Preoperative prediction for mastoid extension of middle ear cholesteatoma using temporal subtraction serial HRCT studies.

OBJECTIVES: This study investigated the utility of temporal subtraction computed tomography (TSCT) obtained with temporal bone high-resolution computed tomography (HRCT) for the preoperative prediction of mastoid extension of middle ear cholesteatomas. METHODS: Twenty-eight consecutive patients with surgically proven middle ear cholesteatomas were retrospectively evaluated. The presence of black color in the mastoid region on TSCT suggested progressive changes caused by bone erosion. Enlarged width of the anterior part of mastoid on HRCT was interpreted as suggestive of mastoid extension. Fis…

Surgical ligation of A Large Mastoid Emissary Vein in A Patient Complaining of Pulsatile Tinnitus.

Pulsatile tinnitus is an uncommon symptom characterized by a perceived sound pulsing like a heartbeat. Here, we report an unusual case of a patient with unilateral pulsatile tinnitus caused by a large, prominent mastoid emissary vein (MEV). A 45-year-old woman presented at our hospital with pulsatile tinnitus. She had persistent tinnitus for 20 years, and her symptoms had worsened in the previous 2 years. She said that she perceived a sound pulsing like a heartbeat. She had some hearing impairment in both the ears for a long time owing to long-term otitis media. The temporal bone computed tomo…

Morphometric analysis of the inter-mastoid triangle for sex determination: Application of statistical shape analysis.

PURPOSE: Sex determination can be done by morphological analysis of different parts of the body. The mastoid region, with its anatomical location at the skull base, is ideal for sex identification. Statistical shape analysis provides a simultaneous comparison of geometric information on different shapes in terms of size and shape features. This study aimed to investigate the geometric morphometry of the inter-mastoid triangle as a tool for sex determination in the Iranian population. MATERIALS AND METHODS: The coordinates of 5 landmarks on the mastoid process on the 80 cone-beam computed tomo…

Selective embolization of the mastoid emissary vein for pulsatile tinnitus treatment: when is it indicated?

BACKGROUND: Venous pulsatile tinnitus (PT) can be caused by the perception of blood flow through the mastoid emissary vein (MEV). However, the causal relationship of this anomaly has not been established as selective occlusion of this vein has never been performed. The aim of this study was to report on a small series of patients suspected of having PT caused by an MEV, who underwent MEV embolization as treatment for PT, and to identify the correct indications for this procedure. METHODS: This was a single center retrospective study (CNIL 2215065) including six patients with isolated disablin…

Surgical results of retrograde mastoidectomy with primary reconstruction of the ear canal and mastoid cavity.

The aim of this study was to retrospectively review the long-term hearing results and the impact of mastoid exclusion/obliteration in patients with cholesteatoma (102 ears) who underwent retrograde tympanomastoidectomy and in whom bone chips/paté were applied as the sole materials during the procedure. In 79 ears, this was combined with ossiculoplasty in a single-stage procedure. In >71% of ears, the results of audiometric testing were monitored for more than 2 years. The results suggested there was a significant gain in hearing following surgery, with respect to the postoperative change in bo…

Reconstruction of low hairline microtia of Treacher Collins syndrome with a hinged mastoid fascial flap.

Treacher Collins syndrome (TCS) is a rare genetic disorder leading to congenital craniofacial malformations. Although this syndrome presents with various symptoms, corrective surgery for bilateral microtia with low hairline is one of the most challenging operations given the complex contours of the external ear. In this technical note, a novel, simple procedure for dealing with the low hairline by using a hinged mastoid fascial flap simultaneously with costal cartilage grafting is described. Several techniques for the reconstruction of low hairline microtia have been reported previously, such …

[Tension pneumocephalus in association with ventriculoperitoneal shunt and congenital bony defect in the mastoid tegmen].

The authors report a case of tension pneumocephalus in association with a congenital bony defect at the mastoid tegmen, and a ventriculoperitoneal (V-P) shunt for obstructive hydrocephalus, due to the presence of a posterior fossa meningioma. After multiple diagnosis and surgical procedures, congenital bony defect at the right mastoid tegmen demonstrated by a middle ear cavity computerized tomography (CT) scan, was identified as the source of entry of the air. The air must have penetrated the lateral ventricle through a porencephalic cyst in the right temporal lobe. Reconstruction of the bony …

Results of endolymphatic sac to mastoid shunt surgery for Menière's disease refractory to medical therapy.

In the ten-year period ending July 1, 1986, 5475 dizzy patients were evaluated and treated medically at the Colorado Ear Clinic. Of these, 214 patients (4%) were diagnosed as having classic Meniere's disease refractory to medical therapy and underwent endolymphatic sac to mastoid shunt surgery. These patients were followed for a minimum of one year after surgery. The average audiometric follow-up was 35.1 months. Based on the American Academy of Otolaryngology-Head and Neck Surgery 1985 criteria for reporting results at one year, 60.3% had lower audiometric thresholds postoperatively (H1) or u…

Obliteration of the middle ear and mastoid cleft in subtotal petrosectomy: indications, technique, and results.

Lateral surgical approaches to the base of the skull through the temporal bone often result in a large cavity with exposed dura and vascular structures and no possibility of reconstruction of the middle ear conductive hearing mechanism. Subtotal petrosectomy with tympanomastoid obliteration provides a relatively safe and secure closure of the surgical defect in the temporal bone and eliminates the problems associated with an open mastoid cavity. Eradication of all accessible air cell tracts and mucosa in the petrous pyramid, obliteration of the eustachian tubal orifice, closure of the external…

Contemporary management of acute mastoiditis in children: Insights from a U.S. national database.

OBJECTIVE: Acute mastoiditis (AM) is a serious complication of acute otitis media. While historically managed with mastoidectomy, recent trends support conservative treatment, including intravenous (IV) antibiotics and pressure equalization tube (PET) placement. International studies demonstrate high success with these approaches, but U.S. pediatric data remains limited. METHODS: Pediatric patients diagnosed with AM identified via Current Procedural Terminology (CPT®) coding and International Classification of Diseases (ICD-10) between January 2010 and October 2021 were reviewed using PearlDi…

Acute mastoiditis-associated cerebral sinus venous thrombosis in children: a retrospective review.

UNLABELLED: Cerebral sinus venous thrombosis (CSVT) is a rare but serious complication of acute mastoiditis in children. Early diagnosis is often delayed due to nonspecific symptoms. This study aimed to identify clinical and laboratory predictors of CSVT in children with mastoiditis to support timely imaging and treatment. We performed a retrospective case-control study across two university-affiliated centers from 1999 to 2022 of children with acute mastoiditis who underwent neuroimaging. Clinical, laboratory, imaging, and treatment data from 81 children (40 with CSVT and 41 without) were com…
