A patient with erythema on the dorsum of the left foot after the removal of gypsum fixation applied for the treatment of bone fracture is reported. Both the clinical and histopathologic features led to the diagnosis of mucor infection.

In two retrospective studies we investigated the additional information provided by follow-up radiographs during the conservative treatment of fractures of the fourth or fifth metacarpal. In the first study we evaluated the number of follow-up radiographs made and their actual influence on the treatment. In 307 consecutive cases, 0-9 (mean 1.93) follow-up radiographs were made. Two cases (0.6%) revealed additional information influencing the treatment policy. Based on physical examination, this alteration was already expected prior to taking the radiograph. In a complementary radiological stud

We report a technique of reducing and stabilizing osteochondral fractures of the metacarpals and phalanges, using the principle that the space within a joint is only "potential." Accurate reduction of the fragment and accurate approximation of the capsular rent effectively traps and holds the fragment without the need for internal fixation. Preservation of all soft tissue attachment to the fragments and early supervised mobilization ensure healing of both the bony and hyaline cartilage components.

Immobilization bone loss, whether due to whole body immobilization or local causes, is associated with an initial rapid phase of trabecular bone loss, but the long-term effects of immobilization on cortical bone are not well described. We have studied metacarpal morphometry in 16 men who had undergone partial or complete traumatic digital amputations 4-71 years earlier. Noninvolved metacarpals from the affected and unaffected hands were used as controls. Cortical bone width was significantly reduced in the metacarpals proximal to the amputated digits (P = 0.001). In the 7 subjects who suffered

A 15-year-old black girl was admitted to St. Jude Children's Research Hospital with a lytic lesion involving the right fourth metacarpal bone with a solitary metastasis to the ipsilateral axillary lymph node. Histologic examination of both the primary lesion and metastasis revealed a primitive small round cell lesion. Abundant rosettes with central lumina were present in the metastasis. Immunohistochemical stains performed on sections of the metastasis were strongly positive for glial fibrillary acidic protein (GFAP) and negative for neuron-specific enolase (NSE), S-100 protein, and Leu-7. The effects of excess dietary calcium on bone growth were quantified in 44 ewes fed ad libitum diets that contained one of four levels of dietary calcium (0.37, 0.56, 1.13 and 1.80%) and 0.42% phosphorus. Animals were slaughtered at one of six ages, circulating concentrations of hormones and minerals were measured and bone morphometry was evaluated. None of the diets impaired normal bone growth of the metacarpal as evaluated by overall length, cortical index and growth plate width. Circulating plasma concentrations of calcium and gastrin increased as dietary calcium levels increased, but all v

Radiographs have an essential role in the treatment of the injured hand. Positioning for routine views must be standardized so that pathology is not missed. When physical findings indicate, special views that demonstrate areas not seen on routine views should be obtained.

One hundred and twenty-six cases of symbrachydactyly were analyzed roentgenographically and clinically. This curious anomaly of the hand is sporadic and of unknown etiology. It is predominantly one-sided, either isolated or combined with ipsilateral pectoral muscle defect (fifty-one cases). On X-ray pictures, the skeletal abnormalities of the hands ranged from mild involvement featured with hypoplastic or aplastic middle phalanges to the most severe involvement in which all the phalanges were missing, providing a continuous spectrum. Even in the mildly affected hands hypoplasia involves all th

When splitting a metacarpal pulley, a diagonal incision allows, after sliding its margins and suturing them, to restore a pulley larger than before. With a small counter incision, it is possible to have a yet larger one. It is a very proper way to treat a "trigger finger". A similar technique may be used in flexor tendons lacerations, which make possible the suture of both tendons. This technique permits the tenolysis with pulley reconstruction.

The 27 cases of skeletal tuberculosis in the hand and wrist reported represented 6.9% of the skeletal tuberculosis seen at our hospital and 32% of the skeletal tuberculosis found in the upper extremity. In a majority of these patients a single bone or joint was involved and 18.5% of the patients had bone and/or joint involvement outside the hand. Pulmonary tuberculosis was present in 10.7% of these cases. The clinical picture of tuberculosis is similar to that of other infections and tumorous conditions, but a history of absent bacille Calmette Guérin protection and positive tine test should a

A retrospective study of 60 patients in whom 80 hands demonstrated a congenital central deficity showed that 23% also had central foot deficits and 15% had a family history of similar hands. The hands are classified based on absence of one, two, or three central rays. The long ray is most affected, followed by the index and ring rays. Many also had lesser deficiencies in border rays. Surgery was performed on the central or border ray structures in 58% of the hands. Function of the hand cannot be predicted from its anatomic state. These patients did not consider their central deficits as a soci

PURPOSE: To report on 4 cases of avascular necrosis of the metacarpal head.
METHODS: We retrospectively reviewed 4 patients who received a diagnosis of avascular necrosis of the metacarpal head and were treated from 2000 to 2016.
RESULTS: All patients were males with involvement of the dominant hand. Three patients had a history of trauma and/or fractures in another finger and one had a history of fracture in the same finger. The diagnosis was confirmed on regular x-rays and magnetic resonance imaging. Nonsurgical management was offered to all patients (rest, placement of an orthosis, and no

We report the use of low-field standing magnetic resonance imaging in the standing horse for the diagnosis of osseous lesions in the metacarpophalangeal (MCP) or metatarsophalangeal (MTP) joint that were not apparent using standard radiography. Thirteen horses were studied and all had thickening of the subchondral bone plate and abnormal signal intensity in the adjacent spongiosa in either the condyles of metacarpal/metatarsal III or the proximal phalanx or both. Abnormalities were characterized by diffuse decreased signal intensity on T1-weighting adjacent to the subchondral bone and within t

Fractures of the base of the metacarpals are usually treated conservatively. The intra-articular fracture of the base of the first metacarpal ('Bennett fracture') is an exception to this rule because inadequate repositioning and fixation of the dislocated radial fragment lead to permanent deformity of the joint and subsequent degenerative joint disease. The dislocated intra-articular fracture of the base of the fifth metacarpal is similar to a Bennett fracture in many aspects. Repositioning of this 'mirrored' Bennett fracture cannot be guaranteed by a plaster cast. Inadequate repositioning wil
