Postprocedural Management: Anticoagulation and Beyond.

Excluding the left atrial appendage in patients with nonvalvular atrial fibrillation is a mechanical way to decrease stroke risk. During endothelialization, the closure device is exposed to circulating blood, which might activate the coagulation cascade. In excessive proportions, possibly resulting in the development of device-related thrombus, requiring a bridging period with optimal antithrombotic treatment. Initial protocol treatment is less suitable for patients with contraindications to anticoagulation. Less intensive antithrombotic regimens investigated suggest safety and efficacy, howev

Interventional imaging: the role of echocardiography.

Interventional echocardiography is a rapidly evolving field requiring imaging expertise. An increasing number of structural heart interventions now require real-time imaging guidance for device placement and immediate functional evaluation. Continuous 2- and 3-dimensional transesophageal echocardiography are now required by many heart teams during complex structural interventions, including percutaneous closure of atrial septal defects, left atrial appendage occlusion, transcatheter aortic valve replacement (TAVR), transcatheter repair of paravalvular regurgitation, and percutaneous mitral val

Left atrial appendage occlusion.

Prevention of stroke represents a goal of primary importance in health systems due to its associated morbidity and mortality. As several patient groups with increased stroke rates have been identified, multiple approaches have been developed and implemented: oral anticoagulation (OAC) for patients with atrial fibrillation, surgical and percutaneous revascularisation in patients with carotid disease, device closure for patients with patent foramen ovale, and now, left atrial appendage occlusion (LAAO) for selected patients with non-valvular atrial fibrillation (NVAF). The latter group of patien

Elongated Channels Between Surgically Excluded LAA and LA: Detected by CTA But Missed by TEE.

BACKGROUND: Systematic left atrial appendage (LAA) exclusion during mitral valve surgery reduces thromboembolic risk in patients with pre- or postoperative atrial fibrillation. Rare residual LAA-left atrial (LA) communications may occasionally occur after surgical exclusion and increase thromboembolic risk, particularly in patients not receiving anticoagulation. CASE SERIES: Among more than 2,000 robotic mitral valve surgeries performed at NYU Langone Health from 2010 to 2025, LAA closure with LA posterior wall plication was performed using a continuous back-and-forth suture technique. 3D Intracardiac Echocardiography for Structural Heart Interventions.

Transcatheter structural heart interventions are expanding into more complex spaces including mitral transcatheter edge-to-edge repair, left atrial appendage occlusion, tricuspid transcatheter edge-to-edge repair, mitral/tricuspid valve-in-valve, and perivalvular leak closures. Transesophageal echocardiography (TEE), with concomitant fluoroscopy, has remained the gold standard for many of these interventions. Although three-dimensional intracardiac echocardiography has been used, applications were often limited to guidance for more "simple" procedures such as patent foramen ovale/atrial septal

Conscious sedation versus general anesthesia for transesophageal echocardiography guidance of percutaneous left atrial appendage occlusion.

OBJECTIVES: This study sought to compare the transesophageal echocardiography (TEE)-guided left atrial appendage occlusion (LAAO) procedures under general anesthesia with conscious sedation. BACKGROUND: In the expanding field of LAAO procedures, developing minimalistic approaches that eliminate the need for general anesthesia is crucial. Conscious sedation is spreading as an alternative for performing TEE-monitored LAAO procedures. METHODS: For this study, patients enrolled in the registry who underwent LAAO under conscious sedation were compared with those who received general anesthesia. O

Recent developments in next-generation occlusion devices.

Transcatheter closure has been widely accepted as a highly effective way to treat abnormal blood flows and/or embolization of thrombus in the heart. It allows the closure of four types of congenital heart defects (CHDs) and stroke-associated left atrial appendage (LAA). The four types of CHDs include atrial septal defect (ASD), patent foramen ovale (PFO), patent ductus arteriosus (PDA), and ventricular septal defect (VSD). Advancements in the materials and configurations of occlusion devices have spurred the transition from open-heart surgery with high complexity and morbidity, or lifelong med

Life-long management of atrial fibrillation in a patient with hypertrophic obstructive cardiomyopathy following alcohol septal ablation: A10-year follow-up case report.

Alcohol septal ablation (ASA) is an established septal reduction therapy for patients with drug-refractory hypertrophic obstructive cardiomyopathy (HOCM). However, its impact on the long-term incidence of atrial fibrillation (AF) remains unclear. We report a case of a 68-year-old man with HOCM who underwent successful ASA with an improvement in heart failure (HF) symptoms. Four years later, recurrent AF developed, requiring catheter ablation twice. Subsequently, subarachnoid hemorrhage occurred during anticoagulation therapy. Because of the high thromboembolic and bleeding risks and unsuitable

Asymptomatic migration of an AMPLATZER™ Amulet™ left atrial appendage occluder through the mitral valve.

An 84-year-old woman with a history of hemorrhagic stroke was admitted for a new cerebral ischemic event. While in hospital, she was diagnosed with paroxysmal atrial fibrillation. Owing to her high thromboembolic risk (CHA2 DS2 -VASc score of 6) we performed left atrial appendage (LAA) closure using a 22-mm AMPLATZER™ Amulet™. The procedure was successful. Systematic follow-up with transthoracic echocardiography 8 hr later revealed that the LAA occluder had migrated and become entrapped in the mitral subvalvular apparatus. At this time, the patient was asymptomatic. The device was recovered pe

WATCHMAN versus ACP or Amulet devices for left atrial appendage occlusion: a sub-analysis of the multicentre LAARGE registry.

AIMS: Several left atrial appendage (LAA) closure systems are available. Due to differences in device design, the results of specific occluders derived from trials cannot simply be generalised to all devices. The present analysis sought to assess two contemporary LAA closure devices in clinical practice. METHODS AND RESULTS: The work represents a non-randomised sub-analysis of the prospective, multicentre, Left-Atrium-Appendage Occluder Register - GErmany (LAARGE) registry. The WATCHMAN (group 1) and the AMPLATZER Cardiac Plug (ACP) or Amulet occluder (group 2) were assessed. A total of 641 p

Left atrial appendage aneurysm: a descriptive systematic review of 177 cases.

INTRODUCTION: The left atrial appendage (LAA) is a distinct structure with unique developmental and structural characteristics. The LAA is involved in the formation of intra-atrial thrombi, particularly in patients with conditions such as atrial fibrillation and mitral valve disease. Left atrial appendage aneurysms (LAAA) are rare abnormal dilations of the LAA that may cause hazardous complications. However, there are limited data on the demographic features, clinical characteristics, management modalities, and prognosis of LAAA patients. This study aimed to conduct a systematic review of the
