Assessing penalized approaches for estimating causal treatment effects under extremely limited overlap in oncology.

BACKGROUND: Overlap weighting (OW) is increasingly used to estimate treatment effects in observational cancer studies. OW has attractive features: it targets the clinical equipoise population and mitigates the influence of extreme propensity score (PS) weights. Additionally, under regularity conditions, when the PS model is fitted using a standard logistic regression model (LRM) with all observed covariates included, OW achieves exact covariate balance between treated and control groups, meaning that the standardized mean differences for the included covariates are zero. However, in oncology d

Flexible yet Sparse Bayesian Survival Models With Time-Varying Coefficients and Unobserved Heterogeneity.

Survival analysis is an important area of medical research, yet existing models often struggle to balance simplicity with flexibility. Simple models require minimal adjustments but come with strong assumptions, while more flexible models require significant input and tuning from researchers. We present a survival model using a Bayesian hierarchical shrinkage method that automatically determines whether each covariate should be treated as static, time-varying, or excluded altogether. This approach strikes a balance between simplicity and flexibility, minimizes the need for tuning, and naturally

Network-based multi-class classifier to identify optimized gene networks for acute leukemia cell line classification.

Unraveling the genetic regulatory networks that underlie diseases is essential for comprehending the intricate mechanisms of these conditions. While various computational strategies were developed, the approaches in the existing studies concerning network-based prediction and classification are based on the pre-estimated gene networks. However, the gene network that is pre-estimated fails to yield biologically meaningful explanations for classifying cell lines into particular clinical states. The reason for this limitation is the lack of inclusion of any information about the clinical status o

Accurate prediction of gene deletion phenotypes with Flux Cone Learning.

Understanding the impact of gene deletions is crucial for biological discovery, biomedicine, and biotechnology. Due to the complexity of genome-wide deletion screens, there is growing interest in computational methods that leverage existing screening data for predictive modeling. Here, we present Flux Cone Learning, a general framework designed to predict the effects of metabolic gene deletions on cellular phenotypes. Using Monte Carlo sampling and supervised learning, our approach identifies correlations between the geometry of the metabolic space and experimental fitness scores from deletion

Noise power properties of a cone-beam CT scanner with unconventional scanning geometry.

PURPOSE: This work aims at investigating, via in-silico evaluations, the noise properties of an innovative scanning geometry in cone-beam CT (CBCT): eCT. This scanning geometry substitutes each of the projections in CBCT with a series of collimated projections acquired over an oscillating scanning trajectory. The analysis focused on the impact of the number of the projections per period (PP) on the noise characteristics. METHODS: In-silico eCT scanner was simulated with a GPU based Monte Carlo software. We employed two homogeneous PMMA phantoms with a diameter of 12 cm and 16 cm whose tomogra

Design optimization and simulated performance evaluation of a self-collimated cardiac SPECT with simultaneous high sensitivity and resolution.

BACKGROUND: Cardiac single photon emission computed tomography (SPECT) is an important noninvasive molecular imaging technology for the diagnosis and risk stratification of heart diseases. However, since its inception, the clinical impact of cardiac SPECT has been constrained by its reliance on mechanical collimation using a metal apertured-plate structure, which causes an inherent inverse interdependency between the system's resolution and sensitivity. Recently, our group introduced self-collimation (SC) detector architecture in single photon emission imaging: in a structure with multiple lay

Automatic, machine-agnostic, convolution-based beam, and fluence modeling for Monte Carlo independent dose calculation.

BACKGROUND: Monte Carlo (MC)-based independent dose calculation is increasingly sought after for plan- and delivery-specific quality assurance (QA) in modern radiotherapy because of its high accuracy. It is particularly valuable for online adaptive radiotherapy, where measurement-based QA solutions are impractical. However, challenges related to beam modeling, commissioning, and plan/delivery-specific fluence calculation have hindered its widespread clinical adoption. PURPOSE: We propose a generic, automated, convolution-based beam and fluence modeling method for MC dose calculation, assuming

Comparative assessment of sewer sampling methods for infectious disease surveillance: Insights from transport modeling and simulations of SARS-CoV-2 emissions.

Emerging infectious diseases like COVID-19 present significant public health challenges, necessitating effective surveillance methods. Wastewater-based epidemiology (WBE), detecting viral pathogens in wastewater, has emerged as a proactive tool for monitoring infections. This study evaluated various wastewter sampling methods through SARS-CoV-2 transport simulations in an urban sewer network in Sejong City, South Korea, to identify cost-effective strategies for accurate infection monitoring. Using the U.S. EPA's Storm Water Management Model (SWMM) and Markov chain Monte Carlo (MCMC) sampling,

Global Burden of Gallbladder Cancer Attributable to Modifiable Risk Factors.

Gallbladder cancer (GBC) is a highly lethal and often overlooked malignancy increasingly affecting young adults. This study quantified the global proportion of GBC cases attributable to 10 key modifiable risk factors, employing Monte Carlo simulations and estimates from field-wide systematic review and meta-analysis. Approximately three-quarters of global GBC cases are attributable to key modifiable factors (74.6%; 95% uncertainty interval, 63.7-83.7), with a slightly higher share observed in females. Central obesity (29.7%; 8.7-49.5), gallstones (27.9%; 16.8-40.7), and physical inactivity (20

Monte Carlo Tree Search for optimal cancer intervention strategies among BRCA mutation carriers.

Breast and ovarian cancers are the second and fifth leading causes of cancer-related death among women in the United States. Compared to non-carriers, BRCA mutation carriers are subject to a higher risk of developing breast and ovarian cancers. Prophylactic surgeries including prophylactic mastectomy and prophylactic bilateral salpingo-oophorectomy can significantly reduce the incidence risks of breast and ovarian cancers for BRCA mutation carriers. However, both prophylactic surgeries are permanent one-time interventions. Determining the optimal age for BRCA carriers to undergo such surgeries

Absorbed dose differences between twin fetuses for pregnancy patients in CT examinations.

BACKGROUND: Estimation of the radiation dose to the fetus is essential for the assessment of radiation risks and benefits to pregnant patients undergoing radiological examinations. During the past decade, the global twinning rate has soared resulting from embryo assistance and increased delivery age. However, to the best of our knowledge, radiation dosimetry in multiple pregnancies from radiological imaging has never been reported before. PURPOSE: The purpose of this study is to develop personalized computational models for twin fetuses based on clinical CT images of real pregnant patients an

This study presents the design and validation of a neonatal head phantom using innovative heterogeneous composite materials customized to replicate the x-ray attenuation properties of neonatal cranial structures. Analysis of Hounsfield Unit (HU) data from 338 neonatal head CT scans informed the design of epoxy resin-based composites with additives such as sodium bicarbonate, fumed silica, and acetone to simulate bone, brain matter, cerebrospinal fluid (CSF) and hyperdense abnormalities. The cranial bone substitute (60% epoxy resin, 40% sodium bicarbonate) achieved a density of 1.60 g cm-3, wit

Modeling the economic impact of different testing strategies for meningitis/encephalitis in adult patients from a US hospital perspective.

INTRODUCTION: Meningitis and encephalitis (ME) are life-threatening conditions whose severity vary by etiology. Often, patients with suspected ME are unnecessarily hospitalized and treated with empiric therapy while clinicians determine etiology. ME diagnosis and treatment algorithms vary across hospitals, with some using a battery of rapid diagnostic tests (RDTs) or multiplex/syndromic testing, and others relying only on bacterial culture and send-out tests. AIM: We modeled the economic impact of 4 ME testing strategies from a US hospital perspective. METHODS: A synthetic sample of 20,000 s

Estimating the burden of mpox among MSM in South Africa.

Despite seeing few cases during the 2022-2023 mpox global outbreak, reports in May-July 2024 of 22 cases among men-who-have-sex-with-men (MSM) in South Africa, including three deaths, have raised concerns about under-reported community transmission. We used a Monte Carlo simulation model to estimate the true epidemic size, considering the increased severity of mpox among MSM living with advanced HIV, documented over-representation of people living with HIV among mpox cases and HIV prevalence in South Africa. We estimate that there have been between 220 and 450 cases among MSM LHIV in South Afr

Preclinical pharmacokinetics/pharmacodynamics studies defining the role of ethambutol in Mycobacterium kansasii lung disease.

Rifampin, isoniazid, and ethambutol are the backbone of the regimen used to treat Mycobacterium kansasii-complex (MKC) lung disease. However, ethambutol pharmacokinetics/pharmacodynamics (PK/PD) studies to inform on optimal exposure target and clinical dose for MKC are lacking. We performed studies to determine ethambutol minimum inhibitory concentration (MIC), mutation frequency (3× MIC), a PK/PD study using the hollow fiber system model of MKC (HFS-MKC) using the reference ATCC#12478 strain, and Monte Carlo simulation experiments for clinical dose selection and susceptibility breakpoint. Adapting Back-calculation Methods to Estimate the Incidence of Tuberculosis.

BACKGROUND: Despite being the leading cause of death, the global tuberculosis (TB) burden is ill-defined. Existing methods to estimate incidence are time and/or resource-intensive and often inaccurate. Back-calculation was developed to estimate HIV incidence by considering reported cases to be a convolution of the disease duration and the incidence of new cases. New estimates of TB natural history parameters allow us to develop Bayesian back-calculation methods for TB to assign case notification data to the time point of onset of disease. METHODS: Recorded counts of TB cases are underestimate
