BACKGROUND: Suicide is a leading cause of death among young males in Australia. Tomorrow Man's "Breaking the Man Code" workshops aim to challenge potentially harmful masculine norms and promote positive attitudes towards help-seeking among adolescent boys in schools.
METHODS: Our stratified cluster randomized trial was undertaken with 1225 boys (mean (SD) age = 15.04 (0.74) years) across 24 Australian secondary schools: 13 randomized to intervention, 11 to waitlist control. Primary outcome was mean change in intentions to seek help for personal or emotional problems 4 to 8 weeks from baseline

OBJECTIVE: Factors that influence the decision for withdrawal of life-supporting treatment (WLST) in patients with severe traumatic brain injury (sTBI) are incompletely understood.
METHODS: The authors conducted a retrospective cohort study using the 2016-2022 American College of Surgeons Trauma Quality Programs database to identify demographic and clinical factors associated with the decision for WLST in patients with sTBI. Multivariable logistic regression analysis was conducted. Hospital length of stay (LOS), intensive care unit LOS, number of days on a ventilator, and disposition outcomes

Adolescent idiopathic scoliosis (AIS), affecting approximately one in every 25 children, presents significant lifelong physical and psychosocial challenges. However, a systematic understanding of the sources of this uncertainty and how individuals communicatively navigate it is lacking. Thus, this qualitative study applied uncertainty management theory (UMT) to examine participants' uncertainty associated with scoliosis. The investigation identified key sources of uncertainty (medical, financial, social, and career) and management strategies through semi-structured interviews with 21 individua

BACKGROUND AND OBJECTIVES: Over half of all deaths after traumatic brain injury (TBI) follow the decision to withdraw life-sustaining therapy (WLST). Despite recent improvements in TBI mortality, rates of WLST have remained unchanged, potentially reflecting outdated prognostic misconceptions. Determinants of the decision to WLST are multifaceted and complex, spanning just beyond clinical prognosis, which is reflected in the significant variability in WLST rates between centers. We aimed to create a machine learning (ML) model that could accurately predict the decision to WLST and hypothesized

BACKGROUND: This study aims to facilitate parental identification of designated emergency facilities for expeditious pediatric care within the framework of Taiwan's newly implemented "regional joint defense" approach to pediatric emergency services. The research seeks to elucidate the mechanisms by which this novel system can enhance timely access to appropriate emergency care for children, potentially improving health outcomes and resource utilization in acute pediatric situations.
METHODS: Factor analysis (FA) and triangular entropy matrix (TEM) analyzed the appearance, breathing and skin o

Penetrating torso trauma is the second leading cause of death following head injury. Traffic accidents, falls and overall blunt trauma are the most common mechanism of injuries in developed countries; whereas, penetrating trauma which includes gunshot and stabs wounds is more prevalent in developing countries due to ongoing violence and social unrest. Penetrating chest and abdominal trauma have high mortality rates at the scene of the incident when important structures such as the heart, great vessels, or liver are involved. Current controversies surround the optimal surgical approach of these

INTRODUCTION: In highly developed countries, as many as 16 percent of children are physically abused each year. Traumatic brain injury (TBI) is the most common injury in non-accidental trauma (NAT) and is responsible for 80 percent of fatal NAT cases, with most deaths occurring in children younger than three years old. Cases of abusers who refuse withdrawal of life-sustaining medical treatment (LSMT) to avoid criminal charges have previously been reported. Therefore, we hypothesized that NAT is associated with a lower risk for withdrawal LSMT in pediatric TBI.
METHODS: The pediatric Trauma Qu

BACKGROUND: Recognition and management of pediatric dysrhythmias is challenging for community emergency department (CED) providers, given their infrequent exposure to these cases.
METHOD: A prospective, interventional study measured adherence of CEDs to pediatric supraventricular tachycardia (SVT) algorithm pre- and postimplementation of an in situ simulation-based collaborative program. CED teams' adherence was scored using a composite adherence score (CAS) based on the number of actions scored correctly on the performance checklist.
RESULTS: A total of 74 multiprofessional teams from nine

Women are increasingly surviving breast cancer, but up to 90% experience unexpected long-term sequelae as a result of treatment. Symptoms may include physical, functional, emotional, and psychosocial changes that can dramatically alter the quality of life for breast cancer survivors. Primary care clinicians, including midwives, are likely to care for these women and should be familiar with common symptoms, treatment, and best practices to avoid permanent dysfunction. A holistic approach to assessment, treatment, and referral as needed is the most effective strategy. Although women experience s

PURPOSE: To examine the feasibility of a new open-area body weight support system (BWSS) to act as both an "assistive" and a "rehabilitative" device within the home.
INTERVENTION: A 5-year-old boy with spina bifida used the BWSS during self-selected activities for 10 weeks. Feasibility, behavioral, and clinical assessments provided a quantification of his activity in and out of the BWSS.
OUTCOMES: On average, the child used the device on 2.7 days/week and for 67 minutes/day during intervention. When in the BWSS (assistive role), the child's locomotor activity and engagement in adapted sports

The patient is a unique case presenting with presumed Restrictive Dermopathy (RD) and intracranial and adrenal calcifications, an association not previously described in the literature. This case postulates the possibility of additional radiographic features that can be included in the spectrum of RD or as secondary events from the underlying pathophysiology of RD.

Advances in critical care medicine have made it possible to sustain vital organ systems in brain-dead patients. One clinical scenario besides donor organ retrieval in which a benefit may be gained from continuing life support is pregnancy. A pregnant woman in her late 30's at 23 weeks gestation exhibiting worsening depression was referred to the Department of Psychiatry. One day after admission she attempted suicide by hanging and suffered a cardiopulmonary arrest. A fetal heart beat and fetal motion was confirmed immediately after resuscitation. Three days after admission, an emergency Cesare

Many patients with chronic obstructive pulmonary disease (COPD) die each year as those with lung cancer but current guidelines make few recommendations on the care for the most severe patients i.e. those with Global Initiative for Chronic Obstructive Lung Disease (GOLD) stages III and IV with chronic respiratory failure. Only smoking cessation and long term oxygen therapy (LTOT) improve survival in COPD. Although non invasive positive pressure ventilation (NPPV) may have an adjunctive role in the management of chronic respiratory insufficiency there is little evidence for its use in the routin

Traumatic injury remains the leading cause of potentially preventable death in Canadians under age 40 years. Although only a minority of patients present with hemodynamic instability, these patients have a significant chance of dying. The causes of instability must be recognized and corrected quickly by using a systematic approach. To allow key supportive interventions to be undertaken swiftly, it is more important to identify and prioritize systemic compromise than to confirm specific diagnoses. Most potentially preventable trauma death relates to airway obstruction, hemopneumothorax, intracr

A 31-year-old woman had fatal intracerebral bleeding at the beginning of the 21st week of gestation. After several days, there was evidence that the brain was dead. Combined enteral and parenteral nutrition was continued until the 32nd week of gestation when cesarean section was performed because of drug-resistant hypotension. A full-term normal 1600-g male was delivered and the later development of the child was normal. This case report demonstrates the possibility of providing nutritional requirements to the fetus even if the mother has had a fatal injury.
