Selective chemical vestibulectomy. Preliminary results.

In an attempt to destroy the peripheral vestibular labyrinth selectively in patients suffering from intractable vertigo as a result of end-stage Ménière's disease, 25 patients underwent selective chemical vestibulectomy between 1989 and 1994. As part of the procedure a certain quantity of streptomycin was placed between the bony and the membranous part of the lateral semicircular canal. The dosage of streptomycin used for successful ablation of vertigo varied between 250 and 100 micrograms. Within 12 months following the procedure all patients experienced a tremendous improvement in their over

Temporal bone studies in anencephaly.

The principal anatomical and histological features of the temporal bones of three anencephalics are described, one a twin-headed monster, with four temporal bones. These may be summarized as follows: Malformation of ossicles. Mondini type malformation of the cochlea. Occasionally cochlear hydrops or collapse of Reissner's membrane, following apparently on its rupture. Distension of the vestibular labyrinth and oedema of the perilymphatic spaces and membranes. Rupture of the saccular wall with 'sealing off' and repair of the tears and gaps by coagulated lymph material. Fibrous obliteration of t

Bilateral Vestibular Dysfunction.

Bilateral vestibular dysfunction (BVD) refers to hypofunction of the vestibular nerves or labyrinths on both sides. Patients with BVD present with dizziness, oscillopsia, and unsteadiness, mostly during locomotion, which worsen in darkness or on uneven ground. Although aminoglycoside ototoxicity, Meniere's disease, infection, and genetic disorders frequently cause BVD, the etiology remains undetermined in up to 50% of the patients. The diagnosis of BVD requires both symptoms and documentation of deficient vestibulo-ocular reflex function using head-impulse, bithermal caloric, and rotatory chai

Functional outcome of treatment of Ménière's disease with the Meniett pressure generator.

CONCLUSION: Use of the Meniett pressure generator in patients with active Ménière's disease (MD) did not affect labyrinthine function. OBJECTIVE: To investigate whether application of the Meniett pressure generator in patients with active MD significantly affects their labyrinthine function. MATERIAL AND METHODS: We measured hearing and labyrinth sensitivity in patients with established MD according to American Academy of Otolaryngology-Head and Neck Surgery (AAO-HNS) criteria at five different time periods: at baseline; before ventilation tube placement; before Meniett treatment; immediatel

Rieger syndrome: case report.

Rieger syndrome is a dysembryogenetic disease in which labyrinthic damage can be associated with other genetic anomalies. The case presented here is of a patient who has bilateral dysgenesis of the iris, with bulbar atrophy and dyscoria. The patient does not present any malformation of the craniofacial structures, of the periumbilical skin, or of the skeletal bones. The case is, therefore, a variant of the Rieger syndrome labeled Axenfeld-Rieger syndrome. The patient reported a progressive sensation of auricular fullness, and liminal audiometry revealed a sensorineural hearing loss. Computed t

Acute vestibulopathy.

Acute vestibulopathy is characterized by the acute or subacute onset of vertigo, dizziness or imbalance with or without ocular motor, sensory, postural or autonomic symptoms and signs, and can last for seconds to up to several days. Acute vestibular lesions may result from a hypofunction or from pathological excitation of various peripheral or central vestibular structures (labyrinth, vestibular nerve, vestibular nuclei, cerebellum or ascending pathways to the thalamus and the cortex). This update focuses on new aspects of the aetiology, pathophysiology, epidemiology, and treatment of (i) acut

Study of systemic lupus erythematosus in temporal bones.

Despite some reports of sensorineural hearing loss with systemic lupus erythematosus (SLE), its pathologic correlate has remained unidentified due to the scarcity of human temporal bone studies. We here present findings in 14 temporal bones from 7 patients with SLE, examined histologically and immunohistochemically for pathologic conditions in the cochlea that might relate to their otologic histories. Blue-staining concretions were seen in the stria vascularis of 6 ears. Most of the cases showed a loss of spiral ganglion cells, with various degrees of hair cell loss and atrophy of the stria va

Further ultrastructural studies of lesions produced in the optic nerve by tumor necrosis factor alpha (TNF-alpha): a comparison with experimental Creutzfeldt-Jakob disease.

We report here that intraocular injection of recombinant TNF-alpha produces lesions in the optic nerve indistinguishable from those reported for the panencephalopathic type of Creutzfeldt-Jakob disease (CJD). The lesions were patchy and confined to the injected optic nerve. Axons show variable features of degenerations. Numerous vacuoles distended the myelin sheath. Hypertrophic astrocytes were numerous and many active macrophages containing digested myelin debris and lyre-like paracrystalline bodies. At high power, myelinated axons were observed as enveloped by astrocytic processes; formation

[Dynamics of neurologic disorders in epileptics].

On the basis of the dynamic study of the neurological and otoneurological picture of epilepsy in 105 patients, the authors ascertained a definite complex of symptoms depending on the stage of the disease. Most frequently observed were symptoms of the involvement of the truncal-subcortical systems, which reflected the stage of the progression of the epileptic process associated with a sharp increase in the frequency (up to the daily pattern) and rhythm of attacks, as well as psychic disturbances, with the structure of paroxysms becoming more complicated and polymorphic. The pathological symptom

[The role of the inner ear biological barrier in targeted drug delivery for inner ear diseases].

The endolymphatic barrier, composed of tight junctions and ion transporters, regulates the ionic gradient between endolymph and perilymph and plays a pivotal role in maintaining inner ear function. Disruption of this biological barrier can result in ionic imbalance, causing both auditory and vestibular dysfunction. Medication needs to enter the perilymph instead of the endolymph by passing through the perilymphatic barrier to reach the hair cells. Targeted drug delivery has emerged as a promising therapeutic strategy for inner ear diseases, including intratympanic injection, intracochlear infu

[Theory and innovative clinical practice of inner ear comorbidity].

Inner ear comorbidity (IEC) refers to a group of diseases caused by lesions in the cochlea, vestibule, and their neural pathways. These diseases are usually accompanied by hearing loss, tinnitus, vertigo and other symptoms simultaneously or dynamically alternating, forming a comorbidity phenomenon with inner ear dysfunction as the core. Inner ear comorbidities increase the complexity and cost of diagnosis and treatment, putting high difficulty and stress on clinicians and health systems. At present, the main symptoms are mostly treated in clinical practice, and the inherent connection between

Intratympanic Gels for Inner Ear Disorders: A Scoping Review of Clinical Trials.

OBJECTIVE: Intratympanic injections are a safe, well tolerated procedure routinely performed by ENT's specialists. Intratympanic injections of gels have the potential to deliver therapeutics into the cochlea through the round window membrane prolonging the release of drugs in the inner ear compartment. Aim of the present review is to summarize clinical trials testing pharmacological treatments for inner ear pathologies through intratympanic gel formulations. DATA SOURCES: Online databases (Google scholar and PubMed) and registers (Clinicaltrials.gov and Euclinicaltrial) were used to identify

Central vestibular dysfunction: don't forget vestibular rehabilitation.

INTRODUCTION: Vestibular rehabilitation (VR) is now a subject of active studies and has been shown to be effective for multiple vestibular disorders, peripheral or central. VR is a physical therapy that helps train the central nervous system to compensate for vestibular dysfunction. There is moderate to strong evidence that VR is safe and effective for the management of peripheral vestibular dysfunction. Nonetheless, the studies on how VR works on central vestibular dysfunction remains scanty. AREAS COVERED: This article addressed the rehabilitation strategies and possible mechanisms, includi

Vestibular migraine: Diagnostic criteria1.

This paper presents diagnostic criteria for vestibular migraine, jointly formulated by the Committee for Classification of Vestibular Disorders of the Bárány Society and the Migraine Classification Subcommittee of the International Headache Society (IHS). It contains a literature update while the original criteria from 2012 were left unchanged. The classification defines vestibular migraine and probable vestibular migraine. Vestibular migraine was included in the appendix of the third edition of the International Classification of Headache Disorders (ICHD-3, 2013 and 2018) as a first step for

Vestibular migraine.

Vestibular migraine is now considered a distinct diagnostic entity by both the Barany Society and the International Headache Society. The recognition of vestibular migraine as a diagnostic entity required decades and was presaged by several reports indicating that a large proportion of patients with migraine headaches have vestibular symptoms and that a large proportion of patients with undiagnosed episodic vestibular symptoms have migraine headache. Despite the availability of diagnostic criteria for vestibular migraine, challenges to diagnosis include variability in terms of the character of

[Superior semicircular canal dehiscence syndrome treated by calcium phosphate cement canal plugging: a case report].

Superior semicircular canal dehiscence (SCD) syndrome exhibits pressure- and/or sound-induced oscillopsia and vertigo due to an absence of bone overlaying the superior semicircular canal. A 33-year-old man with right-ear SCD reported dizziness in a noisy hall three years earlier followed by dizziness during flatulence, straining or coughing, and right-ear autophony. Audiography showed a right-ear low-frequency air-bone gap, with positive Tullio phenomenon and a Valsalva maneuver against closed glottis causing torsional-vertical nystagmus. Temporal-bone computed tomography (CT) led to a diagnos
