LRP4-Related Congenital Myasthenic Syndrome: Clinical, Pathophysiological, and Therapeutic Aspects.

Congenital Myasthenic Syndrome represents a complex and heterogeneous group of inherited neuromuscular disorders, which result from variants in genes involved in different pathophysiological mechanisms related to the neuromuscular junction. LRP4 (Low-density lipoprotein receptor-related protein 4) represents one of the most important proteins involved in this complex signaling pathway, acting in a complex with agrin and Muscle Skeletal Receptor Tyrosine Kinase (MuSK) proteins. LRP4 became known to most neurologists due to the description of anti-LRP4 antibody-related Myasthenia Gravis. There a…

Myasthenia gravis with dysphagia as the first symptom and misdiagnosed as anxiety-depressive state: a case report.

BACKGROUND: Myasthenia gravis (MG) is a rare autoimmune disorder that affects the neuromuscular junction, with a global prevalence of approximately 30 cases per 100,000 individuals. Although dysphagia occurs as an initial symptom in 6%-15% of patients, isolated dysphagia as the sole presenting and predominant manifestation is exceedingly rare, posing a significant diagnostic challenge. Furthermore, a complex bidirectional relationship exists between dysphagia and psychiatric conditions such as anxiety and depression, which often leads to misdiagnosis. Approximately 20% of patients with MG are …

Mismatch Between the Distribution of Neuromuscular Specialists and Myasthenia Gravis Patients in the US Medicare Fee-for-Service Population.

INTRODUCTION/AIMS: There is a mismatch between clinical need and access to neurologists across the US. Myasthenia gravis (MG) incidence and prevalence are increasing, particularly in US patients older than 65 years, raising concern for access to neuromuscular specialists that represent only 4% of neurologists. We sought to compare the prevalence of MG patients over 65 years of age to the number of board-certified neuromuscular physicians (BCNMP) across the US. METHODS: Utilizing Medicare fee-for-service Parts A/B coverage non-HMO claims data, MG cases were ascertained using a validated algori…

Bisphenol A exposure in myasthenia gravis: Potential targets and mechanisms revealed by network toxicology and molecular dynamics.

BACKGROUND: Myasthenia gravis (MG) is a B-cell-mediated autoimmune disease characterized by impaired neuromuscular transmission. Although genetic predisposition and thymic abnormalities are well recognized, they cannot fully explain the increasing incidence and regional heterogeneity of MG, highlighting the potential contribution of environmental factors. Bisphenol A (BPA), a ubiquitous endocrine-disrupting chemical, exhibits estrogenic activity, immunomodulatory effects, and mitochondrial toxicity, and has been implicated in multiple autoimmune disorders. However, the potential role of BPA in…

High risk of hypoxemic COVID-19 pneumonia in myasthenia gravis patients with type I IFN autoantibodies.

Patients with myasthenia gravis (MG) may produce autoantibodies neutralizing type I interferons (AAN-I-IFN), which underlie severe viral diseases, including critical COVID-19 pneumonia, in patients without MG. We studied an international cohort of 85 unvaccinated SARS-CoV-2-infected MG patients not given antiviral treatment. Hypoxemic pneumonia occurred in 48 of these patients, including 22 (45.8%) with AAN-I-IFN, which neutralized both IFN-α2 and IFN-ω in 14 (29.2%) patients. Six (16.2%) of the remaining 37 patients had AAN-I-IFN, neutralizing both IFN-α2 and IFN-ω in three patients. The risk…

Identification of PTGS2 as a genetically supported protective biomarker and potential therapeutic target in myasthenia gravis.

BACKGROUND: Myasthenia Gravis (MG) is an autoimmune disease that damages the neuromuscular junction (NMJ), reduces the transmission of nerve impulses to muscles, and thus causes fluctuating muscle weakness and fatigue. The main types of MG are autoantibodies that target necessary components of the postsynaptic membrane, such as acetylcholine receptors (AChRs) and muscle-specific kinase (MuSK). The above immune-mediated alterations disrupt synaptic transmission and reduce muscle contraction. Study the molecular and cellular mechanisms of MG to find genes that regulate the immune system, cause i…

Accuracy of Antibody Testing in Myasthenia Gravis: The ADAM Study.

BACKGROUND AND OBJECTIVES: Antibody testing has a pivotal role in the diagnosis of myasthenia gravis (MG). However, there are limited data on the accuracy of the different methods available for acetylcholine receptor (AChR) and muscle-specific kinase (MuSK) autoantibody (Ab) detection in a real-world setting. Our aim was to compare the diagnostic accuracy of in-house live cell-based assay (L-CBA), a commercially available fixed CBA (F-CBA), and an indirect ELISA in detecting AChR and MuSK antibodies in patients with suspected MG. METHODS: Between July 2023 and December 2025, we prospectively …

Safe administration of botulinum neurotoxin type A for facial nerve palsy sequelae in a patient with myasthenia gravis.

Botulinum neurotoxin type A (BoNT-A) is an effective treatment for facial nerve palsy sequelae; however, its use in patients with myasthenia gravis (MG) is generally contraindicated due to the risk of exacerbating neuromuscular blockade. We report a female in her 40 s with a 10-year history of generalized, seronegative MG who presented with facial tightness and mild synkinesis due to facial nerve palsy sequelae. As BoNT-A is contraindicated for MG patients in Japan, we obtained approval for off-label use from the Division of Patient Safety Management in our hospital. Under a predefined risk ma…

MuSK myasthenia gravis: from antigen-antibody to clinical translation.

Anti-MuSK antibody-positive myasthenia gravis (MuSK-MG) is characterized by severe and potentially life-threatening weakness. Anti-MuSK antibodies are key pathogenic drivers of this disorder. This review provides a comprehensive summary of the current state-of-the-art with respect to the understanding of MuSK and its role in MuSK-MG. MuSK, a receptor tyrosine kinase, is essential at the neuromuscular junction, where it forms an agrin-LRP4-MuSK tripartite functional complex that initiates and maintains agrin-induced acetylcholine receptor clustering at the postsynaptic membrane. Although anti-M…

Thymoma Type B2 Associated with Myasthenia Gravis Presenting as Progressive Respiratory Distress: A Case Report.

The association between thymoma and myasthenia gravis (MG) is well established; therefore, the significance of this case does not rest on the coexistence of these conditions alone. Its educational value lies in the diagnostic pathway, in which fluctuating myasthenic symptoms preceded tumor recognition, the initial chest radiograph did not demonstrate an evident mediastinal mass, and contrast-enhanced computed tomography (CECT) subsequently identified a necrotic anterior mediastinal lesion. A 54-year-old woman presented with progressive dyspnea, generalized fatigable weakness, dysphagia, facial…

Myasthenia thymus reprograms class-switched B cells into BAFF-dependent survivors.

Myasthenia gravis (MG) presents a clinical challenge where autoantibody titers poorly predict disease severity, and thymectomy provides inconsistent benefits. We hypothesized that thymic B cells acquire survival mechanisms that bypass canonical tolerance checkpoints, enabling persistence independent of antigen-specific selection. Using single-cell RNA sequencing, V(D)J repertoire analysis, and spatial transcriptomics to generate the largest MG atlas to date (237,661 cells), we identified a tolerance checkpoint swap in MG pathogenesis. Pathological class-switched B cells within thymic germinal …

Association between postoperative peripheral blood CD3+ T-cell percentage and recurrence after thymectomy in non-thymomatous myasthenia gravis: a retrospective cohort study.

Postoperative recurrence remains an important concern in patients with myasthenia gravis (MG) after thymectomy. T-cell abnormalities are involved in MG pathogenesis, but the prognostic value of postoperative peripheral blood CD3+ T-cell percentage in non-thymomatous MG is unclear. We evaluated the association between postoperative peripheral blood CD3+ T-cell percentage and recurrence after thymectomy in patients with non-thymomatous MG. This retrospective cohort study used data from an existing MG thymectomy database. Patients with non-thymomatous MG who had available postoperative peripheral…
