Otology & neurotology : official publication of the American Otological Society, American Neurotology Society [and] European Academy of Otology and Neurotology2009eng
OBJECTIVE: To present a new surgical approach to treat idiopathic neuralgia of the sensory auricular branch of the facial nerve.
PATIENTS: : Three patients with chronic ear pain resistant to medical therapy.
INTERVENTION: Sectioning of the sensory auricular branch of the facial nerve along its course at the posterior wall of the external auditory canal by a retroauricular mastoidectomy approach.
MAIN OUTCOME MEASURES: Resolution of otalgia.
RESULTS: All the patients were relieved of their pain at their first follow-up visit, and they have remained symptom-free. The patients were followed a…

A complication of otitis media is defined as a spread of infection beyond the pneumatized area of the temporal bone and the associated mucos … pproach. Surgery of the abscess includes aspiration through a bur hole or craniotomy and by radical mastoidectomy for CSOM.

A 62-year-old female was admitted to our clinic in October 1999 with a 6-year history of hearing loss, tinnitus and otorrhea in the right ea … up examination, 72 months after the operation, no sign of disease was present. The modified radical mastoidectomy approach used in this case proved to be a safe and efficacious method for removing the glomangioma.

This article describes the current value of imaging in patients after stapes surgery and surgery after chronic otitis media including choles … inner ear complications. CT has a high negative predictive value in cases with a free cavity after mastoidectomy. Localized opacities or total occlusion are difficult to distinguish by CT alone. MRI provides important additional information in the differentiation of cholesterol granuloma, cholesteatoma, effusion, granulation and scar tissue.

Various surgical approaches to the petrous apex for exposure and drainage of suppurative processes are available to the otologist. The petrous apex may be conveniently divided into anterior and posterior portions by a line in the coronal plane through the internal auditory canal. The approach to the posterior petrous apex follows fistulous tracts in the sinodural angle, the subarcuate fossa, and the infralabyrinthine tract. The anterior petrous apex may be entered by means of a radical mastoidectomy. Fistulous tracts into an infected anterior petrous apex may be found through the hypotympanum,…

The "blue ear drum" generally refers to a condition in which blood or blood products are found in the middle ear. After all possible causes … ence of glands and cysts and cholesterin granuloma. The recommended procedure is a modified radical mastoidectomy, placement of silicone rubber sheeting in the middle ear and insertion of a ventilation tube. It is to be emphasized that mastoid surgery is rarely indicated for these patients and only after all else has failed.

Objective: Robotic cochlear implantation is an emerging surgical technique for patients with sensorineural hearing loss. Access to the middle and inner ear is provided through a small-diameter hole created by a robotic drilling process without a mastoidectomy. Using the same image-guided robotic system, we propose an electrode lead management technique using robotic milling that replaces the standard process of stowing excess electrode lead in the mastoidectomy cavity. Before accessing the middle ear, an electrode channel is milled robotically based on intraoperative planning. The goal is to f…

Treatment in all cases comprised of broad-spectrum antibiotics, mastoidectomy, and long-range hypocoagulation with warfarin. Transtympanic ventilation tubes were inserted in all cases but one with COMC. Perioperative sigmoid sinus exposure was performed in seven patients, with drainage of perisinus empyema in three cases. BACKGROUND: Mature teratoma composed of all three basic germ cell layers of the head and neck is a rare disease. Teratomas involving the tem … nnected by a thin membranaceous tissue. The mass was removed completely under general anesthesia by mastoidectomy. As of last follow-up (2 years post-surgery), the disease had not relapsed.
CONCLUSION: Pre-operative CT and MRI are necessary for eustachian tube teratoma. Complete surgical resection provided excellent prognosis.

16S rDNA polymerase chain reaction (PCR) in the diagnosis of fastidious organisms is becoming increasingly commonplace. We present the case of a child admitted to an acute paediatric unit of a university teaching hospital with otorrhoea, torticollis, and cervical lymphadenopathy. Examination revealed hepatosplenomegaly associated with pancytopenia. Radiological imaging confirmed a retropharyngeal abscess, bilateral mastoiditis, cerebellar lesions, and venous sinus thrombosis. Swabs of aural discharge grew anaerobes. Drainage of the retropharyngeal abscess and bilateral mastoidectomy were perfo…

Twenty-two percent had uncontrolled infection, a cortical mastoidectomy together with myringoplasty produced a 75% take rate. Graft take was not affected by age, type of surgery, infection status or perforation size. Poor nutritional status and postoperative infection both had negative tendencies. In this small series t …

A 28-year-old woman was admitted to the ENT Department of the Faculty of Medicine, Chiang Mai University because of left otitis externa, chronic mastoiditis and facial nerve palsy (lower motor neurone). Left radical mastoidectomy with wide debridement of granulation and necrotic tissue at ear canal, dura, sigmoid sinus, ossicle and facial nerve were done two weeks after admission. Multiple pus culture from the lesion revealed different types of organisms e.g. proteus mirabilis, beta strep non group A, and enterococci beta type. Several aminoglycosides were alternately given for two months with…

Zygomatic root abscess is a rare extracranial extratemporal complication of otitis media. To the best of our knowledge, there are only a few scattered cases of zygomatic root abscesses reported in the literature. We present an unusual case of a zygomatic root abscess in a 24 years old adult. He presented with one month duration of right zygomatic swelling. Otoscopic examination revealed superior and posterior external auditory canal wall sagging with an intact tympanic membrane. High Resolution Computed Tomography (HRCT) temporal bone revealed a rim enhancing lesion lateral to the zygomatic pr

This case series details two paediatric cases of Mycobacterium avium complex (MAC) mastoiditis that presented to the same hospital within three years of each other and were successfully managed. One female and one male patient, both pre-school aged, presented with otorrhoea and postauricular oedema unresponsive to intravenous antibiotics. Imaging demonstrated opacification of the mastoid air cells and middle ear with cortical breaks, and surgical exploration revealed granulation tissue in the mastoid cavity and antrum on gross and histopathological examination, as well as bony dehiscence. BACKGROUND: Cochlear implantation (CI) in children with malformed ears can be challenging through the standard surgical technique. Several alternative approaches have been described. The endoscopic-assisted approach can be chosen as an effective and safe surgical technique, overcoming the drawbacks of the traditional approach.
MATERIAL: We further describe a combined technique based on a limited mastoidectomy with no posterior tympanotomy and an endoscopic transmeatal approach to the round window (RW): the electrode is driven from the mastoid to the middle ear through the attic.
RESULTS: The…
