Lipid peroxidation is not the primary mechanism of bilirubin-induced neurologic dysfunction in jaundiced Gunn rat pups.

BACKGROUND: Hazardous levels of bilirubin produce oxidative stress in vitro and may play a role in the genesis of bilirubin-induced neurologic dysfunction (BIND). We hypothesized that the antioxidants taurourosdeoxycholic acid (TUDCA), 12S-hydroxy-1,12-pyrazolinominocycline (PMIN), and minocycline (MNC) inhibit oxidative stress and block BIND in hyperbilirubinemic j/j Gunn rat pups that were given sulfadimethoxine to induce bilirubin encephalopathy. METHODS: At peak postnatal hyperbilirubinemia, j/j Gunn rat pups were dosed with sulfadimethoxine to induce bilirubin encephalopathy. Pups were g

Perinatal aspects of bilirubin metabolism.

Study of bilirubin metabolism in the fetus and newborn continues to reveal new insights into fetal physiology and maternal-fetal-neonatal relationships. Therapeutic advances directed toward prevention as well as treatment of hyperbilirubinemia continue to expand our understanding of both the basic metabolic pathways and previously unsuspected interplay between host and environment. Seemingly insignificant alterations in temperature, oxygen, carbohydrate, or light itself have been shown to affect the delicate balances in bilirubin metabolism. Similar impact upon other developing systems undoubt

Heme oxygenase-1 genetic variants and the conundrum of hyperbilirubinemia in African-American newborns.

BACKGROUND: African-American (AA) infants are known to have, overall, lower bilirubin levels than infants of other ethnicities during their birth hospitalization. However, they are known to have a higher incidence of severe hyperbilirubinemia and are over represented in the US Kernicterus Registry. Heme oxygenase-1 (HO) is the rate limiting enzyme in heme metabolism leading to the equimolar production of bilirubin, carbon monoxide (CO) and free iron (Fe). Short (S) (GT)n repeats (<25) in the promoter region of the gene encoding the inducible HO-1 isozyme augment its expression, while long (L)

Component therapy.

The dramatic advances that have taken place in recent years in the care of sick and premature infants also have been matched by a similar increase in the use of blood transfusion therapy. Haematological features indicate that a newborn has a blood volume of 85-125 ml/kg the foetal haemoglobin is 60-85% and average Hb in full term infant is 18 gm/dl. By 2-3 months it falls to 11-12 g/dl the main cause of anemia are iron poor diet, weaning diets recurrent or chronic infections and hemolytic episodes in malarious areas. The red cells transfusions are usually top up transfusions, exchange transfus

Hyperbilirubinemia.

Hyperbilirubinemia is the most common condition requiring evaluation and treatment in newborns. The clinical manifestation of hyperbilirubinemia-jaundice-occurs in 60% of normal newborns and nearly all preterm infants. Compared with conditions that require advanced pharmacologic and technologic treatment strategies, hyperbilirubinemia seems to be overshadowed and may lose the attention it deserves as a condition that has potentially devastating effects. Nurses must be vigilant when caring for babies with "just jaundice" by monitoring bilirubin levels, identifying infants at risk for developing

Hyperbilirubinemia: current guidelines and emerging therapies.

It is estimated that about two thirds of newborns will appear clinically jaundiced during their first weeks of life. As newborns and their mothers spend fewer days in the hospital after birth, the number of infants readmitted yearly in the United States for neonatal jaundice over the last 10 years has increased by 160%. A portion of these infants present to the emergency department, requiring a careful history and physical examination assessing them for the risk factors associated with pathologic bilirubin levels. Although the spectrum of illness may be great, the overwhelming etiology of neon

Follow-up study of survivors after intra-uterine transfusion.

Between 1965 and 1976 a total of 217 infants received intra-uterine transfusions. Perinatal and follow-up data are presented on the 81 survivors, of whom 62 per cent were over five years of age at final follow-up. Assessment included development and general health, audiometric testing and school performance. Abnormalities were found in 29 of the survivors. In 13 cases these were attributed to haemolytic disease, and all had some neurological deficit such as differing degrees of deafness. 10 others had slight abnormalities associated with the procedure of intra-uterine transfusion, while six ha

Risk factors for neurotoxicity in newborns with severe neonatal hyperbilirubinemia.

OBJECTIVE: To evaluate the importance of total serum bilirubin (TSB) and neurotoxicity risk factors in predicting acute bilirubin encephalopathy (ABE) at admission or posttreatment bilirubin encephalopathy (BE) in infants with severe hyperbilirubinemia. METHODS: We analyzed the interaction of TSB and risk factors as determinants of ABE and BE in 249 newborns admitted with a TSB level of ≥ 25 mg/dL (427 μmol/L) to Cairo University Children's Hospital during a 12-month period. RESULTS: Admission TSB values ranged from 25 to 76.4 mg/dL. Forty-four newborns had moderate or severe ABE at admissio

Characterization of severity of hemolytic disease of the fetus and newborn due to Rhesus antigen alloimmunization.

BACKGROUND: Clinical manifestations of hemolytic disease of the fetus and newborn include anemia, hyperbilirubinemia, hydrops fetalis, kernicterus, and fetal or neonatal demise. More than 50 antibodies are linked to hemolytic disease of the fetus and newborn, with Rhesus (including D and c) and Kell antigens carrying the highest risk of disease. To date, a multicenter, comprehensive evaluation of hemolytic disease of the fetus and newborn due to Rhesus antigen alloantibodies in the United States has not been undertaken. OBJECTIVE: This study aimed to implement a novel severity index to charac

The effects of benzodiazepine use during pregnancy and lactation.

Although there are a number of studies and individual case reports concerning the use of benzodiazepines in human pregnancy, the data concerning teratogenicity and effects on postnatal development and behaviour are inconsistent. There is evidence from studies in the 1970s that first trimester exposure to benzodiazepines in utero has resulted in the birth of some infants with facial clefts, cardiac malformations, and other multiple malformations, but no syndrome of defects. Diazepam and chlordiazepoxide are amongst the drugs most frequently implicated in the earlier studies. However, data from

Intermittent phototherapy versus continuous phototherapy for neonatal jaundice.

BACKGROUND: Phototherapy is a widely accepted, effective first-line therapy for neonatal jaundice. It is traditionally used continuously but intermittent phototherapy has been proposed as an equally effective alternative with practical advantages of improved maternal feeding and bonding. The effectiveness of intermittent phototherapy compared with continuous phototherapy is unknown. OBJECTIVES: To assess the safety and effectiveness of intermittent phototherapy compared with continuous phototherapy. SEARCH METHODS: Searches were conducted on 31 January 2022 in the following databases: CENTRA

Neuroinflammation and ER-stress are key mechanisms of acute bilirubin toxicity and hearing loss in a mouse model.

Hyperbilirubinemia (jaundice) is caused by raised levels of unconjugated bilirubin in the blood. When severe, susceptible brain regions including the cerebellum and auditory brainstem are damaged causing neurological sequelae such as ataxia, hearing loss and kernicterus. The mechanism(s) by which bilirubin exerts its toxic effect have not been completely understood to date. In this study we investigated the acute mechanisms by which bilirubin causes the neurotoxicity that contributes to hearing loss. We developed a novel mouse model that exhibits the neurological features seen in human Bilirub

[Follow-up of pregnancies with red-cell allo-immunisation: State-of-the art].

Anti-RhD allo-immunisation has become rare since anti-D prophylaxis was introduced in the seventies; however, it remains the first cause of fetal anemia. It may cause severe fetal complications such as fetal hydrops, cerebral anoxic lesions and fetal death. In the neonatal period, severe jaundices and anemias requiring transfusion or exsanguino-transfusion are still common in case of severe allo-immunisation. Neonatal death and sequellae due to bilirubin encephalopathy have not fully disappeared. Follow-up of pregnancies with maternal allo-immunisation requires identification of the antibody (
