Tilted implantation technique for edentulous patients.

The tilted implantation technique is characterized by placing the implant at an angle of more than 15° and less than 45° from the horizontal plane. This technique can avoid damaging the maxillary sinus, inferior alveolar nerve, nasal base, and other anatomical structures when the height of the upper and lower jaw available bone is insufficient, to maximize the use of available bone and avoid a large range of bone increment. The tilted implantation technique can reduce the trauma of the surgery, increase the possibility of immediate restoration and shorten the treatment cycle, which has been wi

[A 40 year old palatal neurofibroma. A case report].

Neurofibroma is a benign peripheral nerve sheath tumour, which arises from Schwann cells and perineural fibroblasts. It is one of the most frequent tumours of neural origin and its presence is one of the clinical criteria for the diagnosis of type 1 neurofibromatosis (NF-I) also known as von Recklinghausen's disease (VRD). Oral localization is rarer showing an incidence ranging from 4 to 7% in most series of different authors. The most frequent involvement site in oral neurofibromatosis is the tongue, followed by the oral mucosa and floor of the mouth; palate and maxillary-mandibular bones are

Primary T-cell lymphoblastic lymphoma of the cavernous sinus.

The rare occurrence of T-cell lymphoblastic lymphoma as a primary tumor in the cavernous sinus is described. The patient, a 17-year-old girl, presented with right-sided ophthalmic and maxillary neuropathy and diplopia due to neuropathies of cranial nerves III and VI. An enhancing mass in the cavernous sinus was identified on MR imaging. Dexamethasone was prescribed but did not provide symptomatic relief. Rapid progression of symptoms led to open biopsy, and a diagnosis of T-cell lymphoblastic lymphoma was made. The patient promptly underwent aggressive chemotherapy in which a modified hyper-cy

The anatomy of a horizontally impacted maxillary wisdom tooth.

A completely horizontally impacted upper third molar was revealed after routine dissection of a 62-year-old human cadaver of a Caucasian male. The molar was penetrating into the maxillary sinus and there was antral dehiscence of its bony alveolus. The bony alveolus was immediately in front of the greater palatine canal contents, and the bottom of the alveolus was dehiscent towards the greater palatine foramen. Within the greater palatine canal and foramen the greater palatine artery was duplicated and the nerve was found. Such antral relations of an impacted upper third molar predispose to oro

Anatomic and magnetic resonance imaging bases for the naso-maxillo-cheek flap technique.

A transfacial approach to the deep cranio-maxillo-facial areas by the naso-maxillo-cheek flap technique (NMCF) is indicated for the treatment of some bulky tumors of the naso-pharynx. The procedure requires precise preoperative imaging. This study presents the morphologic bases of this surgical access and the reasonable limits of the excision preoperatively determined by magnetic resonance imaging (MRI). 18 facial and skull specimens were submitted to surgical facial dismantling by the NMCF technique according to Curioni's method. The clinical application in a 66-year-old patient suffering fro

Alveolar bone necrosis and tooth exfoliation following herpes zoster infection: a review of the literature and case report.

BACKGROUND: Herpes zoster (HZ) presents as a cutaneous vesicular eruption in the area innervated by the affected sensory nerve, usually associated with severe pain. Oral manifestations of HZ appear when the mandibular or maxillary divisions of the trigeminal nerve are affected. METHODS: This is a case report of a 63-year-old woman with HZ infection with trigeminal nerve involvement that led to a rapid loss of alveolar bone and exfoliation of two teeth. RESULTS: The initial intraoral examination showed redness of the alveolar mucosa and gingiva of the lower right quadrant with multiple well-d

[The formation of a pathological system in rats with neuropathic trigeminal neuralgia].

The intensity of clinical manifestations (rubbing of the oro-facial area with skin injury), bioelectric activity of the somatosensory cortex and microcirculatory disturbances were studied for 6 weeks in Wistar rats with partial infraorbital nerve compression. These parameters were compared among rats of the following groups: without skin rubbing (1), with them (2), with repaired skin (3) after nerve compression and sham-operated rats (4). A correlation between the intensity of clinical manifestations and microcirculatory disturbances was shown. Epileptiform cortex activity was observed in 100%

Pain. Part 2a: Trigeminal Anatomy Related to Pain.

In order to understand the underlying principles of orofacial pain it is important to understand the corresponding anatomy and mechanisms. Paper 1 of this series explains the central nervous and peripheral nervous systems relating to pain. The trigeminal nerve is the 'great protector' of the most important region of our body. It is the largest sensory nerve of the body and over half of the sensory cortex is responsive to any stimulation within this system. This nerve is the main sensory system of the branchial arches and underpins the protection of the brain, sight, smell, airway, hearing and

Trigeminal Neuralgia: Evaluation of the Relationship Between the Region of Neuralgic Manifestation and the Site of Neurovascular Compression Under Endoscopy.

This study aimed to evaluate the relationship among the pain region, branches of trigeminal nerve, and the neurovascular compression (NVC) location. A total of 123 consecutive patients with trigeminal neuralgia (TN) underwent endoscope-assisted microvascular decompression according to positive preoperative tomographic angiography. V2 alone was in 51 cases and V3 alone was in 64 cases. The location of NVC was classified into cranial, caudal, medial, or lateral sites. Some patients with multiple regions were recorded as medial + cranial, lateral + cranial, medial + caudal, and lateral + caudal.

Role of Radiotherapy in an Inflammatory Pseudotumor of the Maxillary Sinus and Imaging Findings: A Case Report.

Inflammatory pseudotumor (IPT) is a benign lesion of inflammatory origin that can mimic a malignant neoplasm because of its aggressive clinical and imaging behavior, particularly in uncommon locations such as the paranasal sinuses. In this article, we present the case of a patient diagnosed with IPT of the left maxillary sinus, which demonstrated extension into adjacent soft tissues and secondary involvement of cranial nerves due to extensive local invasion. Findings on computed tomography (CT) and magnetic resonance imaging (MRI) revealed bone destruction and extension into neighboring anatom

Local and systemic toxicity of intraoral submucosal injections of phentolamine mesylate (OraVerse).

OraVerse, an injectable formulation of phentolamine mesylate (PM), was recently approved by the U.S. Food and Drug Administration (FDA) for reversal of anesthesia of the lip and tongue and associated functional deficits resulting from an intraoral submucosal injection of a local anesthetic containing a vasoconstrictor. Because PM had not been approved previously for submucosal administration, 2 Good Laboratory Practices (GLP) studies in dogs designed to investigate systemic toxicity and the local effects of single and repeated dosing of OraVerse on the inferior alveolar nerve and branches of t

Duplication of Right Superior Cerebellar Artery Leading to Trigeminal Neuralgia: A Rare Neurovascular Conflict.

Trigeminal neuralgia (TN) is a chronic neuropathic pain condition characterized by brief, intense, and electrical shock-like facial pain. The most common causes of this are due to neurovascular compression of the superior cerebellar artery (SCA). However, as seen in this case report, duplication of the SCA resulting in TN is extremely uncommon. A 63-year-old woman presented with a 1-day history of paroxysmal, shock-like pain on the right side of her face affecting both the ophthalmic (V1) and maxillary (V2) branches of the trigeminal nerve. Each episode of pain would last 1-2 min, occur multip

Application of Antidromic Conduction Monitoring in Ganglion Radiofrequency Thermocoagulation for Locating Trigeminal Branches in Trigeminal Neuralgia.

OBJECTIVE: The aim of this study was to investigate whether antidromic conduction monitoring (ACM) can be utilized to map the trigeminal system under sedation as a potential substitute for subjective paresthesia description (SPD) during percutaneous ganglion radiofrequency thermocoagulation (PGRT). METHODS: Eighty-two patients with 152 pain divisions of trigeminal neuralgia (TN) were treated by computed tomography (CT)-guided PGRT. After the puncture needle entered the foramen ovale (FO), sensory and motor stimulation were applied to locate the pain division. And the corresponding voltage val

Longitudinal tear of the inferior rectus muscle in orbital floor fracture.

We report a case of longitudinal avulsion of the inferior rectus muscle following orbital floor fracture and describe its clinical presentation, computed tomography (CT) features and management. A 53-year-old man felt vertical diplopia in all gaze immediately after the trauma. Orthoptic assessment showed left over right hypertropia of 20 prism diopters and left exotropia of 10 prism diopters in primary position. The left orbital floor fracture and the prolapse of orbital contents into the maxillary sinus were presented by CT. Exploration of the orbit was performed under general anesthesia. The

Peripheral Nerve Stimulation Reduces Opioid Use and Facilitates Early Ambulation Following Fibular Flap Harvest: A Retrospective Study.

BACKGROUND: Fibular free flap (FFF) reconstruction is the gold standard for mandibular and maxillary defects, but donor-site pain remains challenging. Multimodal analgesia (MMA) relies on opioids, which risk dependence, systemic side effects, and delayed recovery. Regional nerve blocks reduce pain but have inconsistent efficacy owing to variable drug metabolism, potential effects on flap perfusion, and motor blockade. Temporary percutaneous peripheral nerve stimulation (TPNS) is a promising alternative that selectively modulates sensory pathways while preserving motor function. METHODS: This

How to understand an enlarged Meckel's cave? An anatomical study.

PURPOSE: Dilatation of the trigeminal cavum, or Meckel's cave (MC), is usually considered a radiological sign of idiopathic intracranial hypertension. However, the normal size of the trigeminal cavum is poorly characterized. In this study, we describe the anatomy of this meningeal structure. METHODS: We dissected 18 MCs and measured the length and width of the arachnoid web and its extension along the trigeminal nerve. RESULTS: Arachnoid cysts were clearly attached to the ophthalmic (V1) and maxillary (V2) branches until they entered the cavernous sinus and foramen rotundum, respectively, wi
