Treatment of Congenital Microgastria.

INTRODUCTION: Congenital microgastria is an extremely rare birth defect. The aim of this study was to present an overview of existing literature on the treatment of microgastria. MATERIALS AND METHODS: The term "microgastria" was used in a PubMed and Medline search. Since merely case reports were found, only a narrative synthesis with limited statistical analysis can be given. Data of different treatment modalities were collected and divided into two groups: conservative or less invasive treatment (C/LT, i.e., modified diet or a gastrostomy/jejunostomy) and extensive gastric surgery (EGS, i.e…

Successful salvage of late failure of hepatic portocholecystostomy (gallbladder Kasai) with Roux-en-Y cholecystojejunostomy.

The hepatic portoenterostomy (HPE) is the accepted initial operation for biliary reconstruction for biliary atresia, but in a select group of patients with patent distal extrahepatic bile ducts (PDEBD), a hepatic portocholecystostomy (HPC) may also be considered. A10 year old boy presented with sudden onset of jaundice following a successful HPC at 2 months of age. Radiographic evaluation revealed excretion into a distended gallbladder with distal biliary obstruction. He underwent a successful Roux-en-Y cholecystojejunostomy and remains jaundice-free two years later. Cholecystojejunostomy is a…

Bile duct stricture -- a sign of things to come?

OBJECTIVE: The aim of the report is to highlight this difficult to-treat condition hitherto uncommon in our environment and warn of the likelihood of more to come with the rising incidence of gallstone in our sub region. PATIENT AND METHOD: A case report of an 18-year-old male undergraduate patient. RESULT: A successful management with clinical, biochemical and radiological evidence (OTC and tubograms) despite inadequate investigative tool. PRINCIPAL CONCLUSION: With the rising incidence of gall stones in our environment, it is imperative that the Trainee Surgeon must be schooled in common

Small-bowel intussusception around a gastrojejunostomy tube resulting in ischemic necrosis of the intestine.

Gastrojejunostomy (GJ) tube feeding has made a significant difference in the lives of children with oropharyngeal motor problems and severe gastroesophageal reflux disease. It may, however, also lead to small-bowel intussusception. Previously, these intussusceptions have been reported as intermittent, self limiting, and innocuous. We report a case of a GJ-tube-related small-bowel intussusception that resulted in ischemic necrosis of the bowel. This case suggests that close monitoring and confirmation of successful reduction is necessary in the treatment of GJ-tube-related small-bowel intussusc…

Choledochal cyst in a pediatric patient: creation of a bilijejunal anastomosis with surgical staples. A case report.

Although the use of surgical staples is a well established practice in intestinal tract surgery on adults, their role in biliodigestive anastomoses in adults and children has been more limited. The Authors describe a 12-year-old girl with a type-IV choledochal cyst, who was successfully treated with cyst excision and Roux-en-Y hepaticojejunostomy created with a surgical stapler.

Comparison of 21 vs. 25 mm gastrojejunostomy in the gastric bypass procedure--early results.

BACKGROUND: Many bariatric surgeons consider a small gastrojejunostomy stoma critical to the success of gastric bypass. METHODS: We retrospectively compared a 21 vs. 25 mm gastrojejunostomy in 50 patients undergoing gastric bypass for morbid obesity. RESULTS: 31 patients were constructed with a 21 mm EEA, group I, and 19 with a 25 mm, group II. Average percent of excess body weight lost was 61%, 65%, and 64% at 12, 15, and 18 months follow-up in the group I patients, vs. 61%, 67%, and 69% in the group B patients. Differences were not significant. Complaints of nausea, vomiting, and/or dyspha…

Hepaticojejunostomy after excision of choledochal cyst in two children with previous biliary surgery.

Hepaticojejunostomy Roux-en-Y after excision of choledochal cyst is the treatment of choice for children with choledochal cyst, to prevent the risk of postoperative cholangitis, stone formation and malignancy. The author reports two children with previous biliary surgery for choledochal cyst, one with cystojejunostomy without cholecystectomy and the other with cholecystectomy alone. Two children were admitted to the Pediatric Surgical Unit, Ratchaburi Hospital, with the complaint of chronic abdominal pain. After investigation the two children had cyst excision and hepaticojejunostomy Roux-en-Y…

Radiation-Induced Airway Distortion Complicating Endotracheal Intubation: A Case Report.

Airway management in patients with a history of head and neck radiotherapy presents significant challenges due to radiation-induced fibrosis, anatomic distortion, and friable tissues. These changes increase the risk of difficult mask ventilation, failed intubation, and airway trauma. We present the case of a 77-year-old male with a history of oropharyngeal squamous cell carcinoma treated with chemoradiation who required general anesthesia for jejunostomy tube placement. Preoperative evaluation suggested a potentially difficult airway. Video laryngoscopy revealed a severely distorted and edemat…

IgG4-related disease presenting as a gastric mass necessitating total gastrectomy: a case report.

IgG4-related disease (IgG4-RD) is a multi-system disorder that rarely presents with gastric manifestations. A 26 year old woman presented with post-prandial symptoms of dyspepsia, dysphagia and reflux with a proximal submucosal gastric mass on imaging and endoscopy. She underwent a number of endoscopic and laparoscopic biopsies, which were inconclusive. Suspecting IgG4-RD, steroids were trialled empirically, but did not result in radiological or symptom improvement. Ultimately, she underwent a total gastrectomy, D2 lymphadenectomy and Roux-en-Y oesophago-jejunostomy. On final histopathological…

[Simple gastroschisis complicated by multiple perforations, frozen abdomen, and loss of abdominal domain].

Gastroschisis is a congenital malformation characterized by a visceral hernia and is one of the leading causes of short bowel syndrome in pediatric patients. This condition can result from congenital short bowel, but may also arise due to complications associated with the management of the abdominal wall defect. We present the case of a full-term newborn with simple gastroschisis who developed multiple gastrointestinal complications during initial management and was subsequently referred to our institution. On admission, the patient exhibited an open, frozen abdomen, enteroatmospheric fistulas

A Rare Case of Situs Inversus Partialis With Levocardia in a 73-Year-Old Patient.

Situs inversus partialis (SIP) is an extremely rare congenital disorder in which most of the visceral organs are located on the opposite sid … to insert a nasogastric tube into her reversed stomach and to explore other feeding options, e.g., jejunostomy. This report discusses a rare case of SIP, incidentally discovered in a patient with suspected esophageal malignancy, and its associated surgical challenges in her management.

Patients undergoing major surgery for gastrointestinal cancer are at high risk of developing or worsening malnutrition and sarcopenia. In ma … favouring the enteral route is recommended. Whether this approach should use a nasojejunal tube or jejunostomy is still a matter of debate. In the setting of enhanced recovery programmes with early discharge, nutritional follow-up and care should be continued beyond the short time in hospital. Case Report: Newborns With Pseudohypoaldosteronism Secondary to Excessive Gastrointestinal Losses Through High Output Stoma.

Life-threatening electrolyte imbalance is not uncommon in preemies. Differential diagnosis is important for immediate treatment. The syndrome of pseudohypoaldosteronism (PHA) is characterized by increased aldosterone secretion associated with clinical signs of hypoaldosteronism reflecting mineralocorticoid resistance. There are type I, type II, and secondary type of PHA. Most secondary PHA reported in the pediatric population result from urinary infection and obstructive uropathy and extremely rarely from gastrointestinal fluid loss. Seven preemies accepted jejunostomy or ileostomy, and they s…

Small-Bowel Adaptation: A Case of Morphological Changes Induced by Teduglutide in Short-Bowel Syndrome With Intestinal Failure.

Teduglutide (TED) reduces the need for parenteral support (PS) in patients with short-bowel syndrome with intestinal failure (SBS-IF). It is a glucagon-like peptide-2 analog that improves absorption, induces the expansion of the absorptive epithelium in the small intestine, and may be used in patients with SBS-IF after a 6- to 12-month adaptation period, if PS is always necessary. We described the functional and morphological effect of TED in a 40-year-old female patient with SBS-IF due to Crohn's disease who underwent terminal jejunostomy after 12 months of drug exposition. Marked hypertrophy…

Pancreatico-enteric fistula post pancreatic duct ligation for delayed haemorrhage complicating pancreaticoduodenectomy.

INTRODUCTION: Pancreatic fistula remains the main cause for postoperative morbidity following pancreaticoduodenectomy. The coincidence of sentinel bleed prior to post pancreatectomy haemorrhage (PPH) and pancreatic fistula is associated with very high mortality. PRESENTATION OF CASE: We report a case of pancreaticoduodenectomy complicated by postoperative leak and hematemesis. Severe delayed haemorrhage from the pancreatico-jejunostomy necessitated re-laparotomy and complete disconnection of the pancreatic anastomosis. Hemodynamic instability precluded a pancreatectomy or creation of a new an…

Surgical treatment of megaduodenum in familial visceral myopathy - report of a case and review of the literature.

INTRODUCTION: Familial visceral myopathy (VM) is a rare genetic disease that affects intestinal motility and results in pseudo-obstruction. Medical treatments can provide supportive measures but no curative treatment. CASE REPORT: A 20-year-old male with known diagnosis of VM was referred to our Unit in May 2013 with recurrent episodes of vomiting and hospital admissions not responding to medical treatment. Pre-operative imaging showed megaduodenum with marked delayed transit and normal small and large bowel transit. He underwent an elective Roux-en-Y duodeno-jejunostomy. The post-operative c…
