Stability and morbidity of Le Fort I osteotomy with bioresorbable fixation: a randomized controlled trial.

A randomized controlled clinical trial was conducted to compare the use of bioresorbable and titanium mini-plates and screws in Le Fort I maxillary osteotomies for evaluation of clinical morbidity and stability. Forty patients requiring Le Fort I osteotomies were randomly assigned to two groups. One group underwent bioresorbable mini-plate fixation and the other titanium mini-plate fixation. Stability of the maxilla was determined by serial cephalometric analysis at 2 and 6 weeks and at 3, 6 and 12 months postoperatively. Subjective and objective assessment of clinical morbidity was made prosp

A novel osteosynthesis plate design for routine corrective facial surgery.

Plating systems for the osteosynthesis of facial osteotomies have different requirements to those used to treat facial fractures. The aim of fracture treatment is anatomical fragment reduction, functional/rigid immobilization, restoration of occlusion and aesthetics, and occasionally defect bridging. In corrective facial surgery however there is a need for intra-operative adjustments related to changes in occlusion and aesthetics, and defect bridging occurs frequently. Postoperatively, training elastics are used to control temporary neuromuscular imbalance. To accommodate these demands a new p

Minimal invasiveness in the transcrestal elevation of the maxillary sinus floor: A systematic review.

In the attempt to reduce the invasiveness of a transcrestal sinus floor elevation procedure, different aspects must be considered; that is, the minimization of intra- and postsurgery morbidity, the reduction of treatment time, and the simplification/elimination of the reconstructive technology. Within this context, a systematic literature search was performed for controlled clinical trials evaluating the impact of one or more of these aspects on transcrestal sinus floor elevation invasiveness. Nineteen articles (15 studies) were included. Overall, the results confirmed that transcrestal sinus

The influences of mandible-first versus maxilla-first sequencing on postoperative occlusion.

The aim of this study was to evaluate the influences of surgical sequencing on the postoperative occlusion in bimaxillary osteotomies. Patients who underwent bimaxillary surgery without maxillary segmentation, based on a virtual surgical plan were enrolled. The primary outcome variable was occlusal shift, defined as the difference between the surgical accuracy of the mandible and maxilla. The primary predictor variable was the sequence of surgery, with secondary predictors including inferior maxillary repositioning, counterclockwise rotation and gender. 145 patients were enrolled, 68% female,

Hyrax application as a tooth-borne distractor for maxillary advancement.

Distraction osteogenesis (DO) has become a mainstream surgical technique for patients with jaw deformities. The aim of this study was to report the use of internal DO in the treatment of maxillary hypoplasia in a patient with cleft lip and palate. The case illustrates a 17-year-old boy with class III malocclusion, maxillary deficiency, and cleft lip and palate. Because the patient was experiencing severe hypernasality, it was decided to treat him by DO. The treatment process began by mounting a hyrax in the upper jaw for lateral expansion. A second hyrax was mounted in a way to create anteropo

Auto-stop Drilling Device for Implant Site Preparation: In Vitro Test of Eccentric Sensor Position.

PURPOSE: To quantify the reliability of a new drilling system for implant osteotomy characterized by an eccentric sensor that automatically stops the drill upon contact with soft tissue. This safety mechanism aims to minimize surgical trauma to nerves, vessels, and the maxillary sinus mucosa. The benefits of the eccentric sensor position on planar and angulated surfaces were tested in vitro. MATERIALS AND METHODS: Predicted drill protrusion after auto-stop was validated against experiments on four human cadaver mandibles (30 osteotomies with varying angles). Measurement of the drill's exit ho

Tissue Engineering for Vertical Ridge Reconstruction.

This article provides an overview of basic tissue engineering principles as they are applied to vertical ridge defects and reconstructive techniques for these types of deficiencies. Presented are multiple clinical cases ranging from office-based dentoalveolar procedures to the more complex reconstruction of postresection mandibular defects. Several different types of regenerative tissue constructs are presented; either used alone or in combination with traditional reconstructive techniques and procedures, such as maxillary sinus augmentation, Le Fort I osteotomy, and microvascular free tissue

Extraoral subcondylar ramus osteotomy for correction of mandibular prognathism. The surgical technique and complications.

1178 patients with mandibular prognathism and/or asymmetry, in some cases combined with maxillary retrognathism, were treated by extraoral horizontal or oblique ramus osteotomy during the period from 1939 to 1989. The described percutaneous retromandibular approach is a simple, rapid and reliable technique which can usually be performed under local anaesthesia. Very few complications, particularly neurological, were observed. These aspects justify consideration of this technique when indications for a simple, straight set-back procedure of the mandible exist, and there is no tendency to bite-o

Radiography after orthognathic surgery. Part II. Surgical complications.

In a retrospective review of the radiographs from 100 patients who had undergone orthognathic surgery, the authors found that 10% of radiographs had evidence of surgical complications. These complications were fractures, temporomandibular joint dislocations, transected tooth roots, retained surgical devices, avascular necrosis, malunion or nonunion of osteotomy fragments, and osteomyelitis. Some of these complications are of little significance, while others require immediate intervention. Opacified maxillary sinuses and lucencies around tooth roots are frequent findings that may be confused w
