Megakaryocytic blast crisis at presentation in a pediatric patient with chronic myeloid leukemia.

Patients with chronic myeloid leukemia (CML) infrequently present in blast crisis (BC). While most BC are of myeloid origin, megakaryocytic BC is rare, especially at the time of CML diagnosis. We describe the first pediatric patient presenting with megakaryocytic leukemia and having BCR-ABL1 translocation as the single chromosomal abnormality. Clinical features were more suggestive of CML in megakaryocytic blast crisis than Philadelphia chromosome positive de novo AML. The patient was treated with AML-directed chemotherapy and imatinib mesylate followed by umbilical cord blood stem cell transp

Acquired growth hormone deficiency in a girl with chronic myelogenous leukemia treated with tyrosine kinase inhibitor therapy.

Chronic myelogenous leukemia (CML) is caused by the BCR-ABL1 fusion gene that encodes for a constitutively-active tyrosine kinase. Adults and children with CML are typically treated with imatinib mesylate, a BCR-ABL1 tyrosine kinase inhibitor (TKI), or a second-generation TKI. Several case reports have documented growth delay of unknown mechanism in children with CML treated with imatinib. We report a seven-year-old identical twin with CML who developed significant growth delay, as compared to her twin, during five years of TKI therapy. Detailed endocrine evaluation showed acquired growth horm

MicroRNA expression profiling in Imatinib-resistant Chronic Myeloid Leukemia patients without clinically significant ABL1-mutations.

The development of Imatinib Mesylate (IM), the first specific inhibitor of BCR-ABL1, has had a major impact in patients with Chronic Myeloid Leukemia (CML), establishing IM as the standard therapy for CML. Despite the clinical success obtained with the use of IM, primary resistance to IM and molecular evidence of persistent disease has been observed in 20-25% of IM treated patients. The existence of second generation TK inhibitors, which are effective in patients with IM resistance, makes identification of predictors of resistance to IM an important goal in CML. In this study, we have identifi

Pathological fracture as a manifestation of extramedullary blastic crisis in chronic myelogenous leukemia: report of one case.

A three-year-old girl with chronic myelogenous leukemia (CML) experienced a pathological fracture of her femur after a demonstrated osteolytic bone lesion. Extramedullary disease (EMD) was diagnosed following the histologic findings of a biopsy of the osteolytic lesion. Frank blast crisis in the bone marrow followed one month later. This was the youngest patient to have been reported in English literature of Philadelphia chromosome positive (Ph+) CML with isolated bony EMD and pathological fracture. Treatment with a tyrosine kinase inhibitor, imatinib mesylate (Gleevec), in bone marrow acceler

Monitoring bcr-abl by polymerase chain reaction in the treatment of chronic myeloid leukemia.

The elucidation of the molecular biology of chronic myeloid leukemia (CML) has provided a paradigm for understanding leukemogenesis, targeted drug development, and disease monitoring at the molecular level. Minimal residual disease (MRD) monitoring by fluorescence in situ hybridization and polymerase chain reaction (PCR) has become an important tool in predicting relapse after allogeneic transplant, allowing for early intervention strategies such as donor lymphocyte infusion. MRD monitoring is important for assessment of disease status in patients who obtain a complete cytogenetic remission, a

AgOTf-Assisted β-Glucuronidation Enables Convergent Synthesis of β-(1 → 4)-Linked Polyglucuronates up to the 32-Mer.

β-Glucuronides are widespread motifs in biologically important glycoconjugates and oligo/polysaccharides. However, the C-5 carboxylate of glucuronic acid renders glucuronosyl donors poor electrophiles and glucuronosyl acceptors weak nucleophiles, often leading to unproductive glycosylation. We disclose a robust, general gold(I)-catalyzed β-glucuronidation using glucuronosyl 8-alkynyl-1-naphthoate donors with the assistance of AgOTf catalysis. Mechanistic studies reveal the role of AgOTf: it promotes orthoester-to-β-glycoside conversion, likely through trace acid generated from the interaction

A stereoselective protocol for the synthesis of a molecular cinquefoil (51) knot.

Here, we present a protocol for the complete stereoselective synthesis of a molecular 51 knot. Enantiopure chiral ligands serve as the starting point, while Zn(OTf)2 acts as the template, facilitating the quantitative formation of pentameric circular helicates with 100% d.e. A subsequent sequence of ring-closing metathesis and demetalation steps transforms the structure into a fully organic 51 knot. This protocol expands the scope of strategies employed for chiral knot preparation and paves the way for more complex molecular topologies. For complete details on the use and execution of this pro

DIDMH in combination with triflic acid - A new promoter system for thioglycoside glycosyl donors.

We have explored the possibility of using 1,3-diiodo-5,5-dimethylhydantoin (DIDMH) as an alternative to N-iodosuccinimide (NIS) for activation of glycosyl donors of the thioglycoside type in various glycosylation reactions. DIDMH was found to match NIS when it comes to the capability to activate thioglycosides and provide glycosylation products in good yields. Notably, with the two equivalents of reactive iodonium ions per molecule of DIDMH less mass needs to be added making this activator a more atom economically alternative to NIS. Furthermore, DIDMH was found to be stable upon storage for w

Foveal photoreceptors loss and then recovery after treatment in a chronic myelogenous leukemia patient.

We report a 10-year-old boy with chronic myelogenous leukemia (CML)-related retinopathy of the eyes. Foveal photoreceptors loss was noted in the right eye, but it was restored with a continued ellipsoid zone after systemic 6-week imatinib mesylate and hydroxyurea treatment. Spectral-domain optical coherence tomography images of the foveal photoreceptors change in the right eye were taken. His best-corrected visual acuity of the right eye recovered from 20/100 to 20/20. Prompt treatment of the underlying CML could result in improvement or resolution of the ocular findings, and even foveal photo

Therapeutics for Equine Endocrine Disorders.

Equine endocrine disease is commonly encountered by equine practitioners. Pituitary pars intermedia dysfunction (PPID) and equine metabolic syndrome (EMS) predominate. The most logical therapeutic approach in PPID uses dopamine agonists; pergolide mesylate is the most common. Bromocryptine and cabergoline are alternative drugs with similar actions. Drugs from other classes have a poor evidence basis, although cyproheptadine and trilostane might be considered. EMS requires management changes as the primary approach; reasonable justification for use of drugs such as levothyroxine and metformin m

Imatinib-induced Stevens-Johnsons syndrome.

Imatinib mesylate is a tyrosine kinase inhibitor used widely as the first-line treatment for chronic myeloid leukaemia (CML). The side-effect profile of this drug includes fluid retention, muscle cramps, diarrhoea, myelosuppression and skin rashes. Of these, rashes of the type maculo-papular eruptions and oedema developed most commonly. The cutaneous adverse reactions other than maculo-papular eruptions are rare with imatinib. Severe and life-threatening cutaneous reactions can occur in 5% cases. Here, the author reports a case of newly diagnosed CML that developed Steven-Johnsons syndrome due

The rise and fall of gatekeeper mutations? The BCR-ABL1 T315I paradigm.

The use of tyrosine kinase inhibitors (TKIs) has become an integral component of cancer therapy. Imatinib mesylate, a breakpoint cluster region-Abelson BCR-ABL1 inhibitor, was the first TKI approved in cancer medicine and has served as a model for the development of similar agents for other cancers. An important drawback of TKI therapy is the development of resistance, frequently through the acquisition of mutations. Mutations at the gatekeeper residues of BCR-ABL1 (eg, the threonine-to-isoleucine mutation at codon 315) and other oncogenic kinases have proven highly resistant to currently avai

[Molecular mechanism of imatinib-induced thrombocytopenia in treatment of patients with CML].

Imatinib mesylate has been commonly used in the treatment of patients with chronic myeloid leukemia (CML). However, a significant number of CML patients treated with imatinib developed thrombocytopenia. Platelet-derived growth factor (PDGF)/platelet-derived growth factor receptor (PDGFR) plays a significant role in the regulation of thrombopoiesis. It is suggested that imatinib may block the PDGF/PDGFR and PI3-K/Akt pathway, then inducing the apoptosis of megakaryocytes and developing thrombocytopenia in these patients. In this review, the potential molecular mechanism of imatinib-induced thro

Applying cytogenetic and molecular information in the clinic: implications for the treatment of chronic myeloid leukemia.

The tyrosine kinase inhibitor (TKI) imatinib mesylate has changed dramatically the outcome of patients with chronic myeloid leukemia (CML). Most patients achieve a complete cytogenetic response (CCyR), and many also achieve profound molecular responses. These excellent clinical responses translate into very favorable long-term outcomes. The prognostic impact of achieving cytogenetic and molecular responses at specific time points on response duration and survival has made it even more important to appropriately monitor TKI response in patients with CML. To this end, sophisticated molecular tec

Intraoral granulocytic sarcoma presenting as multiple maxillary and mandibular masses: a case report and literature review.

Granulocytic sarcoma (GS) is an unusual localized tumor composed of immature granulocytic precursor cells that occurs in extramedullary sites. However, GS involving the oral cavity is rare. We report a case of intraoral GS with an unusual clinical presentation, including a history of chronic myelogenous leukemia in remission, multiple maxillary and mandibular gingival masses mimicking acute inflammation that developed over a short period, complete remission after 1 week of treatment with imatinib mesylate (Gleevec), and no bone marrow or peripheral blood involvement over a 6-month follow-up pe

Severe cardiac dysrhythmia in patients using bromocriptine postpartum.

Used worldwide since 1980 for the prevention of breast engorgement in the puerperium, in 1994 bromocriptine mesylate was withdrawn from the American market as an agent suitable for ablactation. The relevant recommendation of the Food and Drug Administration rested on case reports that described severe vasospastic reactions among users of the drug. Some patients so affected suffered stroke, intracranial bleeding, cerebral edema, convulsions, myocardial infarction, and puerperal psychosis. More recently, it has been suggested that the side effects of the drug may also include circulatory collaps

Pyoderma gangrenosum-like necrotizing panniculitis associated with Imatinib: A case report.

Imatinib mesylate is a small tyrosine kinase inhibitor that targets BCR-ABL, ckit and platelet-derived growth factor receptor. It is prescribed by hematologists for chronic myeloid leukemia and acute lymphoblastic leukemia and by oncologists for Gastrointestinal Stromal Tumors (GIST). Cutaneous reactions to Imatinib are common but their incidence and severity widely varies between patients. A self-limited skin rash is the most common adverse effect but there have been reported cases of patients with maculopapular rash, pigmentary changes, superficial edema and rarer and clinically distinctive

Mechanisms of resistance to BCR-ABL kinase inhibitors.

Since the introduction of imatinib mesylate (IM) for the treatment of chronic myeloid leukemia (CML), impressive clinical responses have been observed in the majority of patients in chronic phase. However, not all patients experience an optimal response to IM or even to the more potent, second-generation tyrosine kinase inhibitors (TKIs). Furthermore, responses are not sustained in a number of patients, and it is yet unclear whether the inhibitors can be safely discontinued in patients who achieve long-term remission. The emergence of resistance to TKIs has become a significant problem that ha
