The purpose of this article is to report a case of lip adhesion as a complication of erythema multiforme in an 8-year-old child. This is the second reported case of this complication of oral ulceration associated with erythema multiforme.

A case of reconstruction of the upper lip with a bipedicled submental flap is described. Although the mobility of the flap was restricted, the arterial blood supply was abundant by altering the single-pedicled flap into a bipedicled flap. This flap is a good color and texture match to the face, and surpasses other flaps in reconstruction of the mustache or beard in male patients.

In this paper we will review the results obtained during the last 2 years with the aponeurotic galea and subgalea for vermillion lip augmentation. The survey was carried out on 42 patients who displayed either an absence of or reduction in the vermillion of one or both lips or a senile lip. Micronostril is a cicatricial stenosis that may occur in the caudal-most portion of the nasal cavity of cleft lip patients. This stenosis can be severe and functionally debilitating. Previously proposed solutions have offered disappointing results. Our solution has three key elements: adequate scar release; restoration of the lining and support by way of chondrocutaneous composite graft; and post-operative custom acrylic splinting.

We describe a 6-month-old Turkish girl who was found to have a mass near her nose at birth and incomplete cleft lip. No other clinical and radiological abnormalities were demonstrable elsewhere in her body. Histological examination of the excised mass showed an organoid structure that was consistent with an accessory nose.

Reconstruction of the total upper lip may be easily performed using fan flaps. However, two details must be taken into consideration: 1) the wide pedicle in the commissural angle, including the facial artery, must be designed, and 2) the mucosal flap must be 8 mm longer, in the inner portions, than the skin flap used to reconstruct the lip semimucosa.

Female thoraco-omphalopagus twins were of equal size and had similar abnormalities related to the twinning site. However, one twin was more severely affected and also had cleft lip and palate which the co-twin did not have. The implications of this are discussed.

Aesthetic units of the face have been previously described. The lip itself may be divided into smaller topographic subunits. The lateral subunit is bordered by philtrum column, nostril sill, alar base, and nasolabial crease, while the medial topographic subunit is one-half the philtrum. When a large part of a subunit has been lost, replacing the entire subunit rather than simply patching the defect often gives a superior result. A survey was made of the upper cervical vertebrae in children with cleft lip and palate in order to determine the prevalence of cervical vertebral anomalies. The cleft sample consisted of 105 patients attending for orthodontic treatment. Nine midline sinuses of the upper lip in eight patients have been described previously. They have been located from the skin under the columella to the frenulum, and have always been directed towards the frenulum. Three new cases are presented: a sinus reaching from the skin of the philtrum to the frenulum in a child with Pierre Robin syndrome, another one in the frenulum of a patient with two separate midline sinuses of the nose, and a third in the frenulum of a child with a partial median cleft lip. This longitudinal study analyzed the articulation development of 108 children with cleft lip/palate from three through eight years of age. The data suggest that as the severity of clefting increases the severity of the articulation deficit does also. Age and type of cleft were statistically significant factors in the development of normal articulation skills. A fifteen-year survey of primary lip repairs done at The New York Hospital revealed that lip closure can be accomplished during the first week of life with no mortality and with major morbidity slightly higher than that in repairs done after one week of age. Modern anesthetic techniques have markedly reduced respiratory complications from lip closure. Adequate postoperative sedation coupled with careful patient monitoring should appreciably reduce the incidence of wound dehiscence.
