[Pharmacoeconomics aspects of therapy for obliterating diseases of lower-limb peripheral arteries].

The study was aimed at assessing feasibility of treatment of patients suffering from critical ischaemia of lower extremities with iloprost as compared to the basic therapy by means of pharmacoeconomic analysis. The findings of clinical studies and meta-analyses demonstrated that therapy with iloprost results in a pronounced clinical effect as compared with the basic therapy: significantly (p<0.005) decreasing the number of amputations above the knee joint (23% versus 39%) and more frequently decreasing the size of trophic ulcers (in 49% of cases versus 26%). This provides maintenance of the ab

Rehabilitation of patellar tendinopathy.

Patellar tendinopathy is a common musculoskeletal disorder characterized by progressive activity-related anterior knee pain and patellar tendon dysfunction. It is highly prevalent in sports which involve running and jumping. Various treatment methods are used in the management of PT including rest, activity modification, anti-inflammatory medication, injection therapies, taping, eccentric exercises, extra corporeal shock wave therapy, percutaneous electrolysis, and surgery. Even though various treatment options are available for patellar tendinopathy, no single method has proven to result in a

Medication-Related Osteonecrosis of Jaws: A Low-Level Laser Therapy and Antimicrobial Photodynamic Therapy Case Approach.

Medication-related osteonecrosis of the jaws (MRONJ) can be considered an inability of the alveolar bone to respond to an injury, which frequently leads to severe local and systemic complications. Once the problem is installed, dentist must use all therapeutic approaches recommended. This manuscript reports a successful management of MRONJ handled with antibiotics, conservative debridement, low-level laser therapy (LLLT), and photodynamic therapy (PDT) up to 12 months. As healing of MRONJ may be very slow, combined therapeutic approaches are required. Besides the recommended conventional treat

[Treatment of chronic back pain: current standards].

Back pain is a significant medical problem and one of the most common causes of medical consultations and missed work. In acute low back pain, patients with "red flags" indicating a serious underlying spinal or extraspinal disease must be identified by medical evaluation. Most cases of acute back pain are non-specific, and education, physical activity and pain medication is recommended. In addition, yellow flags (risks of developing chronic pain) should be recognized. The management of low back pain has been addressed by the German National Disease Management Guideline (NVL) low back pain publ

Immunocompetent but Not Immune: A Prolonged Course of Cryptococcal Meningitis.

Cryptococcus neoformans is an encapsulated yeast that typically causes cryptococcal meningitis (CM) in immunocompromised hosts. However, CM can rarely occur in immunocompetent individuals, often with atypical or insidious presentations. Persistently positive cerebrospinal fluid (CSF) cultures despite guideline-directed antifungal therapy are uncommon and pose significant management challenges. In this case report, we describe a previously healthy 40-year-old immunocompetent woman who presented with seizures, progressive headaches, and elevated intracranial pressure (ICP). CSF studies confirmed

Corticosteroid therapy in chronic rhinosinusitis with nasal polyps: from histopathological tissue remodeling to patient adherence and barriers.

BACKGROUND AND OBJECTIVES: This study evaluated mucosal remodeling across different corticosteroid regimens and identified barriers to treatment adherence in patients with chronic rhinosinusitis with nasal polyps (CRSwNP). PATIENTS, MATERIALS AND METHODS: We enrolled 56 consecutive patients divided into two arms: Group 1 (n=22) underwent sinus surgery after topical, systemic, or combined corticosteroid therapy, with mucosal biopsies evaluated via a four-parameter histopathological (HP) score (infiltrate, eosinophils, edema, fibrosis). Group 2 (n=34; 15 excluded due to systemic steroid use) re

Evidence-Based Medical Management of Peripheral Artery Disease.

Peripheral artery disease is an atherosclerotic disease of the lower extremities associated with high cardiovascular mortality. Management of this condition may include lifestyle modifications, medical management, endovascular repair, or surgery. The medical approach to peripheral artery disease is multifaceted and includes cholesterol reduction, antiplatelet therapy, anticoagulation, peripheral vasodilators, blood pressure management, exercise therapy, and smoking cessation. Adherence to this regimen can reduce limb-related complications like critical limb ischemia and amputation, as well as

From policy to practice: Evaluating the global implications of the FDA's PDURS framework.

With the rapid development of digital medicine, drug therapy is gradually shifting from a traditional model centered on static information delivery to a dynamic management paradigm driven by software, data, and patient-centered care. In September 2023, the U.S. Food and Drug Administration (FDA) released the Prescription Drug Use-Related Software (PDURS) framework, providing a regulatory foundation for the regulation of software-generated end-user outputs that are directly related to prescription drug use and treated as part of drug labeling. From a digital medicine perspective, this review sy

Impact of race-based spirometry equations on guideline-based asthma management in clinical trials.

BACKGROUND: The American Thoracic Society recently recommended that race- and ethnicity-specific equations for spirometry be replaced with race-neutral equations. This change could have implications for asthma management. OBJECTIVE: Our aim was to examine the effect of race-specific versus race-neutral spirometry reference equations on the resultant guideline-directed asthma therapy in a study population with a high proportion of Black children. METHODS: We pooled and harmonized data on 2076 children and adolescents across 4 Inner-City Asthma Consortium studies that had all used a computer-a

Herpes zoster and radiation therapy: what radiation oncologists need to know about diagnosing, preventing, and treating herpes zoster.

Herpes zoster, a viral disease that is characterized by a painful and blistering eruption in the skin, represents reactivation of latent varicella zoster (chickenpox) virus infection. In high-risk groups such as elderly or immunocompromised patients, the incidence of zoster can be as high as 50%. Radiation oncologists are likely to see zoster because cancer, and cancer therapy, can adversely affect immune function. A few reports suggest that radiation therapy is a risk factor for zoster, and that the skin eruption is often in or near the radiation treatment field. The diagnosis is typically ma

Caring for patients with advanced COPD: beyond the inhalers….

UNLABELLED: COPD affects millions of people worldwide. Patients with advanced COPD have a high symptom burden. Breathlessness, cough and fatigue are frequent daily symptoms. Guidelines often focus on pharmacological treatment, especially inhaler therapy, but other approaches in combination with medications offer symptomatic benefit. In this review, we take a multidisciplinary approach with contributions from pulmonary physicians, cardiothoracic surgeons and a physiotherapist. The following areas are addressed: oxygen therapy and noninvasive ventilation (NIV), dyspnoea management, surgical and
