Maternal and antenatal risk factors for stillbirths and neonatal mortality in rural Bangladesh: a case-control study.

OBJECTIVE: To identify maternal and antenatal factors associated with stillbirths and neonatal deaths in rural Bangladesh. STUDY DESIGN: A prospective cohort study is being conducted to evaluate a maternal and child nutrition program in rural Bangladesh. Cases were all stillbirths and neonatal deaths that occurred in the cohort between March 7, 2011 and December 30, 2011. Verbal autopsies were used to determine cause of death. For each case, four controls were randomly selected from cohort members alive at age 3-months. Multivariable logistic regression was used to identify factors associated

The use of audit to identify maternal mortality in different settings: is it just a difference between the rich and the poor?

OBJECTIVE: To illustrate how maternal mortality audit identifies different causes of and contributing factors to maternal deaths in different settings in low- and high-income countries and how this can lead to local solutions in reducing maternal deaths. DESIGN: Descriptive study of maternal mortality from different settings and review of data on the history of reducing maternal mortality in what are now high-income countries. SETTINGS: Kalabo district in Zambia, Farafenni division in The Gambia, Onandjokwe district in Namibia, and the Netherlands. POPULATION: Population of rural areas in Z…

Inequalities in Severe Maternal Morbidity and Mortality in High-Income Countries: Patterns, Drivers, and Pathways to Action.

Inequalities in severe maternal morbidity (SMM) and mortality in high-income countries are persistent, socially patterned, and evident across multiple dimensions, including socioeconomic deprivation, race and ethnicity, and migration status. These inequalities are not fully explained by individual clinical risk factors but arise from the interaction of structural disadvantage, intermediate social conditions, and health systems. Many determinants of risk are established before pregnancy; however, variation in access to care, quality of care, and responsiveness to symptoms during pregnancy and c

Intervention Research to Reduce Disparities in Severe Maternal Morbidity and Mortality in the United States: A Scoping Review.

Persistent inequities in severe maternal morbidity (SMM) and maternal mortality in the United States have received national attention over the past decade. This scoping review aimed to outline evidence-based interventions to decrease these inequities. We adapted a socioecologic framework of SMM and maternal mortality to integrate three equity approaches: 1) interventions across subpopulations, 2) analytic strategies, such as subanalyses or reporting outcomes among priority groups, and 3) assessing risk factors unique to priority groups. We conducted a structured search of peer-reviewed medical…

The effect of tranexamic acid during childbirth on maternal mortality: an epidemiological modelling and cost-effectiveness study.

BACKGROUND: Tranexamic acid reduces life-threatening bleeding after childbirth. We aimed to identify the settings where tranexamic acid cost-effectively reduces maternal deaths from bleeding. METHODS: We used WHO mortality data and effect estimates from our individual patient data meta-analysis to develop an epidemiological and cost-effectiveness model to calculate the absolute benefit (postpartum haemorrhage deaths averted) and potential harm (deaths from inadvertent intrathecal tranexamic acid injection) in each country, if all women received 1 g intravenous tranexamic acid when giving birt

Site-randomised trial of a multilevel digital intervention to reduce maternal morbidity and mortality: a study protocol.

INTRODUCTION: Compared with other high-income countries, the USA continues to have the highest rates of pregnancy-related and associated mortality and morbidity (PRAMM), particularly in rural areas and among non-Hispanic black pregnancies. Over 80% of pregnancy-related deaths are preventable; however, the intensity of existing interventions has proven difficult to broadly disseminate. Technology offers the potential to address such barriers. This study will develop a multilevel digital intervention to reduce PRAMM and evaluate its effects using a site-randomised trial. METHODS AND ANALYSIS: T

The Implications of Socioeconomic Status by ZIP Code on Maternal-Fetal Morbidity and Mortality in San Antonio, Texas.

Introduction Over the past 20 years, the number of pregnancy-related fatalities in the United States has been on the rise. Increases in maternal and fetal mortality have been attributed to low socioeconomic status (SES). This raises the question of whether all geographical locations are proportionally affected by this upward trend in pregnancy-related fatalities. San Antonio is one of the largest cities in the United States and is known for its economic segregation. This study aims to compare the maternal and fetal health outcomes of mothers from diverse socioeconomic backgrounds in San Antoni

Evaluation of surveillance system and response for maternal and perinatal mortalities in Meda Welabu District, Ethiopia, 2023.

The weekly disease surveillance system (WDSS) serves as a precursor to possible public health emergencies. The Meda Welabu Woreda Bale Zone in Ethiopia has reporting rates of 87% overall timeliness and 88% completeness in 2023, falling short of the 100% objective. Low reporting rates could mean that epidemics in the province are only discovered later. In the Meda Welabu Woreda Bale Zone of Ethiopia, the study was carried out to assess the WDSS maternal and perinatal death surveillance response (MPDSR). Using the most recent Centers for Disease Control (CDC) criteria for assessing public health

Impact of a pilot workshop on student pharmacists' confidence and comfort in counseling patients at risk for maternal mortality.

BACKGROUND AND PURPOSE: This study examined whether a pilot workshop focused on maternal mortality had an impact on pharmacy students' confidence, comfort, and knowledge regarding preeclampsia, postpartum depression and opioid poisoning. EDUCATIONAL ACTIVITY AND SETTING: The two-hour workshop included lecture, video, discussion and case studies. P1-P3 student pharmacists completed pre- and post-surveys measuring confidence (N = 5) and comfort (N = 15) using a 5-point Likert type scale (1 = strongly disagree to 5 = strongly agree), as well as knowledge with 12 true/false statements. Data were …

Will Brazil comply with the SDG 3.1 of the 2030 Agenda? An analysis of maternal mortality, from 1996 to 2018.

This article aims to analyze if it is possible for Brazil to meet the Sustainable Development Goals (SDG) 3.1, based on a diagnosis of the situation of maternal mortality in the Health Regions (HRs) of Brazil, in 2018, and the main characteristics of this mortality between 1996 and 2018 in the country. The study consists of two articulated phases: (i) bibliographical analysis of maternal mortality in Brazil; (ii) study in the Mortality Information System (SIM, in Portuguese). In 2018, from the 450 HRs, 159 showed a maternal mortality rate (MMR) of above 70 per 100,000 live births (LBs). Betwee…

Protocol for analysing the epidemiology of maternal mortality in Zimbabwe: A civil registration and vital statistics trend study.

BACKGROUND: Sub-Saharan Africa (SSA) carries the highest burden of maternal mortality, yet, the accurate maternal mortality ratios (MMR) are uncertain in most SSA countries. Measuring maternal mortality is challenging in this region, where civil registration and vital statistics (CRVS) systems are weak or non-existent. We describe a protocol designed to explore the use of CRVS to monitor maternal mortality in Zimbabwe-an SSA country. METHODS: In this study, we will collect deliveries and maternal death data from CRVS (government death registration records) and health facilities for 2007-2008 …
