Granulocytic sarcoma or chloroma is defined as an extramedullary solid tumor, composed of granulocytic precursor cells at various levels of differentiation. Granulocytic sarcoma is mostly associated with acute and chronic leukemia, rarely with polycythemia vera and myelofibrosis. The use of special immuno-histochemical stains is mandatory, because this tumor is often misdiagnosed as malignant lymphoma. We report two cases of granulocytic sarcoma (tonsil and ovary) associated with chronic myelogenous leukemia. We describe the first reported association of atypical CML and granulocytic sarcoma.

Liver cell adenoma as a cause of spontaneous haemoperitoneum has been noted with increasing frequency since Baum's initial report (Baum et al., 1973). The relationship between the use of oral contraceptives and bleeding from a hepatic tumour makes awareness of this condition mandatory for the physician involved in the differential diagnosis of the acute abdomen. The report presented here is that of a woman who, while taking oral contraceptives, subsequently bled from her liver 3 years after an initial hepatic haemorrhage.

The technology allowing freezing of RBC units has been available for many decades. The high-glycerol method for RBC storage at -8 degrees C is predominantly used. Several studies have shown satisfactory results regarding the in vitro viability and function of cryopreserved RBCs. Oesophageal complications after endovascular aortic stent repair are rare, but may lead to catastrophic consequences. Early detection is mandatory, but is sometimes difficult because of a lack of specific signs in the early stages. We report 2 cases with opposing results of oesophageal complications after aortic stent graft repair, and discuss the early signs of this disastrous complication and potential methods for early detection.

The present authors report the case of a 9-year-old boy affected by psoriasis, juvenile psoriatic arthritis, enthesitis, and celiac disease. The signs and symptoms of the different comorbidities appeared at different times in the clinical history, complicating the overall diagnostic and therapeutic procedures. Cooperation between dermatologists and rheumatologist is mandatory in similar cases, where sophisticated technology and teams with specialized and integrated knowledge are required.

Tuberous sclerosis is a rare autosomal dominant syndrome in which aortic aneurysm is a classic but little known cause of sudden death. We report a 20-month-old boy with tuberous sclerosis and iliofemoral arterial aneurysms. This case highlights the importance of systematic screening for such vascular complications. Multislice CT or MRI is mandatory as a screening and diagnostic tool in this setting to complement US with Doppler examination.

We report a case of polyarteritis nodosa with a clinical presentation mimicking polymyalgia rheumatica, as well as pathological findings of non-giant-cell arteritis on temporal artery biopsy with symptoms of jaw claudication. Although certain clinical syndromes have been attributed to specific types of systemic vasculitis, considerable overlap occurs. Obtaining tissue biopsy in cases of vasculitis is mandatory for diagnosis and classification.

The authors report a case of a thoraco-abdominal duplication of the foregut, observed in a 10-year old boy. Thoraco-abdominal duplication is an expression of notochordodysraphia, as it is frequently associated with vertebral malformations. Barium meal, ultrasonography and CT-Scan should provide a preoperative diagnosis. One-stage total excision in a single stage is mandatory in order to treat the symptoms and avoid the risks of further progression.

Intrauterine supraventricular tachycardia with consequent congestive heart failure is a high-risk fetal complication. It is one of the causes of non-immunological hydrops fetalis and may lead to premature delivery or even fetal loss. We report the successful intrauterine treatment of a case of hydrops fetalis due to supraventricular tachycardia by administration of digoxin and verapamil to the mother. Echocardiography is diagnostic and mandatory for follow up.

Eight patients with end-stage renal disease (ESRD) who developed bacterial infection of the perirectal area or perineum are reported. The diagnosis was not always straightforward. Bacteremia was seen in 3 of 8 patients and one of these died. Careful examination of the anus, rectum, and perineum should be mandatory in ESRD patients with undiagnosed fever. Treatment consisted of extensive surgical debridement and drainage along with antimicrobial therapy.

The aim of this research study is to identify risk factors typical of different types of suspected child abuse reported at a hospital. The study was based on 114 cases of children for whom some type of abuse was reported. Physical abuse was the most frequently reported of all types of suspected child abuse. Most victims of sexual abuse were female and at least half the cases of neglect and physical abuse were attributed to parents. Most cases were identified in the emergency room by nurses. Children older than 10 were more susceptible to physical abuse and neglect.

Adolescent abuse was studied in 555 school students (mean age 15.6 +/- 1.5 years) selected by multistage random sampling in Beni Suef in 1998. Each student received a general physical examination and a pre-coded questionnaire to identify determinants of abuse. Prevalence of abuse was 36.6%. Emotional, physical, sexual and combined abuse prevalence was 12.3%, 7.6%, 7.0% and 9.7% respectively.
