Would wider screening for primary aldosteronism give any health benefits?

Fifty years ago, Jerome Conn described 'a new clinical syndrome which is designated temporarily as primary aldosteronism' in a young patient with hypertension, severe hypokalaemia and a benign adrenocortical tumour, for whom an adrenalectomy cured both the hypertension and hypokalaemia. His report identified the condition known as aldosterone-producing adenoma, a form of curable hypertension. According to Conn, the prevalence of primary aldosteronism in the hypertensive population referred to his department was 20%, but this estimate was subject to referral bias. Primary aldosteronism has long

What's new in TB diagnosis? A spotlight on active case-finding.

Tuberculosis (TB) remains the deadliest infectious disease globally, with gaps in case detection hindering its control. Active case-finding (ACF) comprises active screening of at-risk populations, including asymptomatic individuals, who constitute over half of TB prevalence in high-burden areas and who may contribute to community-based transmission. Common ACF models in community settings include door- to-door screening or mobile, vehicle-based screening using chest X-ray and/or molecular testing modalities. Other forms include healthcare facility-based screening and mass screening in congrega…

A narrative review of the development and performance characteristics of electronic delirium-screening tools.

BACKGROUND: Electronic delirium-screening tools are an emergent area of research. OBJECTIVE: The objective of this study was to summarise the development and performance characteristics of electronic screening tools in delirium. METHODS: Searches were conducted in Pubmed, Embase, and CINAHL Complete databases to identify electronic delirium-screening tools. RESULTS: Five electronic delirium-screening tools were identified and reviewed. Two were designed for and tested within a medical setting, and three were applied to intensive care. Adaptive design features, such as skip function to reduc

Screening of HbE by the New Screening Test: Alcohol HbE Test (AET).

BACKGROUND: Hemoglobin E (HbE) is common in Thailand and the Indochina subcontinent. Implementation of a simple assay to screen for HbE is crucial for use in reproductive counseling, as HbE/β-thalassemia is a condition with high morbidity and mortality. METHODS: This report developed the alcohol-based HbE screening test named the Alcohol HbE Test (AET). This test was composed of 19% isopropanol in 0.005% Triton-X-100 aliquoted as a 500-µL volume in a 1.5 mL transparent microcentrifuge tube. In performing the AET, 50 µL of EDTA blood was added into the tube, incubated at 37 °C for 30 min befor

Detection of clustered anomalies in single-voxel morphometry as a rapid automated method for identifying intracranial aneurysms.

Unruptured intracranial aneurysms (UIAs) are prevalent neurovascular anomalies which, in rare circumstances, rupture to cause a catastrophic subarachnoid haemorrhage. Although surgical management can reduce rupture risk, the majority of UIAs exist undiscovered until rupture. Current clinical practice in the detection of UIAs relies heavily on manual radiological review of standard imaging modalities. Recent computer-aided UIA diagnoses can sensitively detect and measure UIAs within cranial angiograms but remain limited to low specificities whose output also requires considerable radiologist in

[The ethical aspects of population screening programme of rare diseases].

The Committee on Ethics of the Instituto de Investigación de Enfermedades Raras (CEIIER) of the Spanish National Institute of Health Carlos III, presents this article dealing with ethical guidelines regarding the implementation of screening population programmes with special emphasis on genetic screening. After a critical review it has been addressed 24 recommendations concerning 14 topics: evaluation of the opportunity of the programme, including ethical analysis besides scientific evidences and cost/benefits issues; the need to differentiate between research and public health intervention an

Views of patients and parents of children with genetic disorders on population-based expanded carrier screening.

OBJECTIVE: Faster and cheaper next generation sequencing technologies have enabled expansion of carrier screening for recessive disorders, potentially facilitating population-based implementation regardless of ancestry or family history. Little is known, however, about the attitudes regarding population-based carrier screening among families with genetic disorders. This study assessed views among parents and patients with a recessive disorder and parents of children with Down syndrome (DS) on expanded carrier screening (ECS). METHOD: In total, 85 patients with various recessive disorders, 110

[Epidemiology of congenital hypothyroidism in France: recent data].

Congenital hypothyroidism (CH) is the leading cause of preventable mental retardation. It is mainly due to thyroid dysgenesis or dyshormonogenesis with normally located gland, and detected at birth in developed countries, by mass biochemical screening of newborns. An increase in the incidence of congenital hypothyroidism with a normally located gland has been reported worldwide over the last three decades. The etiology of CH with a normally located gland remains elusive and the factors driving its clinical diversity are largely unknown. A role for known dyshormonogenesis-associated genetic var

[A campaign for detection of beta-thalassemia minor and prevention of beta-thalassemia major in the island of Minorca. 2 years' experience].

The high prevalence of beta-thalassemia minor and the occurrence of thalassemia major in Minorca prompted us to develop a prevention campaign. The selected population consisted of primary school students. The campaign was operative during the terms 1986-87 and 1987-88 and had three stages: 1.) informative; 2.) analytical study and diagnosis of carriers; 3.) report of the results and genetic counselling. We studied overall 1862 schoolchildren. The prevalence of beta-thalassemia minor was 3.38%. We discuss the methodology, the results and the effectiveness of the campaign.

Potassium ion ionization of desorbed species (K+IDS): a rapid method for the screening of urine for organic acidemias.

A new desorption/ionization mass spectrometric technique, K+ ionization of desorbed species (K+IDS), is evaluated as a rapid method for differentiating various organic acidemias, conditions in which excessive levels of organic acid metabolites are present in plasma or urine as a result of some inborn error of metabolism. This method requires no derivatization of the isolated organic acids, unlike that required for gas chromatographic and gas chromatographic/mass spectrometric analyses. 'Batch' mass spectrometric analysis is achieved by deposition of the complex organic acid mixture (from urine
