Patients with episodic or paroxysmal movements or postures often are thought to have hysteric or psychosomatic illnesses. Kinesigenic (movement-induced) posturing similarly is usually misinterpreted. This case is notable because of the presence of symptoms of two distinct diseases with similar symptoms and changes from one dystonic posture to another during three different durations of attack. The condition improved with acetazolamide therapy. The effect of acetazolamide on sodium and potassium ionophores is discussed because of new genetic information about these illnesses.

We measured free choline in cerebrospinal fluid (CSF) of 78 patients with movement disorders of paediatric onset and various controls as a putative index of central phospholipid metabolism. Most of the disorders studied were myoclonic disorders, such as progressive myoclonus epilepsy, the opsoclonus-myoclonus syndrome, and essential myoclonus, but other movement disorders, interictal seizure disorders, and different neurological and nonneurological disorders were also included. There were no significant differences in CSF choline concentrations in myoclonic disorders or other movement disorder…

Anterior cerebral artery territory infarction, which affects the medial frontal lobe and anterior part of the corpus callosum, may cause disinhibitory movement and behavioral disorders. The pathophysiology of disinhibitory movement and behavioral disorders was discussed in relation to the role of the frontal lobe in motor control and hemispheric asymmetry of motor functions. Instinctive grasp reaction is the prototype of disjunctive motor disinhibition syndromes, and diagnostic dyspraxia and compulsive manipulation of tools are the most illustrative forms. Diagnostic dyspraxia can be defined a

Movement disorders (bemichorea-hemiballismus, hemidystonia and isolated tremor) are an uncommon clinical manifestation in ischemic stroke (IS), and their anatomical basis is poorly understood. We analyzed the clinical and neuroimaging characteristics of 22 consecutive patients who bad movement disorders associated with cerebral infarction (MDCI), studied at four institutions over 8 years. In one institution (from the La Alianza-Central Hospital of Barcelona Stroke Registry) nine patients with MDCI were identified among 1099 consecutive first ever stroke patients (0.8%) (908 with IS, 1%). Fifte…

Eye movements were recorded electro-oculographically in three patients with a small ischemic lesion affecting the left frontal eye field (FEF) and in 12 control subjects. Reflexive visually guided saccades (gap and overlap tasks), antisaccades, predictive saccades, memory-guided saccades, smooth pursuit and optokinetic nystagmus (OKN) were studied in the three patients. Staircase saccades and double step saccades were also studied in one of the three patients. For both leftward and rightward saccades, latency in the overlap task (but not in the gap task) and that of correct antisaccades and of

Reports of 62 cases with a movement disorder associated with a focal lesion in the thalamus and/or subthalamic region were analyzed. Thirty-three cases had a lesion confined to the thalamus. Sixteen cases had a thalamic lesion extending into the subthalamic region and/or midbrain. Thirteen cases had a lesion in the subthalamic region or a subthalamic lesion extending into the midbrain. Nineteen cases with dystonia, 18 with asterixis, 17 with ballism-chorea, three with paroxysmal dystonia, and five with clonic or myorhythmic movements have been described. No case with isolated tremor has been d

Hyperkinetic movement disorders may develop as a complication of stereotactic thalamotomy or pallidotomy. However, such movement disorders are uncommon after nonsterotactic intracranial operations. The authors report three cases of involuntary movement disorders unexpectedly developing after intracranial operations. The patients had undergone clipping of an internal carotid aneurysm, removal of an intracerebral hematoma, and resection of a tentorial meningioma. Two patients developed choreic movements and a dystonic posture of the unilateral upper extremity. One patient showed a tremor that ha…

Electromyography of the larynx has been increasingly used during recent years in the diagnosis and differential diagnosis of laryngeal motility disorders. The method is routinely employed at the university ENT department in Göttingen. The qualitative experience gained with regard to the diagnostic and prognostic value of electromyography in the region of the larynx is described and compared with the results and experience reported so far in the literature.

The corticomotoneurone pathways were examined in 21 patients with movement disorders, using the technique of percutaneous electrical stimulation of the motor cortex. Conduction in these pathways was assessed by measuring the latency to onset of electromyographic activity in the muscles of the upper limb after cortical stimulation. In all patients [five with primary (idiopathic) torsion dystonia and two with secondary (symptomatic) hemidystonia, seven with Huntington's disease, four with essential tremor, and three with Parkinson's disease] central motor conduction was normal. This and other ev…

The aim of this study was to evaluate, in an acutely brain-damaged population, the occurrence of motor impersistence and its relation to the side of lesion. All the patients hospitalized in a 10-month period for a stroke involving one hemisphere and able to cooperate were given, in the early days of illness, a motor impersistence battery requiring the maintenance of a position of eyelids, gaze, mouth, and limbs for 10 seconds. A control group of 50 patients without brain damage provided the basis for evaluating pathologic performance. Failure to keep eyes closed was the most common manifestati

Twelve patients with abnormal involuntary movement disorders were treated with clozapine in a double-blind, placebo-controlled trial. The cohort consisted of individuals with Gilles de la Tourette's syndrome, Huntington's disease, and atypical persistent dyskinesia that was drug induced. Two subjects were dropped from the protocol due to complications. Two patients with Huntington's disease showed a marked decrease in movements; other individuals obtained no significant therapeutic benefits. Seven of the 10 patients completing the trial experienced moderate or marked side effects.

BACKGROUND: Drug-related movement disorders (DRMDs) reduce quality of life and contribute to medication noncompliance of patients with psychotic disorders. Little is known about the epidemiology of DRMDs in relatively young patients a few years after onset of psychosis. This is an important period to study, as the impact of the antipsychotic treatment on the long-term potentiation of the neural pathways associated with psychotic disorders and DRMDs is still minimal. This study investigated the prevalence, incidence, persistence, and clinical correlates of DRMDs in patients during their first y…

New wearable technology opens new possibilities for low-cost, easily accessible home-based interventions as a supplement to typical clinical rehabilitation therapy. In this pilot study, we tested a new interactive adjustable Feedback training system on 14 infants at high risk of cerebral palsy between 2 and 12 months of age to facilitate increased movements. The system consists of four wireless motion sensors placed on the infant's limbs. Inertial sensors track the infant's movements which control auditory and visual stimuli that act as motivational feedback. A 15 min usage of the Feedback tra

PURPOSE: Studies of eye movements have evolved from clinical phenomenology to a probe of neural circuitry, cognition, and behavior offering a powerful, noninvasive window into nervous system function. In Parkinson's disease (PD), ocular motor control has emerged as a uniquely informative model system, reflecting dysfunction across motor, cognitive, and executive domains.
RECENT FINDINGS: Recent advances span four interconnected areas: executive behavior and cognition, gait and freezing of gait (FOG), neuromodulation, and machine learning. Antisaccades and related paradigms reveal early and pr

Belly dancer's dyskinesia (BDD) is a rare movement disorder marked by involuntary, rhythmic, undulating movements of the abdominal wall, often mistaken for gastrointestinal or psychiatric conditions. We describe the case of a 13-year-old girl with a seven-year history of abdominal spasms that stopped during sleep and resumed on waking. The episodes were accompanied by loud "whooshing" sounds from the abdomen. Her medical history included primary ciliary dyskinesia and a ventricular septal defect. She had never received medication for her symptoms. On examination, the right side of her upper bo
