in environmental adaptation and iron acquisition. A new study now suggests that the coumarin esculetin may directly inhibit TOR signaling by competing with ATP, linking iron deficiency to growth restriction in plants.

Improving Iron Deficiency Screening in Veterans With Heart Failure.

BACKGROUND: Iron deficiency (ID) is common among patients with heart failure. It is associated with reduced functional capacity, recurrent hospitalization, and worse outcomes. Despite guideline recommendations, ID screening remains low in clinical practice. PROJECT RATIONALE: The aim was to improve adherence to guideline-recommended ID screening among ambulatory veterans with heart failure and left ventricular ejection fraction <50% through a clinician educational intervention. PROJECT SUMMARY: We conducted a single-center quality improvement initiative in a Veterans Affairs ambulatory cardi

Prevalence and Adverse Outcomes of Iron Deficiency in Heart Failure.

INTRODUCTION/OBJECTIVE: Outcomes of iron deficiency (ID) in heart failure (HF) with preserved ejection fraction (HFpEF) or in samples with mixed heart failure subtypes are variably described. Hence, we investigated the prevalence and adverse outcomes of ID in a sample of mixed heart failure subtypes. METHODS: Adult patients admitted with HF over a six-month period were retrospectively studied. ID was defined as serum ferritin<100 mcg/L, or serum ferritin 100-300 mcg/L with serum transferrin saturation<20%. For each case of ID, sex- and age-matched (within five years) controls were selected. T

Iron Deficiency Anemia and Ischemic Stroke in Young Adults.

BACKGROUND: Iron deficiency anemia (IDA) is the most common cause of anemia and is linked to higher arterial stroke risk in children, but its effect in adults remains unclear. METHODS: We performed a retrospective study examining the prevalence of IDA in young adults with and without ischemic stroke. RESULTS: We found that the prevalence of IDA was five times higher in patients with ischemic stroke than those without stroke (5% vs. 1.05%). CONCLUSION: IDA independently increased the odds of an ischemic stroke by 39% (p < 0.001). Given the global burden of IDA, effective screening strategies

Iron Deficiency as an Underrecognized Cause of Dyspareunia and Vulvar Bleeding.

Iron deficiency is a common cause of anemia and is associated with a range of systemic effects. However, its direct impact on vulvovaginal tissue remains poorly described in the literature. We report the case of a 39-year-old woman with a history of gastric bypass surgery who presented with a 2-year history of severe dyspareunia and vulvar and vaginal mucosal tearing, including bleeding with minimal trauma, such as wiping after urination. The patient was seen by Gynecology for these symptoms, and a vulvar biopsy was suggested. Simultaneously, the patient was being treated for iron deficiency a

GmGLU1 and GmRR4 contribute to iron deficiency tolerance in soybean.

Iron deficiency chlorosis (IDC) is a form of abiotic stress that negatively impacts soybean yield. In a previous study, we demonstrated that the historical IDC quantitative trait locus (QTL) on soybean chromosome Gm03 was composed of four distinct linkage blocks, each containing candidate genes for IDC tolerance. Here, we take advantage of virus-induced gene silencing (VIGS) to validate the function of three high-priority candidate genes, each corresponding to a different linkage block in the Gm03 IDC QTL. We built three single-gene constructs to target GmGLU1 (GLUTAMATE SYNTHASE 1, Glyma.03G1

Bilateral Sudden Hearing Loss in Iron Deficiency Anemia.

The present study describes an unusual case of bilateral sudden hearing loss associated with iron deficiency anemia. Although hematologic disorders such as anemia or leukemia have been reported to be associated with sudden hearing loss, bilateral sudden hearing loss, which was presented as the first manifestation of iron deficiency anemia, has not been reported. A 74-year-old man presented with simultaneous bilateral sudden hearing loss without vertigo. A complete blood count test revealed a hemoglobin level of 6.4 g/dL and a ferritin level of 14.5 mg/mL, indicating iron deficiency anemia. Pos

Redefining Iron Deficiency in Patients With Chronic Heart Failure.

A serum ferritin level <15 to 20 μg/L historically identified patients who had absent bone marrow iron stores, but serum ferritin levels are distorted by the systemic inflammatory states seen in patients with chronic kidney disease or heart failure. As a result, nearly 25 years ago, the diagnostic ferritin threshold was increased 5- to 20-fold in patients with chronic kidney disease (ie, iron deficiency was identified if the serum ferritin level was <100 μg/L, regardless of transferrin saturation [TSAT], or 100 to 299 μg/L if TSAT was <20%). This guidance was motivated not by the findings of s

The impact of hematology electronic consultations on the management of iron deficiency.

OBJECTIVES: Delays in the evaluation and treatment of iron deficiency can lead to increased disease-related morbidity and mortality. Electronic consultation (e-consult) is a referral modality that allows providers quicker access to recommendations from a specialist based on electronic chart review. While the use of e-consult is expanding in classical hematology, gaps exist in the understanding of patient outcomes related to its use for iron deficiency. METHODS: We randomly selected 200 e-consults and 200 traditional referrals from 3,336 hematology referrals for iron deficiency at a single cen

Iron Deficiency in Patients with Left Ventricular Assist Devices.

Iron deficiency is a common and independent predictor of adverse outcomes in patients with heart failure. The implications of iron deficiency in patients implanted with a left ventricular assist device (LVAD) are less established. This review recaps data on the prevalence, characteristics and impact of Iron deficiency in the LVAD population. A systematic search yielded eight studies involving 517 LVAD patients, with iron deficiency prevalence ranging from 40% to 82%. IV iron repletion was not associated with adverse events and effectively resolved iron deficiency in most patients. However, the
