Increased weight and BMI are associated with increased failure following meniscus repair in the pediatric and adolescent populations.

INTRODUCTION/OBJECTIVES: Increased BMI has been linked to increased risk of meniscal tears, yet outcomes following meniscus repair in these patients remain unclear. This study aims to compare post-traumatic meniscus repair failure rates and return to sport outcomes in pediatric and adolescent populations based on weight, BMI, BMI percentile, and CDC BMI Category. METHODS: This retrospective cohort study included patients aged <19 years who underwent meniscus repair at our multi-center institution between 2017 and 2023, including both isolated and concomitant repairs. The primary outcome was m…

Characterizing the Transition Zone Between the Meniscotibial Ligament and the Medial Meniscus: A Hidden Trigger for Ramp Lesions.

BACKGROUND: The meniscotibial ligament (MTL) attaches to the posterior horn of the medial meniscus, which is essential in knee joint stability. Yet, its histological characteristics remain understudied. Understanding the histological composition of the MTL holds significant implications for comprehending its role in meniscal function and knee stability. PURPOSE: To histologically analyze the MTL from its tibial insertion to its attachment to the meniscus, providing a 3-dimensional perspective. STUDY DESIGN: Descriptive laboratory study. METHODS: Twelve paired glycerin-conserved knee specime…

Stable valley-like concavity at the midbody of the medial meniscus mimicking meniscocapsular separation: a case report.

A 39-year-old woman presented with catching, pain during deep knee flexion, and intermittent locking. Magnetic resonance imaging showed no definite meniscal tear, and diagnostic arthroscopy was performed. Arthroscopy revealed hypermobility of the posterior horn of the lateral meniscus with no tear identified. This hypermobility was considered the source of the patient's mechanical symptoms and was stabilized with three all-inside sutures. In addition, systematic inspection of the medial compartment incidentally identified a smooth, well-demarcated valley-like concavity at the meniscocapsular j…

Arthroscopic partial meniscectomy should be performed carefully for medial meniscus tear with the anteromedial meniscofemoral ligament.

The clinical relevance of the anteromedial meniscofemoral ligament (AMMFL) in medial meniscus (MM) tears remains unclear. This study investigated whether arthroscopic partial meniscectomy (APM) for MM tears with AMMFL is associated with joint degeneration. Among 2568 patients who underwent arthroscopic surgery in 2003 to 2017, 10 with AMMFL and ≥2 years of magnetic resonance imaging follow-up were selected for this retrospective comparative study. Five underwent APM for MM tears (excluding subtotal meniscectomy), while the other 5 did not. Procedures involving the lateral meniscus were exclude…

Medial meniscus extrusion during squatting is correlated with lower limb kinematics in patients with knee osteoarthritis.

BACKGROUND: Increased medial meniscal extrusion (MME) under mechanical stress is a risk factor associated with progression of knee osteoarthritis (OA). Patients with knee OA often have unique features of abnormal kinematics during squatting, which might induce joint loading and increased MME; however, the detailed relationships involved remains unknown. RESEARCH QUESTION: In this study, we investigated the effects of knee kinematics on increased MME during squatting in patients with knee OA. METHODS: Sixteen patients with symptomatic knee OA (mean age 56.31 ± 11.46 years) performed squatting

Age and anatomical region-related differences in vascularization of the porcine meniscus using microcomputed tomography imaging.

Meniscal lesions in vascularized regions are known to regenerate while lack of vascular supply leads to poor healing. Here, we developed and validated a novel methodology for three-dimensional structural analysis of meniscal vascular structures with high-resolution microcomputed tomography (µCT). We collected porcine medial menisci from 10 neonatal (not-developed meniscus, n-) and 10 adults (fully developed meniscus, a-). The menisci were cut into anatomical regions (anterior horn (n-AH and a-AH), central body (n-CB and a-CB), and posterior horn (n-PH and a-PH). Specimens were cut in half, fix…

Effectiveness of capsulodesis in ganglion cysts caused by a medial meniscotibial ligament tear and medial meniscus extrusion.

INTRODUCTION AND IMPORTANCE: Meniscotibial ligament (MTL) has received attention as a major meniscus stabilizer. An MTL injury results in instability and extrusion of the meniscus. Cases of knee ganglion cyst formation due to an MTL tear and medial meniscus extrusion (ME) are extremely rare. CASE PRESENTATION: A 42-year-old female Japanese childcare worker presented painful ganglion cysts on the proximal medial side of her left tibia. An MTL tear and medial ME were thought to be involved in the ganglion formation. Joint fluid flowed through the meniscotibial side of the extruded meniscus into…

The Usefulness of Posterior Shiny Corner Lesions in the Early Diagnosis of Medial Meniscus Posterior Root Tears.

INTRODUCTION: Posterior shiny corner lesions (PSCLs) have been reported to be useful for the early diagnosis of medial meniscus posterior root tears (MMPRTs) in surgical patients. However, the usefulness of PSCLs in outpatients, particularly regarding the optimal timing of magnetic resonance imaging (MRI) examinations after injury, remains unknown. We hypothesized that PSCLs would normally be observed in patients with MMPRTs within one month of injury. MATERIALS AND METHODS: This study included 144 patients with knee pain who visited our hospital between January 2021 and May 2023. MRI finding…

Knee adduction moment is correlated with the increase in medial meniscus extrusion by dynamic ultrasound in knee osteoarthritis.

BACKGROUND: An increase in medial meniscus extrusion (MME) due to abnormal biomechanical stress leads to knee osteoarthritis (OA) progression. MME evaluation during walking is a key method of detecting dynamic changes in the meniscus, and in combination with motion analysis, can provide a deeper understanding of the mechanisms involved in the increase of MME. OBJECTIVE: To validate the feasibility of MME dynamic evaluation in combination with a motion analysis system based on the correlation between the increase in MME and biomechanical factors. METHODS: Twenty-three knees from 23 patients w…
